Connecting people to the right supportsBy Allevia’s Support Coordination TeamThe past year has presented us withmany challenges as Allevia’s SupportCoordination team. The CoVID impacthas required creative thinking andresourcefulness in an environment wherepeople needed to change and adapt todifferent ways of getting the servicesthey required. As a team we supportedeach other using online team meetingsto debrief and check in on each other’swellbeing. Support Coordination is ademanding role where you are mostlyworking alone with people at a time ofsignificant change and often in crisis.Resilience, peer support, and sharing ofknowledge and skills has meant Allevia’steam has gone from strength to strength.Sometimes the impact of what we do islife changing!“Reflecting on this past year my workwith a 17-year-old participant with PraderWilli Syndrome stands out as an exampleof why I love my job.Razi (name changed) had been in and outof home care arrangements for much ofthe past year. A crisis occurred when hishealth became at risk due to his increasingweight gain. Razi’s family were strugglingto assist him refrain from compulsiveeating. This resulted in Razi spending 4days in social admission at the Children’sHospital in Randwick followed by timein crisis out of home respite before hecould move into support in short-termaccommodation (STA).My role as Razi’s Support Coordinatorcommenced in September 2020 wherethe priority was finding suitable longtermaccommodation. Everyone wasrelieved when a long-term supportedliving arrangement was found that he andhis family were happy with.Paying attention to detail throughRazi’s transition from the short-termaccommodation to his new home provedto be a successful strategy. We developeda transition plan, monitored the progress,and held collaborative team meetingsas well as liaising with all stakeholdersincluding the NDIS Complex PathwayTeam, and Department of Communitiesand Justice on a regular basis, and Policeas required.This wasn’t an easy process. It was aroller coaster of escalations of behaviours,constant setbacks, and staff burnout.However, my consistent and intensivesupport paid off. Razi has lost a significantamount of weight and his health hasimproved, he has built a good rapportwith his support workers and has startedovernight stays at the family home.The family have been delighted withthe result saying the consistent effort,working hand in hand with them from thebeginning to the end, and the empathyshown has been life changing for their sonand them.”Allevia 2020/21 Annual Report • 29
