Results from the BEST study should be interpreted with caution • Imbalance of risk factors – epoetin alfa patients were older, had lower PS, more advanced disease and more thrombotic risk factors • Mortality imbalance mostly caused by disease progression within first 4 months – unlikely to be related to epoetin • Unusual features of the study population – a high number of early deaths in both groups – small difference in Hb levels between groups – a high proportion of patients in the placebo group did not become anaemic • Problems in design, conduct and analysis complicate interpretation Leyland-Jones et al.
