Adult Hearing<br />

To provide comprehensive patient-centred direct access adult hearing service for age related<br />

hearing loss in line with national guidance <strong>and</strong> local requirements.<br />

Podiatry<br />

To provide a range of community podiatry services in line with the specification developed<br />

for the national implementation pack, with some minor amendments.<br />

Wheelchair Services<br />

A Single Point of Contact (SPOC) service with traditional online or phone based referral.<br />

Each service user will then be case managed through the pathway dependent on their<br />

level of complexity. In addition to the provision of appropriate services, based on clinical<br />

assessment, service users will be provided with ongoing support.<br />

Diagnostics<br />

Using the published implementation pack service specification with minor amendments.<br />

Adult Hearing<br />

The vision for people with age related hearing problems is for them to receive high quality,<br />

efficient services delivered closer to home, with short waiting times <strong>and</strong> high responsiveness<br />

to the needs of local communities.<br />

Podiatry<br />

High quality podiatric care delivered efficiently <strong>and</strong> cost effectively to increase mobility <strong>and</strong><br />

independence for adults <strong>and</strong> children.<br />

Wheelchair Services<br />

Enhancing quality of life for people with long-term conditions. Helping people to recover from<br />

episodes of ill health or following injury. Ensuring that people have a positive experience of care.<br />

Diagnostics<br />

‘Local direct access services, using equipment <strong>which</strong> complies with the latest national<br />

guidance, with connection to <strong>NHS</strong> image transfer solutions.<br />
