Plaquemines Parish Head Start
Confidentiality Policies and Procedures

Plaquemines Parish Head Start is committed to the maintenance and security
of children’s records. All Head Start personnel will receive training in the proper
method of collecting, filing, and maintaining information on children and families.
Head Start staff will also receive training on the confidentiality and security
of all Head Start records.
Plaquemines Parish head Start will maintain two sets of records on each child
enrolled in the program. One set will be kept in a locked file in the Head Start
Office. The other set will be kept in a locked file cabinet at the individual center.
Center staff will have the key to the locked cabinet available at all times.
Each cabinet will be labeled with a list of personnel who will have access to the
files. This list will include, but not be limited to the following: Head Start Staff,
School Principal, School Counselors, Special Education Pupil Appraisal Staff,
School Nurses, and authorized representatives of state and federal agencies,
whose duty it is to review our program.
Parents will be informed that they may have access to their child’s records.
Any authorized person who reviews a file, must complete and sign a form indicating
the date, their name, their title, and the reason for reviewing the file.
Plaquemines Parish Head Start staff will be instructed to the fact that any disclosure
of information about the children or families in our program, or knowingly
to permit said disclosure, to any unauthorized person will be grounds for dismissal.
