Continuing Healthcare (children) (All cluster PCTs) A single care model is being developed for the delivery of Children and Young People’s Continuing care across NHS North Essex/ South Essex Clusters and the Local Authority. Glaucoma (North East Essex only) The service is for the provision of assessment of suspected Glaucoma (for patients who meet the referral criteria) in the community setting. The service will include initial assessments/ tests appropriate to clinical need, support to patients for self-management and, if required, referral into secondary care. Continued Overleaf 26

Any Qualified Provider offer for 2012 by commissioning organisation Short description of service vision Size of market opportunity PCT Cluster contact detail North Essex PCT Cluster Continued MSK Choice of manual therapy provider, reduced waiting times to access the service and delivery of treatment to enable patients to reach their individual treatment goals sooner, resulting in improved quality of life, earlier return to work and helping patients to manage their pain more effectively. In addition, the aim is to provide community services that have a strong emphasis on patient education and self-management, thereby promoting active, healthy lifestyles and reducing risk of recurrence of injury or illness. Continuing Healthcare (children) To recognise the importance and advantages of joint/partnership working between parents / guardians of the patient, and other agencies to provide the best possible service together for the benefit of the patient. To support the parents/guardians of the patient with complex health needs enabling the patient to live at home with his/her family and within the wider community. This model aims to provide consistency in assessment and delivery of provision to ensure equitable and appropriate resource allocation based on individual need. This service aims to set an equitable, transparent and timely process for assessing, deciding and agreeing bespoke packages of continuing care for those children and young people under the age of 18 who have continuing care needs that cannot be met by existing universal and specialist services alone. Glaucoma The aim of the Glaucoma Referral Refinement pathway in NHS North East Essex is to reduce unnecessary referrals to the hospital eye service, reducing patient anxiety and increasing capacity within the overburdened hospital glaucoma clinics. This will provide a more cost effective service with a greater number of patients managed within the primary care setting. The referral refinement pathway will allow accredited optometrists to repeat diagnostic tests to confirm the risk of disease and thus improve the accuracy of referrals and deflect unnecessary referrals.
