for oral management of many types of glaucoma DARANIDE DICHLORPHENAMIDE usually most successful when given in conjunction with miotics * An oral carbonic anhydrase inhibitor which frequently reduces increased intraocular pressure. • Often provides effectiveness over a prolonged period of administration. •Onset of action usually within an hour of administration. • Effective for approximately 6 to 12 hours. • Effective as a means of enhancing the pressure-regulating effects of topical miotic agents, thus facilitating smoother management for the glaucoma patient. Contraindications:To avoid blood pH decreases below 7.20, do not use if arterial pCO2 greater than 63 mm. Hg; hepatic insufficiency; renal failure; hyperchloremic acidosis; adrenocortical insufficiency. Indications: Treatment of chronic simple (wide angle) glaucoma, acute congestive (narrow angle) glaucoma, chronic congestive glaucoma, secondary glaucoma (acute phase), and preoperative control of intraocular tension of glaucoma; respiratory acidosis of pulmonary insufficiency. Use in conjunction with other agents as indicated. Precautions and Side Effects: Side effects (characteristic of carbonic anhydrase inhibitors) may include gastrointestinal disturbances (anorexia, nausea, vomiting), constipation, urinary frequency, mild skin eruptions, pruritus, headache, weakness, nervousness, globus hystericus, sedation, lassitude, depression, confusion, disorientation, dizziness, ataxia, tremor, tinnitus, paresthesias of hands, feet, tongue. If they occur, reduce dosage or discontinue drug temporarily. As with other carbonic anhydrase inhibitors: agranulocytosis, thrombocytopenia, or renal calculi are possible; a close check on electrolyte balance, blood pH, and blood gases should be kept during therapy. In high doses, may cause some decrease in renal blood flow and glomerular filtration rate. Use with caution in severe respiratory acidosis or in the presence of chronic pulmonary infection; discontinue if blood pH falls below 7.20.
