Continuing Healthcare (children)<br />

To recognise the importance <strong>and</strong> advantages of joint/partnership working between parents /<br />

guardians of the patient, <strong>and</strong> other agencies to provide the best possible service together for<br />

the benefit of the patient.<br />

To support the parents/guardians of the patient with complex health needs enabling the<br />

patient to live at home with his/her family <strong>and</strong> within the wider community.<br />

This model aims to provide consistency in assessment <strong>and</strong> delivery of provision to ensure<br />

equitable <strong>and</strong> appropriate resource allocation based on individual need.<br />

This service aims to set an equitable, transparent <strong>and</strong> timely process for assessing, deciding<br />

<strong>and</strong> agreeing bespoke packages of continuing care for those children <strong>and</strong> young people under<br />

the age of 18 who have continuing care needs that cannot be met by existing universal <strong>and</strong><br />

specialist services alone.<br />
