This nomination form must be completely filled out. It must also be accompanied by 2
letters of support along with a resume or biography of the nominee. For the best results
it is suggested that the nominator work with the nominee to ensure that the
nomination form includes all pertinent information.

In order to help the Committee make a decision please tell us
the following:
1. Provide background on nominee’s volunteer work (inside and outside
the community).

2. Provide examples how the nominee promotes unity and strength
within the family and community.
3. Provide examples how the nominee demonstrates the ability to create
change or may be known for their advocacy role on behalf of others.
4. State what the nominee has done to bridge the gap between Natives
and Non-Natives.

5. Provide examples of how this person may have a legacy of helping
others, giving freely of their own time, not for recognition but the benefit of
others.
6. Demonstrate through examples how the nominee instills and/or
promotes the values of respect, honesty, forgiveness and sharing into their
life and others whom they are in contact with.
