Time Limited Primary Care Counselling<br />
The service will provide high quality, evidence based time limited counselling within primary<br />
care to service users, over the age of 18, identified through assessment by a GP.<br />
Community Ophthalmology<br />
The service will provide high quality, evidence-based, adult opthamolgy assesment <strong>and</strong><br />
treatment in a community setting. It will be consultant-led, offering a triage of ophthalmic<br />
conditions, assessment <strong>and</strong> treatment of specified conditions.<br />
Minor Oral Surgery<br />
The service will provide minor oral surgery in a primary care setting. Patients will be able to<br />
access consultant-led services providing a specific list of treatments that can be done safely<br />
<strong>and</strong> appropriately closer to home in a primary care environment.<br />
Time Limited Primary Care Counselling<br />
A responsive <strong>and</strong> accessible service within primary care to people experiencing common<br />
mental health problems. To deliver NICE-compliant <strong>and</strong> other evidence based counselling for<br />
people suffering from mild to moderate mental health problems <strong>and</strong> result in a reduction in<br />
the number of people being referred to secondary mental health services.<br />
Community Ophthalmology<br />
The service will reduce the number of outpatient visits to a secondary care environment by<br />
appropriately seeing service users in primary care.<br />
Minor Oral Surgery<br />
The service will deliver a robust patient pathway <strong>and</strong> pricing structure <strong>which</strong> demonstrates<br />
value for money. It will provide an appropriate level of quality <strong>and</strong> patient care <strong>which</strong> can be<br />
carried out closer to home in a primary care environment.<br />
