QUALITATIVE DETECTION OF <strong>IgM</strong> ANTIBODIES TO<br />
TOXOPLASMA GONDII IN HUMAN SERUM OR<br />
PLASMA BY ENZYME IMMUNOASSAY
1. INTENDED USE<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> is an immunoassay using immunocapture format for<br />
qualitative detection of <strong>IgM</strong> antibodies to T. gondii in human serum or plasma.<br />
2. CLINICAL VALUE<br />
T. gondii is a protozoan causing infection in numerous species of mammals<br />
and birds. <strong>Toxo</strong>plasmosis, frequent in humans and animals, are more typically<br />
silent. The prevalence of this infection in the population, established using<br />
serological tests, may differ depending upon the country of origin and the age.<br />
<strong>Toxo</strong>plasmosis during pregnancy has been implicated in serious congenital<br />
abnormalities (in particular, impaired brain functions) and sometimes stillbirth.<br />
Demonstration of <strong>Toxo</strong> IgG antibody in women prior to conception provides<br />
assurance of fetal protection from possible toxoplasmosis during pregnancy.<br />
Predisposition to severe toxoplasmosis infection is common in persons known<br />
to have Acquired Immune Deficiency Syndrome (AIDS), or who are otherwise<br />
immunocompromised. These infections are mainly due to reactivation of<br />
T. gondii cysts present prior to the HIV infection.<br />
Specific diagnosis of T. gondii infection can be complicated and isolation of<br />
the parasite is rare. Serologic confirmation of T. gondii antibody is indicative of<br />
exposure to the parasite and has become widely accepted as a means to<br />
determine immune status and susceptibility to infection. Screening of several<br />
isotypes allows either the dating of the T. gondii and the implementation of<br />
appropriate therapy in case of recent infection or the proposal of prophylactic<br />
recommendations: hygiene-diet guidelines in pregnant women, chemoprophylaxy<br />
in immunocompromised population.<br />
3. PRINCIPLE<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> is a qualitative test for detection of <strong>IgM</strong> antibodies to<br />
T. gondii in human serum or plasma by enzyme immunoassay with capture of<br />
the <strong>IgM</strong> on the solid phase.<br />
Anti-human µ-chains antibodies are coated on the solid phase (wells of the<br />
microplate). A mixture of the T. gondii antigen and the monoclonal anti-<br />
T. gondii antigen antibody labeled with peroxydase is used as the conjugate.<br />
The test uses the following steps :<br />
• Step 1<br />
Patients samples, calibrator and controls are diluted 1/21 and then distributed<br />
in the wells of the microplate. During this incubation of one hour at 37°C, <strong>IgM</strong><br />
antibodies present in the sample bind to the anti-µ antibodies coated on the<br />
microplate wells. After incubation, IgG and other serum proteins are removed<br />
by washings.<br />

• Step 2<br />
The conjugate (mixture of T. gondii antigen and anti-T. gondii monoclonal<br />
antibody labeled with peroxydase) is added to the microplate wells. During this<br />
incubation of one hour at 37°C, the conjugate binds to the specific <strong>IgM</strong> anti-<br />
T. gondii antibodies that were eventually captured on the microplate. The<br />
unbound conjugate is removed by washings at the end of the incubation.<br />
• Step 3<br />
The presence of immune-complexes (Anti-human µ-chains / <strong>IgM</strong> anti-T. gondii<br />
/ T. gondii Antigen / anti-T. gondii monoclonal antibody labeled with<br />
peroxydase) is demonstrated by the addition in each well of an enzymatic<br />
development solution.<br />
• Step 4<br />
After incubation at room temperature (+18-30°C), the enzymatic reaction is<br />
stopped by addition of 1N sulfuric acid solution. The optical density reading<br />
obtained with a spectrophotometer set at 450/620 nm is proportional to the<br />
amount of <strong>IgM</strong> antibodies to T. gondii present in the sample.<br />
4. PRODUCT INFORMATION<br />
Supplied quantities of reagents have been calculated to allow 96 tests. All<br />
reagents are exclusively for in vitro diagnostic use.<br />

5. WARNINGS AND PRECAUTIONS<br />
The reliability of the results depends on correct implementation of the following<br />
Good Laboratory Practices:<br />
• Do not use expired reagents.<br />
• Do not mix or associate within a given run reagents from different lots.<br />
REMARK: For Washing Solution (R2, label identification: 20x colored<br />
green), Chromogen (R9, label identification: TMB colored turquoise)<br />
and Stopping Solution (R10, label identification: 1N colored red), it is<br />
possible to use other lots than those contained in the kit, provided<br />
these reagents are strictly equivalent and the same lot is used within<br />
a given test run.<br />
REMARK: In addition, the Washing Solution (R2, label identification:<br />

• Before use, wait for 30 minutes to allow reagents to reach room temperature<br />
(+18-30°C).<br />
• Carefully reconstitute or dilute the reagents avoiding any contamination.<br />
• Do not carry out the test in the presence of reactive vapors (acid, alkaline,<br />
aldehyde vapors) or dust that could alter the enzyme activity of the<br />
conjugate.<br />
• Use glassware thoroughly washed and rinsed with deionized water or,<br />
preferably disposable material.<br />
• Washing the microplate is a critical step in the procedure: follow the<br />
recommended number of washings cycles and make sure that all wells are<br />
completely filled and then completely emptied. Incorrect washings may lead<br />
to inaccurate results.<br />
• Do not allow the microplate to dry between the end of the washings<br />
operation and the reagent distribution.<br />
• Never use the same container to distribute the conjugate and the<br />
development solution.<br />
• The enzymatic reaction is very sensitive to metal or metal ions.<br />
Consequently, do not allow any metal element to come into contact with the<br />
various solutions containing the conjugate or the chromogen.<br />
• Chromogen solution (R9) should be colorless. The appearance of a blue<br />
color indicates that the reagent cannot be used and must be replaced.<br />
• Use a new pipette tip for each sample.<br />
• Check the pipettes and other equipments for accuracy and correct<br />
operations.<br />
HEALTH AND SAFETY INSTRUCTIONS<br />
Human origin material used in the preparation of reagents has been tested and<br />
found non-reactive for hepatitis B surface antigen (HBs Ag), antibodies for<br />
hepatitis C virus (anti-HCV), and to human immunodeficiency virus (anti-HIV1<br />
and anti-HIV2). Because no method can absolutely guarantee the absence of<br />
infectious agents, handle reagents of human origin and patient samples as<br />
potentially capable of transmitting infectious diseases:<br />
• Any material, including washings solutions, that comes directly in contact<br />
with samples and reagents containing materials of human origin should be<br />
considered capable of transmitting infectious diseases.<br />
• Wear disposable gloves when handling samples and reagents.<br />
• Do not pipette by mouth.<br />
• Avoid spilling samples or solutions containing samples. Spills must be<br />
rinsed with bleach diluted to 10 %. In the event of a spill with an acid, it<br />

must be first neutralized with sodium bicarbonate, and then cleaned with<br />
bleach diluted to 10% and dried with adsorbent paper. The material used<br />
for cleaning must be discarded in a contaminated residue container.<br />
• Patient samples, reagents containing human origin material, as well as<br />
contaminated material and products should be discarded after<br />
decontamination only:<br />
- either by immersion in bleach at the final concentration of 5 % of sodium<br />
hypochloride during 30 minutes,<br />
- or by autoclaving at 121°C for 2 hours at the minimum.<br />
CAUTION: Do not introduce solutions containing sodium hypochloride<br />
into the autoclave<br />
• Avoid any contact of reagents, including those considered as not<br />
dangerous, with skin and mucosa.<br />
• Chemical and biological residues must be handled and disposed off in<br />
accordance with Good Laboratories Practices.<br />
• All reagents in the kit are exclusively for in vitro diagnostic use.<br />
Caution: Some of the reagents contain ProClin 300 < 1.5%<br />
R43: May cause sensitisation by skin contact<br />
S28-37: After contact with skin, wash immediately with plenty of<br />
Xi - Irritant water and soap. Wear suitable gloves<br />
6. SAMPLES<br />
1. Serum and plasma (EDTA, heparin or citrate) are the recommended sample<br />
types.<br />
2. Observe the following recommendations for handling, processing and<br />
storage of blood samples:<br />
• Collect all blood samples observing routine precaution for venipuncture.<br />
• For serum, allow samples to clot completely before centrifugation.<br />
• Keep tubes stoppered at all times.<br />
• After centrifugation, separate the serum or plasma from the clot or red<br />
cells in a tightly stoppered storage tube.<br />
• The specimens can be stored at +2-8°C if test is performed within<br />
7 days.<br />
• If test will not be completed within 7 days, or for shipment, freeze the<br />
samples at -20°C or colder.<br />
• Do not use samples that have been thawed more than 3 times.<br />
Previously frozen specimens should be thoroughly mixed (Vortex) after<br />
thawing prior to testing.<br />

3. Samples containing 90 g/l of albumin or 100 mg/l of unconjugated bilirubin,<br />
lipemic samples containing the equivalent of 36 g/l of triolein (triglyceride),<br />
and hemolysed samples containing up to 10 g/l of hemoglobin do not affect<br />
the results.<br />

7. ASSAY PROCEDURE<br />
7.1 Materials required but not provided<br />
• Vortex mixer.<br />
• Microplate reader equipped with 450 nm and 620 nm filters (*).<br />
• Microplate incubator thermostatically set at 37±1°C (*).<br />
• Automatic, semi-automatic or manual microplate washer (*).<br />
• Sterile distilled or deionized water.<br />
• Disposable gloves.<br />
• Goggles or safety glasses.<br />
• Adsorbent paper.<br />
• Automatic or semi-automatic, adjustable or preset, pipettes or multipipettes,<br />
to measure and dispense 10 µL to 1000 µL, and 1 mL, 2 mL and<br />
10 mL.<br />
• Graduated cylinders of 25 mL, 50 mL, 100 mL and 1000 mL capacity.<br />
• Sodium hypochloride (bleach) and sodium bicarbonate.<br />
• Container for biohazard waste.<br />
• Disposable tubes.<br />
(*) Consult our technical department for detailed information about the<br />
recommended equipment.<br />
7.2 Reagents reconstitution<br />
• R1: Allow 30 minutes at room temperature (+18-30°C) before opening the<br />
bag. Take out the carrier tray, return unused strips in the bag immediately<br />
and check the presence of desiccant. Carefully reseal the bag and store it at<br />
+2-8°C.<br />
• R2: Dilute 1/20 the washing solution R2 in distilled water: for example 50 mL<br />
of R2 and 950 mL of distilled water to get the ready-to-use washing<br />
solution. Prepare 350 mL of diluted washing solution for one plate of<br />
12 strips if washing manually.<br />
• R3, R4, R5: Dilute 1/21 in Diluent (R7) (example: 300 µL of R7 + 15 µL of<br />
Calibrator or Control).<br />
• R6a: T. gondii Antigen is lyophilized. For running 6 strips, reconstitute one<br />
vial of lyophilized antigen by adding 14 mL of Diluent (R7). Mix thoroughly.<br />
Once diluted, the antigenic solution (R6a+R7) must be perfectly clear.<br />

• R6 (R6a+R6b) - Conjugate working solution: Add 140 µL of conjugate (R6b)<br />
to each vial of reconstituted T. gondii antigen (diluted R6a). Mix thoroughly.<br />
The conjugate working solution must be reconstituted at least 1 hour before<br />
use.<br />
7.3 Storage and validity of opened and / or reconstituted reagents<br />
The kit must be stored at +2-8°C. When the kit is stored at +2-8°C before<br />
opening, each component can be used until the expiration date indicated on<br />
the outer label of the kit.<br />
• R1: Once opened, the strips remain stable for up to 8 weeks if stored at<br />
+2-8°C in the same carefully closed bag (check the presence of desiccant).<br />
• R2: Once diluted, the Washing Solution can be kept for 2 weeks at +2-30°C.<br />
Once opened, the concentrated Washing Solution stored at +2-30°C, in<br />
absence of contamination, is stable until the expiration date indicated on<br />
the label.<br />
• R3, R4, R5, R6b, R7: Once opened and without any contamination, the<br />
reagents stored at +2-8°C are stable for up to 8 weeks.<br />
• R6 (R6a+R6b): Once reconstituted, the conjugate working solution is stable<br />
for 8 hours at room temperature (+18-30°C) or 2 weeks at +2-8°C.<br />
• R9: Once opened and without any contamination, the reagent stored at<br />
+2-8°C is stable for up to 8 weeks.<br />
• R10: Once opened and without any contamination, the reagent stored at<br />
+2-8°C is stable until the expiration date indicated on the label.<br />
7.4 Procedure<br />
Strictly follow the assay procedure and Good Laboratory Practices.<br />
Before use, allow reagents to reach room temperature (+18-30°C).<br />
The use of breakable wells requires a special attention during handling.<br />
Use calibrator and controls with each run to validate the assay results.<br />
1. Carefully establish the distribution and identification plan for calibrator,<br />
controls and patients samples.<br />
2. Prepare the diluted Washing Solution (R2) [Refer to Section 7.2].<br />
3. Take the carrier tray and the strips (R1) out of the protective pouch [Refer to<br />
Section 7.2].<br />
4. Prepare the conjugate working solution R6 (R6a+R6b) [Refer to Section 7.2].<br />
5. In individually identified tubes, dilute Calibrator (R4) and Controls (R3, R5)<br />
and patients samples (S1, S2…) in Diluent (R7) to give a 1/21 dilution:<br />
300 µL of Diluent (R7) and 15 µL of sample. Vortex diluted samples.<br />
6. Strictly following the indicated sequence below, distribute in each well with<br />
200 µL of diluted calibrator, controls and patient samples:<br />

7. Cover the microplate with an adhesive plate sealer, then press firmly onto<br />
the plate to ensure a tight seal. Incubate the microplate immediately in a<br />
thermostat controlled water bath or in a dry incubator for 1 hour ± 5 minutes<br />
at 37°C ± 1°C.<br />
8. At the end of the first incubation period, remove the adhesive plate sealer.<br />
Aspirate the content of all wells into a container for biohazard waste<br />
(containing sodium hypochloride). Wash microplate 4 times with 350 µL of<br />
the Washing Solution (R2). Invert the microplate and gently tap on<br />
adsorbent paper to remove remaining liquid.<br />
9. Distribute immediately 200 µL of the conjugate working solution (R6) in all<br />
wells. The solution must be shaken gently before use.<br />
10. Cover the microplate with an adhesive plate sealer, then press firmly onto<br />
the plate to ensure a tight seal. Incubate the microplate immediately in a<br />
thermostat controlled water bath or in a dry incubator for 1 hour ± 5 minutes<br />
at 37°C ± 1°C.<br />
11. At the end of the second incubation period, remove the adhesive plate<br />
sealer. Aspirate the content of all wells into a container for biohazard waste<br />
(containing sodium hypochloride). Wash microplate 4 times with 350 µL of<br />
the Washing Solution (R2). Invert the microplate and gently tap on<br />
adsorbent paper to remove remaining liquid.<br />
12. Quickly distribute into each well and away from light 200 µL of Chromogen<br />
solution (R9). Allow the reaction to develop in the dark for 30 ± 5 minutes<br />
at room temperature (+18-30°C). Do no use adhesive plate sealer during<br />
this incubation.<br />
13. Stop the enzymatic reaction by adding 100 µL of Stopping Solution (R10) in<br />
each well. Use the same sequence and rate of distribution as for the<br />
development solution.<br />
14. Carefully wipe the plate bottom. Read the optical density at 450/620 nm<br />
using a plate reader within 30 minutes after stopping the reaction. The strips<br />
must always be kept away from light before reading.<br />

15. Before reporting results, check for agreement between the reading and the<br />
distribution plan of plate and samples.<br />
8. INTERPRETATION OF RESULTS<br />
8.1 Calculation of the Cut-Off value (CO)<br />
The Cut-Off value (CO) corresponds to the mean value of the optical densities<br />
(OD) of the cut-off Control duplicates (R4):<br />
CO = mean of OD R4<br />
8.2 Calculation of the Sample Ratio<br />
Sample result is expressed by Ratio using the following formula:<br />
Sample Ratio = Sample OD/CO<br />
8.3 Quality Control<br />
Include the calibrator and controls for each microplate and for each run, and<br />
analyse the obtained results. For validation of the assay, the following criteria<br />
must be met:<br />
• Optical density values:<br />
CO ≥ 0.300<br />
0.80 x CO < OD R4 Repl.1 < 1.20 x CO<br />
0.80 x CO < OD R4 Repl.2 < 1.20 x CO<br />
(Individual OD of each replicate of the Cut-Off control (R4) must not differ more<br />
than 20% of the CO value).<br />
• Optical density ratios:<br />
Ratio R3 (OD R3 / CO) ≤ 0.30<br />
Ratio R5 (OD R5 / CO) ≥ 1.80<br />
If those quality control criteria are not met, the test run should be repeated.<br />
8.4 Interpretation of results<br />
Sample Ratio Result Interpretation<br />
Ratio < 0.80 Negative The sample is considered non reactive for the<br />
presence of <strong>IgM</strong> antibodies to T. gondii.<br />
0.80 ≤ Ratio < 1.00 Equivocal The sample is considered equivocal for the<br />
presence of <strong>IgM</strong> antibodies to T. gondii. The<br />
result must be confirmed by another test done<br />
on a second sample drawn at least 3 weeks<br />
later after the first examination.<br />
Ratio ≥ 1.00 Positive The sample is considered reactive for the<br />
presence of <strong>IgM</strong> antibodies to T. gondii.<br />

8.5 Trouble Shooting Guide<br />
Non validated or non repeatable reactions are often caused by:<br />
• Inadequate microplate washings.<br />
• Contamination of negative samples by serum or plasma with a high<br />
antibody titer.<br />
• Contamination of the development solution by chemical oxidizing agents<br />
(bleach, metal ions...).<br />
• Contamination of the Stopping Solution.<br />
9. PERFORMANCES<br />
Performances of <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> were evaluated at 2 sites using a total of<br />
863 samples from pregnant women and blood donors.<br />
9.1 Prevalence<br />
Prevalence of anti-<strong>Toxo</strong> <strong>IgM</strong> antibodies using <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> was<br />
estimated on a panel of 500 samples from pregnant women. 15 samples were<br />
positive for anti-<strong>Toxo</strong> <strong>IgM</strong> antibodies. Prevalence measured with <strong>Platelia</strong><br />
<strong>Toxo</strong> <strong>IgM</strong> assay is established at 3% (15/500).<br />
9.2 Specificity<br />
Specificity was estimated using a panel of 737 samples found negative with<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> TMB (72751) from 2 sites located in France and split as<br />
follows:<br />
• 154 sera from blood donors<br />
• 583 sera from pregnant women<br />

The discrepant positive sample with <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> assay was confirmed<br />
positive with an ISAGA method.<br />
In addition, 57 samples from a panel of 19 seroconversions were tested. Among<br />
these 19 seroconversions, 18 were detected in a comparable way and one<br />
seroconversion presented a shift of one sample in favour of the reference method.<br />
9.4 Cross Reactivity<br />
A panel of 205 samples including 167 positive samples for CMV, Rubella, EBV,<br />
HSV, VZV, mumps, measles and HIV and 38 positive samples for rheumatoid<br />
factor, auto-antibodies and heterophile antibodies along with myeloma<br />
samples were tested with <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> and a commercialized EIA assay<br />
for screening of anti-T. gondii <strong>IgM</strong> antibodies.<br />
Among theses samples, 2 were found positive and concordant with the EIA kit<br />
used for comparison: 1 sample positive for anti-EBV and 1 sample positive for<br />
anti-HSV 2 IgG.<br />
9.5 Precision<br />
• Within-run precision (repeatability):<br />
In order to evaluate intra-assay repeatability, one negative and three positive<br />
samples were tested 32 times during the same run. The ratio (Sample OD/CO)<br />
was determined for each sample. Mean of ratio, Standard Deviation (SD) and<br />
Coefficient of Variation (%CV) for each specimen are listed in the table below:<br />

• Between-run precision (reproducibility) :<br />
In order to evaluate inter-assay reproducibility, the four samples (one negative<br />
and three positive) were tested in duplicate in two runs per day over a 20 days<br />
period. The concentration (AU/ml) was determined for each positive sample.<br />
Results for the negative sample are expressed in ratio. Mean of concentrations<br />
and ratio, Standard Deviation (SD) and Coefficient of Variation (%CV) for each<br />
of the four specimens are listed in the table below:<br />

10. LIMITATIONS OF THE PROCEDURE<br />
Diagnosis of T. gondii infection can only be established on the basis of a<br />
combination of clinical and biological data. The result of a single test of<br />
titration of anti-T. gondii <strong>IgM</strong> antibodies does not constitute sufficient proof for<br />
the diagnosis of a recent infection.<br />
• Diagnosis of a recent infection can only be made with complete patient<br />
information including clinical and biological data (significant increase of anti-<br />
T. gondii IgG antibodies on 2 patient sera drawn at 3 weeks interval and<br />
tested in the same run, presence of anti-T. gondii <strong>IgM</strong> at a significant level,<br />
demonstration of low IgG avidity).<br />
• Presence of anti-T. gondii <strong>IgM</strong> antibodies does not constitute a sufficient<br />
proof to confirm a recent infection because <strong>IgM</strong> can persist several months<br />
or even years after infection. When <strong>IgM</strong> are detected, a quantitative<br />
determination of anti-T. gondii IgG antibodies should be performed, as well<br />
as a follow-up of the evolution of anti-T. gondii antibodies on at least a<br />
second serum sampled three weeks later.<br />

• If a sample is tested too early during a recent primo-infection, anti-T. gondii<br />
<strong>IgM</strong> antibodies could be not yet present. If a suspicion exists, a second<br />
sample should be drawn about 3 weeks later on which <strong>IgM</strong> testing will be<br />
performed again.<br />

All manufactured reagents are prepared according to our Quality System,<br />
starting from reception of raw material to commercialization of the final<br />
product. Each lot is submitted to quality control assessments and is released<br />

1. DOMAINE D’UTILISATION<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> est un test immunoenzymatique de type immunocapture<br />
pour la détection qualitative des anticorps <strong>IgM</strong> dirigés contre T. gondii dans le<br />
sérum ou le plasma humain.<br />
2. INTERET CLINIQUE<br />
T. gondii est un protozoaire capable d’infecter de nombreuses espèces de<br />
mammifères et d’oiseaux. Ces infections, courantes chez l’homme et les<br />
animaux, se déroulent le plus souvent de façon inapparente sur le plan<br />
clinique. La prévalence de cette infection dans la population, détectée par la<br />
présence d’anticorps spécifiques dans le sérum, est variable en fonction de la<br />
région et de l’âge.<br />
Cette infection peut, dans le cas d’une primo-infection de la mère au cours de<br />
la grossesse, être la cause de graves séquelles pour le foetus (en particulier<br />
une altération des fonctions cérébrales) ou même d’avortement. Une immunité<br />
même ancienne de la mère, démontrée par la présence d’anticorps IgG dès le<br />
début de la grossesse, protège le fœtus de l’infection par ce parasite.<br />
La deuxième population sensible à cette infection est représentée par les<br />
patients immuno-déprimés, dont les malades atteints de SIDA. Ces infections<br />
sont dues, presque exclusivement, à une infection à partir d’un foyer<br />
parasitaire (kyste) quiescent du patient, préexistant à l’infection par le virus HIV.<br />
Le diagnostic de certitude de l’infection toxoplasmique est apporté par la mise<br />
en évidence du parasite, mais sa recherche par examen direct est difficile pour<br />
ne pas dire aléatoire. La sérologie représente la base du diagnostic et du suivi<br />
de la toxoplasmose. La mise en évidence d’anticorps spécifiques permet<br />
d’affirmer une contamination par T. gondii ; l’étude combinée des anticorps<br />
appartenant à différents isotypes permet généralement de dater l’infection et<br />
d’orienter la thérapeutique en cas d’infection récente, ou de proposer des<br />
mesures prophylactiques adaptées au risque de survenue d’une<br />
toxoplasmose : mesures hygiéno-diététiques chez les femmes enceintes non<br />
immunisées, chimioprophylaxie chez les sujets immunodéprimés séropositifs<br />
pour T. gondii.<br />

3. PRINCIPE<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> est un test permettant la détection qualitative des<br />
anticorps <strong>IgM</strong> anti-T. gondii dans le sérum ou le plasma humain par une<br />
méthode immunoenzymatique avec immuno-capture des <strong>IgM</strong> sur phase<br />
solide.<br />
Des anticorps anti-chaîne µ humaines sont utilisés pour sensibiliser la<br />
microplaque. Un mélange d’antigène T. gondii et d’anticorps monoclonal antiantigène<br />
T. gondii marqué à la peroxydase est utilisé comme conjugué. La<br />
mise en œuvre du test comprend les étapes suivantes :<br />
• Etape 1<br />
Les échantillons à étudier ainsi que le calibrateur et les contrôles sont dilués au<br />
1/21 puis déposés dans les cupules de la microplaque. Durant cette<br />
incubation de 1 heure à 37°C, les <strong>IgM</strong> présentes dans l’échantillon sont<br />
captées par les anticorps anti-µ fixés sur les cupules de la microplaque. Les<br />
IgG et les autres protéines sériques sont éliminées par les lavages pratiqués à<br />
la fin de l’incubation.<br />
• Etape 2<br />
Le conjugué (mélange d’antigène T. gondii et d’anticorps monoclonal anti-<br />
T. gondii marqué à la peroxydase) est déposé dans toutes les cupules de la<br />
microplaque. Durant cette incubation de 1 heure à 37°C, si l’échantillon testé<br />
contient des anticorps <strong>IgM</strong> spécifiques du T. gondii, ceux-ci vont fixer le<br />
conjugué. Le conjugué en excès non lié est éliminé par les lavages pratiqués à<br />
la fin de l’incubation.<br />
• Etape 3<br />
La présence des complexes immuns (anti-chaîne µ humaine / <strong>IgM</strong> anti-<br />
T. gondii / Antigène T. gondii / Anticorps monoclonal anti-T. gondii marqué à la<br />
peroxydase) éventuellement formés est révélée par l’addition dans chaque<br />
cupule d’une solution de révélation enzymatique.<br />
• Etape 4<br />
Après incubation à température ambiante (+18-30°C), la réaction enzymatique<br />
est stoppée par addition d’une solution d’acide sulfurique 1N. La densité<br />
optique lue à 450/620 nm, interprétée par rapport à une valeur seuil, permet de<br />
confirmer ou d’infirmer la présence d’<strong>IgM</strong> anti-T. gondii dans l’échantillon<br />
testé.<br />

• La réaction enzymatique est très sensible à tous métaux ou ions métalliques.<br />
Par conséquent, aucun élément métallique ne doit entrer en contact avec<br />

• Le Chromogène (R9) doit être incolore. L’apparition d’une coloration bleue<br />
indique que le réactif est inutilisable et doit être remplacé.<br />
• Utiliser un cône de distribution neuf pour chaque échantillon.<br />
• Vérifier l’exactitude des pipettes et le bon fonctionnement des appareils<br />
utilisés.<br />
CONSIGNES D’HYGIENE ET DE SECURITE<br />
Les composants d’origine humaine utilisés dans la préparation des réactifs<br />
ont été testés et trouvés non réactifs pour l’antigène de surface de l’hépatite B<br />
(Ag HBs), les anticorps dirigés contre le virus de l’hépatite C (anti-VHC) et les<br />
anticorps dirigés contre les virus de l’immunodéficience humaine (anti-VIH1 et<br />
anti-VIH2). Du fait qu’aucune méthode ne peut garantir de façon absolue<br />
l'absence d’agents infectieux, considérer les réactifs d’origine humaine ainsi<br />
que tous les échantillons de patients comme potentiellement infectieux et les<br />
manipuler avec les précautions d'usage :<br />
• Considérer le matériel directement en contact avec les échantillons et les<br />
réactifs d’origine humaine ainsi que les solutions de lavage comme des<br />
produits contaminés.<br />
• Porter des gants à usage unique lors de la manipulation des réactifs et des<br />
échantillons.<br />
• Ne pas pipeter à la bouche.<br />
• Eviter les éclaboussures d’échantillons ou de solutions les contenant.<br />
Nettoyer les surfaces souillées avec de l’eau de javel diluée à 10 %. Si le<br />
liquide contaminant est un acide, neutraliser au préalable les surfaces<br />
souillées avec du bicarbonate de soude, puis nettoyer à l’aide d’eau de javel<br />
et sécher avec du papier absorbant. Le matériel utilisé pour le nettoyage<br />
devra être jeté dans un conteneur spécial pour déchets contaminés.<br />
• Eliminer les échantillons, les réactifs d’origine humaine ainsi que le matériel<br />
et les produits contaminés après décontamination :<br />
- soit par immersion dans de l’eau de javel à la concentration finale de 5 %<br />
d’hypochlorite de sodium pendant 30 minutes.<br />
- soit par autoclavage à 121°C pendant 2 heures minimum.<br />
ATTENTION : ne pas introduire dans l’autoclave des solutions<br />
contenant de l’hypochlorite de sodium<br />

• Eviter tout contact des réactifs, y compris ceux considérés comme non<br />
dangereux, avec la peau et les muqueuses.<br />
• La manipulation et l’élimination des déchets chimiques et biologiques<br />
doivent être faites selon les Bonnes Pratiques de Laboratoire.<br />
• Tous les réactifs de la trousse sont destinés au seul usage diagnostic in<br />
vitro.<br />
Attention : certains réactifs contiennent du ProClin 300 < 1,5%<br />
R43 : Peut entraîner une sensibilisation par contact avec la peau<br />
S28-37 : Après contact avec la peau, se laver immédiatement et<br />
Xi - Irritant abondamment avec de l’eau et du savon. Porter des gants appropriés<br />
6. ECHANTILLONS<br />
1. Les tests sont effectués sur des échantillons de sérum ou de plasma<br />
recueilli sur anticoagulant de type EDTA, héparine ou citrate.<br />
2. Respecter les consignes suivantes pour le prélèvement, le traitement et la<br />
conservation de ces échantillons de sang :<br />
• Prélever un échantillon de sang selon les pratiques en usage.<br />
• Pour les sérums, laisser le caillot se former complètement avant<br />
centrifugation.<br />
• Conserver les tubes fermés.<br />
• Après centrifugation, extraire le sérum ou le plasma et le conserver en<br />
tube fermé.<br />
• Les échantillons seront conservés à +2-8°C si le test est effectué dans<br />
les 7 jours.<br />
• Si le test n’est pas effectué dans les 7 jours, ou pour tout envoi, les<br />
échantillons seront congelés à -20°C (ou plus froid).<br />
• Il est recommandé de ne pas procéder à plus de 3 cycles de congélation/<br />
décongélation. Les échantillons devront être soigneusement<br />
homogénéisés (Vortex) après décongélation et avant la réalisation du test.<br />
3. Les résultats ne sont pas affectés par les échantillons contenant 90 g/l<br />
d’albumine ou 100 mg/l de bilirubine non conjuguée, les échantillons<br />
lipémiques contenant l’équivalent de 36 g/l de trioléïne (triglycéride) ou les<br />
échantillons hémolysés contenant 10 g/l d’hémoglobine.<br />
4. Ne pas chauffer les échantillons.<br />
7. MODE OPÉRATOIRE<br />

7.2 Reconstitution des réactifs<br />
• R1 : Laisser revenir 30 minutes à température ambiante (+18-30°C) avant<br />
ouverture du sachet. Sortir le cadre et replacer immédiatement les barrettes<br />
non utilisées dans le sachet en vérifiant la présence du dessicant. Refermer<br />
soigneusement le sachet et le replacer à +2-8°C.<br />
• R2 : Diluer au 1/20 la solution R2 avec de l’eau distillée : 50 mL de R2 dans<br />
950 mL d’eau distillée. On obtient ainsi la solution prête à l’emploi. Prévoir<br />
350 mL de solution de lavage diluée pour une plaque entière de 12 barrettes<br />
en lavage manuel.<br />
• R3, R4, R5 : Diluer au 1/21 dans le Diluant R7 (exemple : 300 µL de R7<br />
+ 15 µL de Calibrateur ou Contrôle).<br />
• R6a : L’antigène T. gondii est présenté sous forme lyophilisée. Pour la<br />
réalisation de 6 barrettes, reprendre le contenu d’un flacon par 14 mL de<br />
Diluant (R7). Bien homogénéiser. Une fois diluée, la solution antigénique<br />
(R6a+R7) doit être parfaitement limpide.<br />
• R6 (R6a+R6b) – Solution de travail du conjugué : Ajouter 140 µL de<br />
conjugué (R6b) dans chaque flacon d’antigène T. gondii reconstitué (R6a<br />
dilué). Bien homogénéiser. La solution de travail du conjugué doit être<br />
reconstituée au moins 1 heure avant emploi.<br />
7.3 Conservation et validité des réactifs ouverts et/ou reconstitués<br />
La trousse doit être conservée à +2-8°C. Chaque élément de la trousse<br />
conservée avant ouverture à +2-8°C peut être utilisé jusqu’à la date de<br />
péremption indiquée sur le coffret.<br />
• R1: Après ouverture, les barrettes conservées dans le sachet correctement<br />
refermé sont stables pendant 8 semaines à +2-8°C (vérifier la présence du<br />
dessicant).<br />

• R2: Après dilution, la Solution de Lavage se conserve 2 semaines à<br />
+2-30°C. Après ouverture et en l’absence de contamination, la Solution de<br />
Lavage concentrée peut être conservée à +2-30°C jusqu’à la date indiquée<br />
sur l’étiquette.<br />
• R3, R4, R5, R6b, R7 : Après ouverture et en l’absence de contamination,<br />
les réactifs conservés à +2-8°C sont stables pendant 8 semaines.<br />
• R6 (R6a+R6b) : Après reconstitution, la solution de travail du conjugué est<br />
stable 8 heures à température ambiante (+18-30°C) et pendant 2 semaines<br />
à +2-8°C.<br />
• R9 : Après ouverture et en l’absence de contamination, le réactif conservé à<br />
+2-8°C est stable pendant 8 semaines.<br />
• R10 : Après ouverture et en l’absence de contamination, le réactif conservé<br />
à +2-8°C est stable jusqu’à la date d’expiration indiquée sur l’étiquette.<br />
7.4 Mode opératoire<br />
Suivre strictement le protocole proposé et appliquer les Bonnes Pratiques de<br />
Laboratoire.<br />
Avant utilisation, laisser tous les réactifs revenir à température ambiante<br />
(+18-30°C).<br />
L’utilisation de puits sécables requiert une attention particulière lors de la<br />
manipulation.<br />
Utiliser le calibrateur et les contrôles à chaque mise en œuvre du dosage pour<br />
valider la qualité du test.<br />
1. Etablir soigneusement le plan de distribution et d’identification du<br />
calibrateur, des contrôles et des échantillons de patients.<br />
2. Préparer la Solution de Lavage diluée (R2) [Se référer au chapitre 7.2].<br />
3. Sortir le cadre support et les barrettes (R1) de l’emballage protecteur<br />
[Se référer au chapitre 7.2].<br />
4. Préparer la solution de travail du conjugué R6 (R6a+R6b) [Se référer au<br />
chapitre 7.2].<br />
5. Dans des tubes identifiés individuellement, diluer le calibrateur (R4) et les<br />
contrôles (R3, R5) ainsi que les échantillons de patient à tester (S1, S2…) au<br />
1/21 dans le Diluant (R7), soit 300 µL de Diluant (R7) puis 15 µL<br />
d’échantillon [Se référer au Chapitre 7.2]. Bien homogénéiser (Vortex).<br />
6. Distribuer dans chaque cupule 200 µL du calibrateur, des contrôles et des<br />
échantillons dilués selon le schéma suivant :<br />

7. Couvrir la microplaque d’un film adhésif en appuyant bien sur toute la<br />
surface pour assurer l’étanchéité. Puis incuber immédiatement la<br />
microplaque au bain-marie thermostaté ou dans un incubateur sec de<br />
microplaques pendant 1 heure ± 5 minutes à 37°C ± 1°C.<br />
8. A la fin de la première incubation, retirer le film adhésif, aspirer le contenu de<br />
toutes les cupules dans un conteneur pour déchets contaminés (contenant<br />
de l’hypochlorite de sodium) et procéder à 4 lavages avec 350 µL de la<br />
Solution de Lavage (R2). Sécher les barrettes par retournement sur une<br />
feuille de papier absorbant et taper légèrement afin d’éliminer la totalité de<br />
la Solution de Lavage.<br />
9. Distribuer immédiatement 200 µL de la solution de travail du conjugué (R6)<br />
dans toutes les cupules. Agiter délicatement cette solution avant l’emploi.<br />
10. Couvrir la microplaque d’un film adhésif neuf en appuyant bien sur toute la<br />
surface pour assurer l’étanchéité. Incuber la microplaque au bain-marie<br />
thermostaté ou dans un incubateur sec de microplaques pendant 1 heure<br />
± 5 minutes à 37°C ± 1°C.<br />
11. A la fin de la deuxième incubation, retirer le film adhésif, aspirer le contenu<br />
de toutes les cupules dans un conteneur pour déchets contaminés<br />
(contenant de l’hypochlorite de sodium) et procéder à 4 lavages avec<br />
350 µL de la Solution de Lavage (R2). Sécher les barrettes par retournement<br />
sur une feuille de papier absorbant et taper légèrement afin d’éliminer la<br />
totalité de la Solution de Lavage.<br />
12. Distribuer rapidement, et à l'abri de la lumière vive, 200 µL du Chromogène<br />
(R9) dans toutes les cupules. Laisser la réaction se développer à<br />
l’obscurité pendant 30 ± 5 minutes à température ambiante (+18-30°C).<br />
Lors de cette incubation, ne pas utiliser de film adhésif.<br />
13. Arrêter la réaction enzymatique en ajoutant 100 µL de la Solution d’Arrêt<br />
(R10) dans chaque cupule. Adopter la même séquence et le même rythme<br />
de distribution que pour la solution de révélation.<br />
14. Essuyer soigneusement le dessous des plaques. Lire la densité optique à<br />
450/620 nm à l’aide d’un lecteur de plaques dans les 30 minutes qui suivent<br />
l’arrêt de la réaction. Les barrettes doivent toujours être conservées à l’abri<br />
de la lumière avant la lecture.
15. S'assurer, avant la transcription des résultats, de la concordance entre la<br />
lecture et le plan de distribution des plaques et des échantillons.<br />
8. INTERPRÉTATION DES RÉSULTATS<br />
8.1 Calcul de la Valeur Seuil (VS)<br />
La valeur Seuil VS correspond à la moyenne des densités optiques (DO) des<br />
duplicats du Calibrateur (R4) :<br />

8.2 Calcul du Ratio Echantillon<br />
Les résultats pour un échantillon donné sont exprimés sous forme d’un ratio à<br />

Analyser les résultats de DO obtenus avec le Calibrateur et les Contrôles sur<br />
chaque microplaque et pour chaque série. Pour valider la manipulation, les<br />

Si ces critères ne sont pas respectés, recommencer la manipulation.<br />

8.5 Expertise des causes d’erreur<br />
L’origine des réactions non validées ou non reproductibles est souvent en<br />
relation avec les causes suivantes :<br />
• Lavage insuffisant des microplaques.<br />
• Contamination des échantillons négatifs par un sérum ou un plasma<br />
contenant un titre élevé d’anticorps.<br />
• Contamination ponctuelle de la solution de révélation par des agents<br />
chimiques oxydants (eau de javel, ions métalliques...).<br />
• Contamination ponctuelle de la Solution d’Arrêt.<br />
9. PERFORMANCES<br />
Les performances du kit <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> ont été évaluées sur 2 sites sur un<br />
total de 863 échantillons provenant de femmes enceintes et de donneurs de<br />
sang.<br />
9.1 Prévalence<br />
La prévalence des <strong>IgM</strong> anti-<strong>Toxo</strong> mesurée à l’aide du test <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong><br />
a été estimée sur un panel de 500 échantillons provenant de femmes<br />
enceintes. 15 échantillons se sont révélés positifs pour la présence d’anticorps<br />
anti-<strong>Toxo</strong> <strong>IgM</strong>. La prévalence mesurée à l’aide du kit <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong><br />
s’établit donc à 3% (15/500).<br />
9.2 Spécificité<br />
La spécificité a été déterminée sur un panel de 737 échantillons trouvés<br />
négatifs à l'aide du <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> TMB (72751), provenant de 2 sites<br />
situés en France et se répartissant comme suit :<br />

L’échantillon positif discordant avec la trousse <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> a été<br />
confirmé positif par méthode ISAGA.<br />
De plus, 57 échantillons provenant d'un panel de 19 séroconversions ont été<br />
testés. Sur ces 19 séroconversions, 18 ont été détectées de manière<br />
comparable et une séroconversion a présenté un décalage d’un prélèvement<br />
en faveur de la méthode de référence.<br />
9.4 Réactivité croisée<br />
Un panel de 205 échantillons comprenant 167 échantillons positifs pour les<br />
marqueurs CMV, Rubéole, EBV, HSV, VZV, oreillons, rougeole, HIV et<br />
38 échantillons positifs en facteurs rhumatoïdes, auto-anticorps, anticorps<br />
hétérophiles ainsi que des échantillons de myélome ont été testés avec le test<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> et un test EIA commercialisé pour le dépistage des<br />
anticorps <strong>IgM</strong> anti-T. gondii.<br />
Parmi ces échantillons, 2 se sont révélés positifs concordants avec la trousse<br />
EIA utilisée en référence : 1 échantillon positif en <strong>IgM</strong> anti-EBV et 1 échantillon<br />
positif en IgG anti-HSV 2.<br />
9.5 Précision<br />
• Précision intra-essai (répétabilité) :<br />
Afin d'évaluer la répétabilité intra-essai, un échantillon négatif et trois<br />
échantillons positifs ont été testés à 32 reprises dans une même série. Le ratio<br />
(DO Echantillon/VS) a été déterminé pour chaque échantillon. La moyenne des<br />
ratios, la déviation standard (DS) et le coefficient de variation (%CV) pour<br />
chaque échantillon sont donnés dans le tableau suivant.<br />

10. LIMITES D’UTILISATION<br />
Le diagnostic d’infection par T. gondii ne peut être définitivement établi que sur<br />
un ensemble de données cliniques et biologiques. Le résultat d’un seul test de<br />
détection des anticorps <strong>IgM</strong> anti-T. gondii ne constitue pas en soi une preuve<br />
suffisante pour poser le diagnostic d’infection récente par T. gondii.<br />
• Seul un ensemble de données cliniques et biologiques (augmentation<br />
significative du titre des IgG anti-T. gondii entre 2 sérums issus d’un même<br />
patient prélevés à 3 semaines d’intervalle et testés au cours d’un seul<br />
dosage, présence d’<strong>IgM</strong> anti-T. gondii à un taux significatif et mise en<br />
évidence d’une avidité faible des IgG) permet le diagnostic d’une infection<br />
récente par le parasite.<br />
• La seule présence d’<strong>IgM</strong> anti-T. gondii ne permet pas de conclure à une<br />
infection récente évolutive en raison d’une possible persistance des <strong>IgM</strong><br />
plusieurs mois, voire plusieurs années, après l’infection. Lorsque des <strong>IgM</strong><br />
sont détectés, une détermination quantitative du titre en anticorps IgG anti-<br />
T. gondii devra être réalisée, de même qu’un suivi de l’évolution de la<br />
réponse en anticorps anti-T. gondii du patient sur au moins un deuxième<br />
prélèvement réalisé 3 semaines plus tard.<br />

• Si le prélèvement est effectué trop précocement lors d’une primo-infection<br />
débutante, les anticorps <strong>IgM</strong> anti-T. gondii peuvent ne pas être encore<br />
présents. En cas de doute, un second prélèvement doit être effectué<br />
3 semaines plus tard sur lequel la recherche des <strong>IgM</strong> sera répétée.<br />
11. CONTROLE QUALITE DU FABRICANT<br />
Tous les produits fabriqués et commercialisés par la société Bio-Rad sont<br />
placés sous un système d’assurance qualité de la réception des matières<br />
premières jusqu’à la commercialisation des produits finis. Chaque lot de<br />
produit fini fait l’objet d’un contrôle de qualité et n’est commercialisé que s’il<br />
est conforme aux critères d’acceptation. La documentation relative à la<br />

DETECCIÓN CUALITATIVA DE ANTICUERPOS <strong>IgM</strong><br />
ANTI-TOXOPLASMA GONDII, EN SUERO O PLASMA<br />
HUMANO, POR MÉTODO INMUNOENZIMÁTICO
1. CAMPO DE APLICACIÓN<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> es una prueba inmunoenzimática por inmunocaptura para<br />
la detección cualitativa de anticuerpos <strong>IgM</strong> dirigidos contra T. gondii en suero<br />
o plasma humano.<br />
2. INTERÉS CLÍNICO<br />
T. gondii es un protozoario capaz de infectar a numerosas especies de<br />
mamíferos y pájaros. Estas infecciones, habituales en el hombre y en los<br />
animales, se desarrollan generalmente de forma inapreciable desde el punto<br />
de vista clínico. La prevalencia de esta infección en la población, detectada<br />
por la presencia de anticuerpos específicos en el suero, varía en función de la<br />
región y de la edad.<br />
En caso de una primoinfección de la madre durante el embarazo, esta<br />
infección puede provocar graves secuelas en el feto (en especial, una<br />
alteración de las funciones cerebrales) o un aborto. La inmunidad antigua de la<br />
madre, demostrada mediante la presencia de anticuerpos IgG al inicio del<br />
embarazo, protege al feto de la infección por este parásito.<br />
La segunda población sensible a esta infección está representada por<br />
lospacientes inmunodeprimidos, como los enfermos afectados de SIDA. Estas<br />
infecciones se deben, casi exclusivamente, a una infección a partir de un foco<br />
parasitario (quiste) quiescente del paciente, cuya existencia es previa a la<br />
infección por el virus HIV.<br />
El diagnóstico de la infección toxoplásmica se confirma con la demostración<br />
de la presencia del parásito, pero su búsqueda mediante el examen directo es<br />
difícil, por no decir aleatorio. La serología representa la base del diagnóstico y<br />
del seguimiento de la toxoplasmosis. La presencia de anticuerpos específicos<br />
permite confirmar una contaminación por T. gondii; el estudio combinado de<br />
los anticuerpos pertenecientes a diferentes isotipos permite, generalmente,<br />
fechar la infección y orientar la terapéutica en caso de infección reciente, o<br />
establecer medidas profilácticas adecuadas al riesgo de aparición de una<br />
toxoplasmosis: medidas higiénico-dietéticas en mujeres embarazadas que no<br />
presenten anticuerpos, quimiofilaxia en los sujetos inmunodeprimidos<br />
seropositivos para T. gondii.<br />

3. PRINCIPIO<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> es una prueba que permite la detección cualitativa de los<br />
anticuerpos <strong>IgM</strong> anti-T. gondii en suero o plasma humano mediante un<br />
método inmunoenzimático por inmunocaptura de las <strong>IgM</strong> en fase sólida.<br />
Se utilizan anticuerpos anti-cadena µ humana para sensibilizar la microplaca.<br />
Se utiliza como conjugado una mezcla de antígeno T. gondii y anticuerpo<br />
monoclonal anti-antígeno T. gondii marcado con peroxidasa. La aplicación de<br />
la prueba consta de las etapas siguientes:<br />
• Etapa 1<br />
Las muestras a estudiar y los controles se diluyen en proporción 1/21 y se<br />
depositan en los pocillos de la microplaca. Durante la incubación de 1 hora a<br />
37ºC, las <strong>IgM</strong> son captadas por los anticuerpos anti-µ fijados a las cúpulas de<br />
los pocillos de la microplaca. Las IgG y el resto de las proteínas séricas son<br />
eliminadas mediante lavados al final de la incubación.<br />
• Etapa 2<br />
Se coloca el conjugado (mezcla de antígeno T. gondii y de anticuerpo anti-<br />
T. gondii marcado con la peroxidasa) en todos los pocillos de la microplaca.<br />
Durante esta incubación de 1 hora a 37°C, si la muestra probada contiene<br />
anticuerpos <strong>IgM</strong> específicos de T. gondii, éstos se fijarán al conjugado. El<br />
conjugado sobrante que no se une es eliminado mediante lavados al final de la<br />
incubación.<br />
• Etapa 3<br />
La presencia de los complejos inmunes (anti-cadena µ humana / <strong>IgM</strong> anti-<br />
T. gondii / Antígeno T. gondii / Anticuerpo monoclonal anti-T. gondii marcado<br />
con peroxidasa) que se forman eventualmente es revelada mediante la adición<br />
de una solución enzimática de revelado en cada pocillo.<br />
• Etapa 4<br />
Tras incubación a temperatura ambiente (+18-30ºC) la reacción enzimática se<br />
detiene mediante la adición de una solución de ácido sulfúrico 1N. La<br />
densidad óptica leída a 450/620 nm, interpretada en relación con un valor<br />
umbral, permite confirmar o informar de la presencia de <strong>IgM</strong> anti-T. gondii en<br />
la muestra probada.<br />

Un mal lavado puede provocar resultados incorrectos.<br />
• No dejar secar la microplaca entre el final de los lavados y la distribución de<br />
los reactivos.<br />
• No utilizar nunca el mismo recipiente para distribuir el conjugado y la<br />
solución reveladora.<br />
• La reacción enzimática es muy sensible a todos los metales o iones<br />
metálicos. Por consiguiente, ningún elemento metálico debe entrar en<br />
contacto con las diferentes soluciones que contienen el conjugado o el<br />
cromógeno.<br />
• El cromógeno (R9) debe ser incoloro. La aparición de un color azul indica<br />
que el reactivo no es utilizable y debe reemplazarse.<br />
• Utilizar un cono de distribución nuevo para cada muestra.<br />
• Comprobar la exactitud de las pipetas y el correcto funcionamiento de los<br />
aparatos utilizados.<br />
INSTRUCCIONES DE HIGIENE Y SEGURIDAD<br />
Los componentes de origen humano que se utilizan en la preparación de los<br />
reactivos se han sometido a tests y han resultado no reactivos para el<br />
antígeno de superficie de la hepatitis B (Ag HBs), los anticuerpos dirigidos<br />
contra el virus de la hepatitis C (anti-VHC) y los anticuerpos dirigidos contra<br />
los virus de la inmunodeficiencia humana (anti-VIH1 y anti-VIH2). Dado que<br />
ningún método es capaz de garantizar de manera absoluta la ausencia de<br />
agentes infecciosos, ha que considerar los reactivos de origen humano y las<br />
muestras de los pacientes como potencialmente infecciosos y manipularlos<br />
con las precauciones habituales:<br />
• Considerar el material que está directamente en contacto con las muestras<br />
y los reactivos de origen humano así como las soluciones de lavado, como<br />
productos contaminados.<br />
• Utilizar guantes de un solo uso para manipular reactivos y muestras.<br />
• No pipetear con la boca.<br />
• Evitar las salpicaduras de muestras o de soluciones que las contengan.<br />
Limpiar las superficies manchadas con lejía diluida al 10%. Si el líquido<br />
contaminante es un ácido, neutralizar previamente con bicarbonato sódico<br />
las superficies manchadas y a continuación limpiar con lejía y secar con<br />
papel absorbente. El material utilizado para la limpieza deberá desecharse<br />
en un contenedor especial para desechos contaminados.<br />
• Eliminar las muestras, los reactivos de origen humano y el material y los<br />
productos contaminados después de su descontaminación:<br />
- ya sea mediante inmersión en lejía a una concentración final de 5% de<br />
hipoclorito de sodio durante 30 minutos.<br />
- o bien mediante autoclave a 121ºC durante al menos 2 horas.<br />

CUIDADO: no introducir en el autoclave soluciones que contengan<br />
hipoclorito de sodio<br />
• Evitar todo contacto de los reactivos, incluidos los considerados como no<br />
peligrosos, con la piel y las mucosas.<br />
• La manipulación y la eliminación de desechos químicos y biológicos deben<br />
hacerse siguiendo las Buenas Prácticas de Laboratorio.<br />
• Todos los reactivos del kit están destinados a un solo uso diagnóstico in vitro.<br />
Cuidado: ciertos reactivos contienen ProClin 300 < 1,5%<br />
R43: Puede provocar una sensibilización por contacto con la piel.<br />
S28-37: Después del contacto con la piel, lavarse de inmediato y<br />
Xi - Irritante abundantemente con agua y jabón. Utilizar guantes adecuados.<br />
6. MUESTRAS<br />
1. Las pruebas se realizan sobre muestras de suero o de plasma recogido<br />
sobre anticoagulante de tipo EDTA, heparina o citrato.<br />
2. Respetar las instrucciones siguientes para la extracción, el tratamiento y la<br />
conservación de las muestras de sangre:<br />
• Extraer una muestra de sangre siguiendo las prácticas en uso.<br />
• Para los sueros, dejar que se forme el coágulo totalmente antes de<br />
centrifugar.<br />
• Conservar los tubos cerrados.<br />
• Después de la centrifugación, extraer el suero o el plasma y conservarlo<br />
en tubo cerrado.<br />
• Si la prueba se realiza en el plazo de 7 días, las muestras se conservarán<br />
a +2-8°C.<br />
• Si la prueba no se realiza en el plazo de 7 días, o para cualquier envío, las<br />
muestras se congelarán a -20°C (o más frío).<br />
• Se recomienda no proceder a más de 3 ciclos de congelación/<br />
descongelación. Las muestras deberán homogeneizarse minuciosamente<br />
(vórtex) después de descongelar y antes de realizar la prueba.<br />
3. Los resultados no se ven afectados por muestras que contengan 90 g/l de<br />
albúmina o 100 mg/l de bilirrubina no conjugada, las muestras lipémicas<br />
que contengan el equivalente de 36 g/l de trioleína (triglicérido) o muestras<br />
hemolizadas que contengan 10 g/l de hemoglobina.<br />
4. No calentar las muestras.<br />
7. MODO DE ACTUACIÓN<br />
7.1 Material necesario no suministrado<br />
• Agitador tipo vórtex.<br />
• Lector de microplacas equipado con filtros 450/620 nm (*).<br />
• Incubador de microplacas que admite termostato a 37°C ± 1°C (*).<br />

• Hipoclorito de sodio (lejía) y bicarbonato de sodio.<br />
• Contenedor de desechos contaminados.<br />

7.2 Reconstitución de los reactivos<br />
• R1: Esperar a que adquiera la temperatura ambiente (+18-30°C) antes de<br />
abrir la bolsa. Sacar el soporte y volver a poner inmediatamente en la bolsa<br />
las tiras no utilizadas, comprobando que esté presente el secante. Volver a<br />
cerrar minuciosamente la bolsa y poner de nuevo a +2-8°C.<br />
• R2: Diluir a 1/20 la solución R2 con agua destilada: 50 mL de R2 en 950 mL<br />
de agua destilada. Se obtiene de este modo la solución lista para usar.<br />
Tener previstos 350 mL de solución de lavado diluida para una placa entera<br />
de 12 tiras en modo de lavado manual.<br />
• R3, R4, R5 : : Diluir a 1/21 en el diluyente (R7) (ejemplo: 15 µL R3<br />
+ 300 µL R7).<br />
• R6a : : El antígeno de T. gondii está liofilizado. Para analizar 6 tiras,<br />
reconstituir un vial de antígeno liofilizado añadiéndole 14 mL de diluyente<br />
(R7). Mezclar bien. Una vez diluida, la solución antigénica (R6a+R7) debe<br />
quedar completamente transparente.<br />
• R6 (R6a+R6b) : – Solución activa de conjugado: Añadir 140 µL de<br />
conjugado (R6b) en cada vial de antígeno de T. gondii reconstituido (R6a<br />
diluido). Mezclar bien. La solución activa de conjugado debe reconstituirse<br />
al menos 1 hora antes de usarla.<br />
7.3 Conservación y validez de los reactivos abiertos y/o reconstituidos<br />
El kit debe conservarse a +2-8°C. Cada elemento del kit conservado a +2-8°C<br />
antes de la apertura puede utilizarse hasta la fecha de caducidad indicada en<br />
el envase.<br />
• R1: Después de abrir, las tiras conservadas correctamente de nuevo en la<br />
bolsa cerrada permanecen estables durante 8 semanas a +2-8°C<br />
(comprobar que esté presente el secante).<br />
• R2: Después de la dilución, la solución de lavado se conserva 2 semanas a<br />
+2-30°C. Después de abrir y si no hay presencia de contaminación, la<br />
solución de lavado concentrada puede conservarse a +2-30°C hasta la<br />
fecha indicada en la etiqueta.
• R3, R4, R5, R6b, R7: Después de abrir y si no hay presencia de<br />
contaminación, los reactivos conservados a +2-8°C son estables durante<br />
8 semanas.<br />
• R6 (R6a+R6b): Después de reconstruir, la solución de trabajo del<br />
conjugado es estable 8 horas a temperatura ambiente (+18-30°C) y<br />
2 semanas a +2-8°C.<br />
• R9: Después de abrir y si no hay presencia de contaminación, el reactivo<br />
conservado a +2-8°C es estable durante 8 semanas.<br />
• R10: Después de abrir y si no hay presencia de contaminación, el reactivo<br />
conservado a +2-8°C es estable hasta la fecha de caducidad indicada en la<br />
etiqueta.<br />
7.4 Modo de actuación<br />
Seguir al pie de la letra el protocolo propuesto y aplicar las Buenas Prácticas<br />
de Laboratorio.<br />
Antes de usar, esperar a que todos los reactivos se pongan a temperatura<br />
ambiente (+18-30°C).<br />
El uso de pocillos frágiles requiere especial cuidado durante la manipulación.<br />
Utilizar el calibrador y los sueros de controles en cada dosificación para validar<br />
la calidad de la prueba.<br />
1. Establecer cuidadosamente el plan de distribución y de identificación del<br />
calibrador, de los sueros de control y de las muestras de pacientes.<br />
2. Preparar la solución de lavado diluida (R2) [Consultar el Capítulo 7.2].<br />
3. Sacar el soporte y las tiras (R1) del envase protector [Consultar el<br />
Capítulo 7.2]<br />
4. Preparar la solución de conjugado R6 (R6a+R6b) [Consultar el Capítulo 7.2].<br />
5. Disolver el calibrador (R4), los sueros de control (R3, R5) y las muestras de<br />
pacientes a someter a prueba (S1, S2, etc.) a 1/21 en el diluyente (R7), es<br />
decir 15 µL de muestra y 300 µL de diluyente (R7) [Consultar el Capítulo 7.2].<br />
Homogeneizar correctamente (vórtex).<br />
6. Distribuir en cada pocillo 200 µL del calibrador, de los sueros de control y<br />
de las muestras diluidas según el esquema siguiente:<br />

7. Cubrir la microplaca con una película adhesiva apoyando bien sobre toda la<br />
superficie para garantizar la estanqueidad, a continuación incubar<br />
inmediatamente la microplaca al baño maría con termostato o en una<br />
incubadora seca de microplacas durante 1 hora ± 5 minutos a 37°C ± 1°C.<br />
8. Al final de la primera incubación, retirar el film adhesivo, aspirar el contenido<br />
de todos los pocillos en un contenido para desechos contaminados (que<br />
contienen hipoclorito de sodio) y proceder a 4 lavados con 350 µl de la<br />
solución de lavado (R2). Secar las tiras empapándolas en una hoja de papel<br />
absorbente y golpear ligeramente para eliminar la totalidad de la solución<br />
de lavado.<br />
9. Distribuir 200 µL de la solución de trabajo del conjugado (R6) en todos los<br />
pocillos. Agitar delicadamente esta solución antes de usarla.<br />
10. Tapar la microplaca de un film adhesivo apoyando bien sobre toda la<br />
superficie para garantizar la estanqueidad. Incubar la microplaca al baño<br />
maría con termostato o en una incubadora seca de microplacas durante<br />
1 hora ± 5 minutos a 37°C ± 1°C.<br />
11. Al final de la primera incubación, retirar el film adhesivo, aspirar el contenido<br />
de todos los pocillos en un contenido para desechos contaminados (que<br />
contienen hipoclorito de sodio) y proceder a 4 lavados con 350 µL de la<br />
solución de lavado (R2). Secar las tiras empapándolas en una hoja de papel<br />
absorbente y golpear ligeramente para eliminar la totalidad de la solución<br />
de lavado.<br />
12. Distribuir rápidamente, y al abrigo de la luz viva, 200 µL del cromógeno<br />
(R9) en todas las cúpulas. Dejar la reacción desarrollarse en la oscuridad<br />
durante 30 ± 5 minutos a temperatura ambiente (+18-30°C). Durante esta<br />
incubación, no utilizar el film adhesivo.<br />
13. Detener la reacción enzimática añadiendo 100 µL de la solución de parada<br />
(R10) en cada pocillo. Adoptar la misma secuencia y el mismo ritmo de<br />
distribución que para la solución de revelado.<br />
14. Secar minuciosamente la parte de abajo de las placas. Leer la densidad<br />
óptica a 450/620 nm con ayuda de un lector de placas en los 30 minutos<br />
que siguen a la parada de la reacción. Las tiras deben conservarse siempre<br />
al abrigo de la luz antes de la lectura.<br />
15. Asegurarse, antes de la transcripción de los resultados, de la concordancia<br />
entre la lectura y el plan de distribución de las placas y de las muestras.<br />
8. CÁLCULO E INTERPRETACIÓN DE LOS RESULTADOS<br />
8.1 Cálculo del valor umbral (VU)<br />
El valor umbral VU corresponde a la media de las densidades ópticas (DO) de<br />
los duplicados del Calibrador (R4):<br />

8.2 Cálculo de la Ratio de la Muestra<br />
Los resultados para una muestra dada se expresan bajo forma de una ratio<br />
con ayuda de la fórmula siguiente:<br />

8.3 Control de calidad<br />
Analizar los resultados de DO obtenidos con el calibrador, los sueros de<br />
control en cada microplaca y para cada serie. Para validar la manipulación,<br />
hay que respetar los criterios siguientes:<br />
• Valores de las densidades ópticas:<br />
VU ≥ 0,300<br />
0,80 x VU < DO R4 Repl.1 < 1,20 x VU<br />

(La DO individual de cada uno de los duplicados del Calibrador R4 no debe<br />
separarse más de 20% del valor umbral)<br />

Si estos criterios no se cumplen, volver a empezar la manipulación.<br />
8.4 Interpretación de los resultados<br />

8.5 Posibles causas de error<br />
El origen de las reacciones no validadas o no reproducibles guarda a menudo<br />
en relación con las causas siguientes:<br />
• Lavado insuficiente de las microplacas.<br />
• Contaminación de las muestras negativas por un suero o un plasma que<br />
contenga un título elevado de anticuerpos.<br />

• Contaminación puntual de la solución de revelado por agentes químicos<br />
oxidantes (lejía, iones metálicos, etc.).<br />
• Contaminación puntual de la solución de parada.<br />
9. RESULTADOS<br />
Los resultados de <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> fueron evaluados en 2 centros sobre un<br />
total de 863 muestras de mujeres embarazadas y donantes de sangre.<br />
9.1 Prevalencia<br />
La prevalencia de anticuerpos <strong>IgM</strong> anti-<strong>Toxo</strong> con <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> se<br />
calculó sobre un panel de 500 muestras procedentes de mujeres<br />
embarazadas. 15 muestras eran positivas en anticuerpos <strong>IgM</strong> anti-<strong>Toxo</strong>. La<br />
prevalencia medida con el ensayo <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> está establecida en el<br />
3 % (15/500).<br />
9.2 Especificidad<br />
La especificidad se calculó sobre un panel de 737 muestras negativas con<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> TMB (72751), procedentes de 2 centros situados en<br />
Francia y divididas como sigue:<br />
• 154 sueros de donantes de sangre<br />

La muestra positiva discrepante con <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> fuera confirmada<br />
positiva con método ISAGA.<br />
Además, se probaron 57 muestras sobre un panel de 19 seroconversiones.<br />
Sobre estas 19 seroconversiones, 18 se detectaron de manera comparable y<br />
una seroconversión presentó un desfase de una muestra a favor del método<br />
de referencia.<br />
9.4 Reactividad cruzada<br />
Se analizó un panel de 205 muestras, incluidas 167 muestras positivas en<br />
CMV, rubéola, EBV, HSV, VZV, paperas, sarampión y VIH, y 38 muestras<br />
positivas en factor reumatoide, autoanticuerpos y anticuerpos heterófilos, y<br />
muestras de mieloma con <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> y un ensayo inmunoenzimático<br />
comercial para la detección de anticuerpos <strong>IgM</strong> anti-<strong>Toxo</strong>.<br />
De esas muestras, 2 resultaron positivas y acordes con el kit de ensayo<br />
inmunoenzimático usado para la comparación : 1 muestra positiva en anti-<br />
EBV y 1 muestra positiva en anti-HSV 2 IgG.<br />

9.5 Precisión<br />
• Precisión intra-análisis (repetibilidad):<br />
Con el fin de evaluar la repetibilidad intra-análisis, una muestra negativa y tres<br />
muestras positivas han sido testadas en 32 ocasiones, en una misma serie. La<br />
ratio (DO muestra /VU) se ha determinado para cada muestra. La media de la<br />
ratios, la desviación estándar (DS) y el coeficiente de variación (%CV) para<br />
cada muestra se ofrecen en el cuadro siguiente.<br />

ha determinado para cada muestra. La media de la ratios, la desviación<br />
estándar (DS) y el coeficiente de variación (%CV) para cada muestra se<br />
ofrecen en el cuadro siguiente.<br />

10. LÍMITES DE USO<br />
Sólo puede establecerse el diagnóstico de infección por T. gondii basándose<br />
en una combinación de datos clínicos y biológicos. El resultado de una única<br />
prueba de titulación de anticuerpos <strong>IgM</strong> anti-T. gondii no constituye prueba<br />
suficiente para el diagnóstico de una infección reciente.<br />
• El diagnóstico de una infección reciente sólo puede realizarse con<br />
información completa sobre el paciente que incluya datos clínicos y<br />
biológicos (aumento significativo de anticuerpos IgG anti-T. gondii en<br />
2 sueros del paciente extraídos con un intervalo de 3 semanas y analizados<br />

a la vez, presencia de <strong>IgM</strong> anti-T. gondii en un nivel significativo,<br />
demostración de baja avidez de los IgG).<br />
• La presencia de anticuerpos <strong>IgM</strong> anti-T. gondii no constituye prueba<br />
suficiente para confirmar una infección reciente, ya que los <strong>IgM</strong> pueden<br />
permanecer varios meses o incluso años tras la infección. Cuando se<br />
detectan <strong>IgM</strong>, debería realizarse una determinación cuantitativa de los<br />
anticuerpos IgG anti-T. gondii, así como un seguimiento de la evolución de<br />
los anticuerpos anti-T. gondii en al menos un segundo suero extraído tres<br />
semanas más tarde.<br />
• Si se analiza una muestra demasiado temprano durante una primoinfección<br />
reciente, es posible que los anticuerpos <strong>IgM</strong> anti-T. gondii no estén aún<br />
presentes. Si existen sospechas, debería extraerse y analizarse una<br />
segunda muestra unas 3 semanas más tarde.<br />
11. CONTROL DE CALIDAD DEL FABRICANTE<br />
Todos los productos fabricados y comercializados por la sociedad Bio-Rad<br />
están bajo un sistema de garantía de la calidad desde la recepción de las<br />
materias primas hasta la comercialización de los productos acabados. Cada<br />
lote de producto acabado es objeto de un control de calidad y sólo se<br />
comercializa si cumple con los criterios de aceptación. La documentación<br />
relativa a la producción y al control de cada lote queda en manos del<br />

ODER PLASMA MITTELS ENZYMIMMUNOASSAY
1. VERWENDUNGSZWECK<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> ist ein Immunoassay unter Anwendung des Immunocapture-Prinzips<br />
für den qualitativen Nachweis von <strong>IgM</strong>-Antikörpern gegen<br />
T. gondii in Humanserum oder –plasma.<br />
2. KLINISCHE BEDEUTUNG<br />
T. gondii ist ein Protozoon, das zahlreiche Säugetier- und Vogelarten infizieren<br />
kann. Diese Infektionen treten sehr häufig beim Menschen und bei Tieren auf<br />
und verlaufen oft klinisch ganz symptomlos. Die Prävalenz dieser Infektion in<br />
der Population wird durch das Vorhandensein spezifischer Antikörper im<br />
Humanserum nachgewiesen und variiert je nach Gegend und Alter.<br />
Im Falle einer Primärinfektion bei schwangeren Frauen kann diese Infektion<br />
schwere Folgen für den Fötus (insbesondere zerebrale Funktionsstörungen)<br />
oder sogar Fehlgeburten verursachen. Eine durch den Nachweis von<br />
IgGAntikörpern zu Beginn der Schwangerschaft festgestellte auch lange<br />
zurückliegende Immunität der Mutter schützt den Fötus vor der Infektion durch<br />
diesen Parasiten.<br />
Die zweite durch diese Infektion gefährdete Gruppe sind immunsupprimierte<br />
Patienten, insbesondere AIDS-Patienten. Diese Infektionen sind fast<br />
ausschließlich auf eine aus einem ruhenden parasitären Herd (Zyste)<br />
reaktivierte Infektion, die vor der HIV-Infektion bestand, zurückzuführen. Eine<br />
sichere Diagnose der <strong>Toxo</strong>plasmose-Infektion wird durch den Parasitennachweis<br />
erbracht, aber seine Bestimmung durch eine direkte Untersuchung<br />
ist schwierig, wenn nicht sogar ungewiss. Die Serologie stellt die Grundlage für<br />
Diagnostik und Follow-up der <strong>Toxo</strong>plasmose dar.<br />
Durch den Nachweis spezifischer Antikörper kann eine Infektion mit T. gondii<br />
bestätigt werden. Die gleichzeitige Untersuchung einer Probe auf verschiedene<br />
Antikörper ermöglicht es generell, den Infektionszeitpunktfestzulegen und ggf.<br />
bei neueren Infektionen das therapeutische Vorgehen zu bestimmen bzw. auch<br />
dem Erscheinungsrisiko einer <strong>Toxo</strong>plasmose angemessene Vorbeugungsmaßnahmen<br />
zu empfehlen: hygienisch-diätetische Maßnahmen bei seronegativen<br />
schwangeren Frauen, Chemoprophylaxe bei immunsupprimierten, T. gondiiseropositiven<br />
Patienten.<br />

3. PRINZIP<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> ist ein qualitativer Test zum Nachweis von <strong>IgM</strong>-Antikörpern<br />
gegen <strong>Toxo</strong>plasma gondii in Humanserum oder –plasma mittels<br />
Enzymimmunoassay mit Bindung der <strong>IgM</strong>-Antikörper an die Festphase.<br />
Humane µ-Ketten-Antikörper werden an die Festphase (die Vertiefungen der<br />
Mikrotiterplatte) gebunden. Eine Mischung aus T. gondii-Antigen und mit<br />
Peroxidase markiertem, monoklonalem T. gondii-Antikörper wird als Konjugat<br />
verwendet. Der Test erfolgt in folgenden Schritten:<br />
• Schritt 1<br />
Patientenproben und Kontrollen werden im Verhältnis 1:21 verdünnt und<br />
anschließend in die Vertiefungen der Mikrotiterplatte gegeben. Während der<br />
1-stündigen Inkubationsphase bei 37°C binden die in der Probe vorhandenen<br />
<strong>IgM</strong>-Antikörper an die in den Vertiefungen der Mikrotiterplatte vorhandenen µ-<br />
Ketten-Antikörper. Nach der Inkubation werden ungebundene unspezifische<br />
Antikörper und weitere Serumproteine durch Waschgänge entfernt.<br />
• Schritt 2<br />
Das Konjugat (eine Mischung aus T. gondii-Antigen und mit Peroxidase<br />
markiertem T. gondii-Antikörper) wird in die Vertiefungen der Mikrotiterplatte<br />
gegeben. Während dieser 1-stündigen Inkubation bei 37°C bindet das<br />
Konjugat an die spezifischen T. gondii-<strong>IgM</strong>-Antikörper. Das ungebundene<br />
Konjugat wird am Ende der Inkubation durch Waschgänge entfernt.<br />
• Schritt 3<br />
Durch Zugabe von enzymatischer Entwicklungslösung in jede Vertiefung wird<br />
das Vorliegen von Immunkomplexen (humane µ-Ketten-Antikörper / T. gondii-<br />
<strong>IgM</strong>-Antikörper / T. gondii-Antigen / mit Peroxidase markierter monoklonaler<br />
T. gondii-Antikörper) sichtbar gemacht.<br />
• Schritt 4<br />
Nach einer Inkubationsphase bei Raumtemperatur (+18-30°C) wird die<br />
Enzymreaktion durch Zugabe von 1N Schwefelsäurelösung gestoppt. Die<br />
Extinktion wird durch Ablesen mittels Spektrophotometer bei 450/620 nm<br />
proportional zur Menge an in der Probe vorhandenen <strong>IgM</strong>-Antikörpern gegen<br />
T. gondii ermittelt.<br />

• Die Chromogenlösung (R9) sollte farblos sein. Ist eine Blaufärbung sichtbar,<br />
darf das Reagenz nicht verwendet und muss ersetzt werden.<br />
• Für jede neue Probe die Pipettenspitze wechseln.<br />
• Pipetten und Geräte auf Genauigkeit und korrekte Funktion prüfen.<br />

HYGIENE- UND SICHERHEITSVORSCHRIFTEN<br />
Das Humanmaterial zur Vorbereitung der Reagenzien wurde getestet und als<br />
nicht reaktiv für Hepatitis-B-Oberflächenantigen (HBs Ag), Antikörper gegen<br />
das Hepatitis-C-Virus (Anti-HCV) und gegen die humane Immundefizienz-Virus<br />
(anti-HIV1 und anti-HIV2) befunden. Da keine Testmethode eine potentielle<br />
Infektionsgefahr mit absoluter Sicherheit ausschließen kann, müssen alle<br />
Reagenzien, die Humanmaterial enthalten, und Patientenproben als potentiell<br />
infektiös behandelt werden.<br />
• Alle Materialien einschließlich der Waschlösung, die mit Proben und<br />
Reagenzien, die Humanmaterial enthalten, in Berührung kommen, sollten<br />
als potentiell infektiös betrachtet werden.<br />
• Beim Umgang mit Reagenzien und Proben Einweghandschuhe tragen.<br />
• Nicht mit dem Mund pipettieren.<br />
• Probenspritzer bzw. Spritzer probenhaltiger Lösungen vermeiden. Spritzer<br />
müssen mit 10%iger Natriumhypochloritlösung behandelt werden. Ist die<br />
verunreinigende Lösung eine Säure, die Oberflächen zunächst mit<br />
Natriumbicarbonat neutralisieren, dann mit 10%iger Natriumhypochloritlösung<br />
reinigen und mit Papiertüchern abtrocknen. Das zum Reinigen<br />
verwendete Material ist in einen Behälter für Sondermüll zu geben.<br />
• Die Patientenproben und Reagenzien humanen Ursprungs sowie<br />
kontaminierte Materialien und Produkte dürfen nur nach einer Dekontaminierung<br />
entsorgt werden:<br />
- entweder durch Eintauchen in Natriumhypochloritlösung mit einer<br />
Natriumhypochlorit-Endkonzentration von 5% für die Dauer von<br />
30 Minuten<br />
- oder durch Autoklavieren bei 121°C über mindestens 2 Stunden.<br />
VORSICHT: Natriumhypochlorit-haltige Lösungen nicht in den<br />
Autoklaven stellen.
• Kontakt der Reagenzien, auch der als nicht gefährlich eingestuften<br />
Reagenzien, mit Haut und Schleimhäuten vermeiden.<br />
• Chemikalien und biologische Proben müssen gemäß den GLP-Richtlinien<br />
verwendet und entsorgt werden.<br />
• Alle Reagenzien des Kits sind ausschließlich für die in vitro-Diagnostik<br />
bestimmt.<br />
Vorsicht: Einige Reagenzien enthalten < 1,5% ProClin TM 300.<br />
R43: Sensibilisierung durch Hautkontakt möglich.<br />
S28-37: Bei Berührung mit der Haut sofort mit viel Wasser und<br />
Xi - Reizend Seife abwaschen. Geeignete Schutzhandschuhe tragen.<br />
6. ENTNAHME, VORBEREITUNG UND LAGERUNG DER<br />
PROBEN<br />
1. Als Probenmaterial wird Serum oder Plasma (Heparin, EDTA und Citrat)<br />
empfohlen.<br />
2. Folgende Empfehlungen für die Handhabung, Vorbereitung und Lagerung<br />
der Blutproben beachten:<br />
• Alle Blutproben unter Beachtung der üblichen Vorsichtsmaßnahmen für<br />
Venenpunktion entnehmen.<br />
• Serumproben vor dem Zentrifugieren vollständig gerinnen lassen.<br />
• Probenröhrchen stets verschlossen halten.<br />
• Nach dem Zentrifugieren Serum oder Plasma vom Blutkuchen bzw. den<br />
Erythrozyten trennen und in einem fest verschlossenen Röhrchen<br />
aufbewahren.<br />
• Die Proben können bei +2-8°C aufbewahrt werden, sofern sie innerhalb<br />
von 7 Tagen getestet werden.<br />
• Wird der Test nicht innerhalb von 7 Tagen durchgeführt oder bei Versand<br />
der Proben, die Proben bei mind. -20°C einfrieren.<br />
• Keine Proben verwenden, die mehr als driemal eingefroren und wieder<br />
aufgetaut wurden. Zuvor tiefgefrorene Proben sollten nach dem Auftauen<br />
und vor der Analyse gründlich gemischt werden (Vortex).<br />
3. Proben mit einer Albuminkonzentration bis 90 g/l oder einer Konzentration<br />
an ungebundenem Bilirubin bis 100 mg/l, lipämische Proben mit einer<br />
entsprechenden Triolein-(Triglyzerid-)-Konzentration bis 36 g/l und<br />
hämolytische Proben mit einer Hämoglobinkonzentration bis 10 g/l<br />
beeinträchtigen das Testergebnis nicht.<br />
4. Proben nicht erhitzen.<br />
7. TESTVERFAHREN<br />

7.2 Rekonstitution der Reagenzien<br />
• R1: Vor dem Öffnen den Beutel 30 Minuten bei Raumtemperatur (+18-30°C)<br />
aufbewahren. Den Halterahmen und die benötigte Anzahl an Streifen<br />
entnehmen und die unbenutzten Streifen unverzüglich wieder in den Beutel<br />
stecken. Überprüfen Sie, ob Trockenmittel im Beutel vorhanden ist. Den<br />
Beutel wieder sorgfältig verschließen und bei +2-8°C lagern.<br />
• R2: Die Waschlösung R2 im Verhältnis 1:20 mit destilliertem Wasser<br />
verdünnen: Um die gebrauchsfertige Waschlösung herzustellen, zum<br />
Beispiel 50 mL R2 mit 950 mL destilliertem Wasser verdünnen. Wenn<br />
manuell gewaschen wird, 350 mL verdünnte Waschlösung für eine Platte<br />
mit 12 Streifen vorbereiten.<br />
• R3, R4, R5: Im Verhältnis 1:21 mit Verdünnungsmittel R7 verdünnen<br />
(Beispiel: 15 µL R3 + 300 µL R7).<br />
• R6a: Lyophilisiertes T. gondii-Antigen. Zur Testung von 6 Streifen eine<br />
Ampulle mit lyophilisiertem Antigen durch Zugabe von 14 mL<br />
Verdünnungsmittel (R7) rekonstituieren. Gründlich mischen. Die verdünnte<br />
Antigenlösung (R6a+R7) muss vollkommen klar sein.<br />
• R6 (R6a+R6b) – Konjugatarbeitslösung : In jedes Gefäß mit rekonstituiertem<br />
T. gondii-Antigen (verdünnte R6a) 140 µL unbehandeltes Konjugat (R6b)<br />
geben. Gründlich mischen. Die Konjugatarbeitslösung muss mindestens<br />
1 Stunde vor dem Gebrauch hergestellt werden.<br />

7.3 Lagerung und Haltbarkeit geöffneter bzw. rekonstituierter<br />
Reagenzien<br />
Das Kit bei +2-8°C lagern. Bei dieser Lagertemperatur können alle<br />
Kitkomponenten bis zu dem auf der Packung angegebenen Verfallsdatum<br />
verwendet werden.<br />
• R1: Nach dem Öffnen sind die Streifen 8 Wochen haltbar, sofern sie in dem<br />
sorgfältig wieder verschlossenen Originalbeutel bei +2-8°C aufbewahrt<br />
werden (überprüfen Sie, ob Trockenmittel enthalten ist).<br />
• R2: Nach der Verdünnung kann die Waschlösung bei +2-30°C 2 Wochen<br />
lang aufbewahrt werden. Die konzentrierte Waschlösung kann bei +2-30°C<br />
ohne Kontamination bis zu dem auf dem Etikett angegebenen Verfallsdatum<br />
aufbewahrt werden.<br />
• R3, R4, R5, R6b, R7: Nach dem Öffnen sind die bei +2-8°C gelagerten<br />
Reagenzien ohne Kontamination bis zu 8 Wochen haltbar.<br />
• R6 (R6a+R6b): Die rekonstituierte Konjugatarbeitslösung ist bei<br />
Raumtemperatur (+18-30°C) 8 Stunden bei +2-8°C.<br />
• R9: Nach dem Öffnen ist das bei +2-8°C gelagerte Reagenz ohne<br />
Kontamination bis zu 8 Wochen haltbar.<br />
• R10: Nach dem Öffnen ist das bei +2-8°C gelagerte Reagenz ohne<br />
Kontamination bis zu dem auf dem Etikett angegebenen Verfallsdatum<br />
haltbar.<br />
7.4 Testdurchführung<br />
Die vorliegende Beschreibung und die GLP-Richtlinien sind strikt zu befolgen.<br />
Die Reagenzien vor der Verwendung auf Raumtemperatur (+18-30°C) bringen.<br />
Die Verwendung von abknickbaren Vertiefungen erfordert während der<br />
Handhabung besondere Aufmerksamkeit.<br />
Zur Validierung der Testergebnisse sollten die Kalibrator, die negative und die<br />
positive-Kontrolle in jedem Testansatz mitgeführt werden.<br />
1. Probenverteilung und Identifikationsplan für Kontrollen und Patientenproben<br />
sorgfältig festlegen.<br />
2. Die verdünnte Waschlösung (R2) vorbereiten (siehe Abschnitt 7.2).<br />
3. Den Halterahmen und die Streifen (R1) aus der Schutzhülle nehmen (siehe<br />
Abschnitt 7.2).<br />
4. Die Konjugatarbeitslösung vorbereiten R6 (R6a+R6b)(siehe Abschnitt 7.2).<br />
5. Die Kalibrator (R4), die Kontrollen (R3, R5) und die Patientenproben (S1,<br />
S2...) in Verdünnungsmittel (R7) zu einem 1:21-Verhältnis verdünnen: 15 µL<br />
Probe und 300 µL Verdünnungsmittel (R7)(siehe Abschnitt 7.2). Die<br />
verdünnten Proben auf dem Vortex mischen.<br />
6. Die nachfolgend angegebene Reihenfolge unbedingt einhalten. In jede<br />
Vertiefung 200 µL verdünnte Kalibrator, Kontrollen und Patientenproben<br />
geben:<br />

7. Die Mikrotiterplatte mit Folie abdecken und fest auf die Platte drücken,<br />
damit sie dicht versiegelt ist. Die Mikrotiterplatte in einem Wasserbad mit<br />
Temperaturregelung oder in einem Mikrotiterplatten-Inkubator bei 37°C<br />
± 1°C eine Stunde ± 5 Minuten inkubieren.<br />
8. Nach Beendigung der ersten Inkubationsphase die Klebefolie entfernen.<br />
Den Inhalt aller Vertiefungen in einen Behälter für infektiöse Abfälle (mit<br />
Natriumhypochlorit) absaugen. Die Mikrotiterplatte viermal mit 350 µL<br />
Waschlösung (R2) waschen. Die Mikrotiterplatte umdrehen und vorsichtig<br />
mit Papiertüchern abtupfen, um die restliche Flüssigkeit zu entfernen.<br />
9. 200 µL der Konjugatarbeitslösung (R6) in alle Vertiefungen geben. Vor der<br />
Verwendung muss die Lösung vorsichtig geschüttelt werden.<br />
10. Die Mikrotiterplatte mit Folie abdecken und fest auf die Platte drücken,<br />
damit sie dicht versiegelt ist. Die Mikrotiterplatte in einem Wasserbad mit<br />
Temperaturregelung oder in einem Mikrotiterplatten-Inkubator bei 37°C<br />
± 1°C eine Stunde ± 5 Minuten inkubieren.<br />
11. Nach Beendigung der zweiten Inkubationsphase die Klebefolie entfernen.<br />
Den Inhalt aller Vertiefungen in einen Behälter für infektiöse Abfälle (mit<br />
Natriumhypochlorit) absaugen. Die Mikrotiterplatte viermal mit 350 µL<br />
Waschlösung (R2) waschen. Die Mikrotiterplatte umdrehen und vorsichtig<br />
mit Papiertüchern abtupfen, um die restliche Flüssigkeit zu entfernen.<br />
12. 200 µL Chromogenlösung (R9) schnell und unter Lichtausschluss in alle<br />
Vertiefungen pipettieren. Für die Entwicklung der Reaktion die Platte<br />
lichtgeschützt bei Raumtemperatur (+18-30°C) 30 ± 5 Minuten stehen<br />
lassen. Während dieser Inkubation keine Folie verwenden.<br />
13. Die enzymatische Reaktion durch Zugabe von 100 µL Stopplösung (R10) in<br />
jede Vertiefung stoppen. In der gleichen Reihenfolge und im gleichen<br />
Zeitrahmen wie die Entwicklungslösung verteilen.<br />

14. Den Boden der Platte sorgfältig abwischen. Innerhalb von 30 Minuten nach<br />
Stoppen der Reaktion die Extinktion mit einem Plattenleser bei 450/620 nm<br />
ablesen. Die Streifen müssen vor dem Ablesen stets lichtgeschützt<br />
aufbewahrt werden.<br />
15. Vor dem Weiterleiten alle Ergebnisse hinsichtlich der Übereinstimmung<br />
zwischen den Werten sowie gegen die Platten- und Probenverteilung<br />
überprüfen.<br />
8. BERECHNUNG UND AUSWERTUNG DER ERGEBNISSE<br />
8.1 Berechnung des Grenzwerts (GW)<br />
Der Grenzwert (GW) entspricht der mittleren Extinktion (ΔE) der Kalibrator (R4)<br />
in Doppelbestimmung:<br />

8.2 Berechnung des Probenverhältnisses<br />
Das Ergebnis der Probe wird ausgedrückt als Verhältnis, unter Anwendung<br />
folgender Formel:<br />

8.3 Qualitätskontrolle<br />
Für jede Mikrotiterplatte und jede Testreihe alle Kalibrator und Kontrollen<br />
mitführen und die ermittelten Ergebnisse analysieren. Für die Validierung des<br />
Tests müssen folgende Kriterien erfüllt werden:<br />
• Extinktionswerte:<br />

8.5 Fehlerursachen<br />
Nicht validierte oder nicht reproduzierbare Reaktionen haben häufig folgende<br />
Ursachen:<br />

Die Leistung des <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> wurde an 2 Standorten mit insgesamt<br />
863 Proben von Schwangeren und Blutspendern ermittelt.<br />
9.1 Prävalenz<br />
Die Prävalenz von anti-<strong>Toxo</strong>-<strong>IgM</strong>-Antikörper mit dem <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong><br />
wurde mit einem Panel von 500 Proben von Schwangeren untersucht.<br />
15 Proben waren positiv für anti-<strong>Toxo</strong>-<strong>IgM</strong>-Antikörper. Die mit dem <strong>Platelia</strong><br />
<strong>Toxo</strong> <strong>IgM</strong>-Assay bestimmte Prävalenz lag demnach bei 3% (15/500).<br />
9.2 Spezifität<br />
Die Spezifität wurde mit einem Panel von 737 Proben mit <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong><br />
TMB (72751), an 2 Standorten in Frankreich ermittelt. Folgende Proben wurden<br />

Desweiteren wurden 57 Seren von 19 Serokonversionen getestet. Hiervon<br />
wurden 18 Serokonversionen bestätigt, 1 Serokonversion zeigte eine<br />
Abweichung zu den ergebnissen der Referenzmethode.<br />

9.4 Kreuzreaktivität<br />
Ein Panel von 205 Proben, darunter 167 für CMV, Röteln, EBV, HSV, VZV,<br />
Mumps, Masern oder HIV - positive Proben sowie 38 Rheumafaktor-,<br />
Autoantikörper- heterophile Antikörper – und Myeloma - positive Proben,<br />
wurde mit <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> und einem handelsüblichen <strong>Toxo</strong> <strong>IgM</strong> - EIA<br />
untersucht. Zwei Seren zeigten in beiden Testsystemen ein positives<br />
Testergebnis : eine anti-EBV Ak. Positives Serum und ein anti-HSV 2 IgG<br />
positives Serum.<br />
9.5 Präzision<br />
• Intraassay-Präzision (Reproduzierbarkeit):<br />
Zur Bestimmung der Intraassay-Reproduzierbarkeit wurden 1 negative und<br />
3 positive Proben 32mal in einer Messreihe getestet. Das Verhältnis (Extinktion<br />
der Probe/Grenzwert) wurde für jede Probe bestimmt. In der nachfolgenden<br />
Tabelle sind für alle vier Proben jeweils das mittlere Verhältnis, die<br />

10. GRENZEN DES VERFAHRENS<br />
Die Diagnose einer T. gondii-Infektion kann nur auf Basis einer Kombination<br />
von klinischen und biologischen Daten gestellt werden. Das Ergebnis einer<br />
einzelnen Titration von Anti-T. gondii-<strong>IgM</strong>-Antikörpern ist als Beweis zur<br />
Diagnose einer kürzlichen Infektion mit T. gondii nicht ausreichend.<br />
• Eine kürzlich erfolgte Infektion kann nur auf der Basis vollständiger<br />
Patienteninformationen diagnostiziert werden, einschließlich klinischer und<br />
biologischer Daten (signifikante Erhöhung von Anti-T. gondii-IgG-<br />
Antikörpern in 2 Patientenseren, die im Abstand von 3 Wochen<br />
abgenommen und im selben Lauf getestet werden, Vorhandensein von Anti-<br />
T. gondii-<strong>IgM</strong> mit signifikantem Titer, Nachweis einer niedrigen IgG-Avidität).<br />
• Das Vorhandensein von Anti-T. gondii-<strong>IgM</strong>-Antikörpern ist kein<br />
ausreichender Beweis zur Bestätigung einer kürzlich erfolgten Infektion, da<br />
<strong>IgM</strong> mehrere Monate oder sogar Jahre nach der Infektion persistieren.<br />
Werden <strong>IgM</strong> nachgewiesen, empfiehlt sich eine quantitative Ermittlung der<br />
Anti-T. gondii-IgG-Antikörper sowie eine Nachbestimmung der Entwicklung<br />
von Anti-T. gondii-<strong>IgM</strong>-Antikörpern mit mindestens einer zweiten<br />
Serumprobe, die drei Wochen später entnommen wird.<br />
• Wird eine Probe zu früh während einer kürzlich erfolgten Primärinfektion<br />
getestet, sind möglicherweise noch keine Anti-T. gondii-<strong>IgM</strong>-Antikörper<br />
vorhanden. Liegt ein entsprechender Verdacht vor, ist 3 Wochen später eine<br />
zweite Probe zu entnehmen, mit der die Testung auf <strong>IgM</strong>-Antikörper<br />
wiederholt wird.<br />
11. QUALITÄTSKONTROLLE DES HERSTELLERS<br />
Alle von der Firma Bio-Rad hergestellten Reagenzien unterliegen einem<br />
Qualitätssicherungssystem vom Rohstoffeingang bis zur Vermarktung der<br />
Fertigprodukte. Jede Fertigproduktcharge wird einer Qualitätskontrolle<br />
unterzogen und nur dann verkauft, wenn sie den Freigabekriterien entspricht.<br />

<strong>IgM</strong> ANTI-TOXOPLASMA GONDII NEL SIERO O NEL<br />
PLASMA UMANO MEDIANTE DOSAGGIO<br />
IMMUNOENZIMATICO
1. USO PREVISTO<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> è un dosaggio che utilizza il metodo ad immunocattura per<br />
la determinazione qualitativa degli anticorpi <strong>IgM</strong> anti-<strong>Toxo</strong>plasma gondii nel<br />
siero o nel plasma umano.<br />
2. INTERESSE CLINICO<br />
T. gondii è un protozoo in grado di infettare numerose specie d mammiferi e<br />
uccelli. Questa infezione comune nell'uomo e negli animali, spesso non dà<br />
manifestazioni dal punto di vista clinico. La prevalenza dell'infezione nella<br />
popolazione, rilevata dalla presenza di anticorpi specifici nel siero varia in<br />
funzione della zona geografica e dell'età. L'infezione può, nel caso di infezione<br />
primaria della madre durante la gravidanza, essere causa di gravi conseguenze<br />
per il feto (in particolare un'alterazione delle funzioni cerebrali) o di aborto.<br />
Un'immunità pregressa della madre, dimostrata dalla presenza di anticorpi IgG<br />
fin dall'inizio della gravidanza, protegge il feto dall'infezione da parte del<br />
parassita.<br />
Una popolazione sensibile a questo tipo di infezione è costituita dai pazienti<br />
immunodepressi, quali i malati affetti da AIDS. In tal caso essa è dovuta, quasi<br />
esclusivamente, lla riattivazione di un focolaio parassitario (ciste) quiescente<br />
nel paziente e preesistente all'infezione da virus HIV. La diagnosi certa di<br />
infezione da toxoplasma si ottiene attraverso l'evidenziazione del parassita,<br />
tuttavia l’isolamento risulta difficile e raro. La sierologia rappresenta la base<br />
della diagnosi e dello screening della toxoplasmosi. La rilevazione di anticorpi<br />
specifici è indice di esposizione a da T. gondii; lo studio combinato di anticorpi<br />
appartenenti a diversi isotopi consente generalmente di datare l'infezione e<br />
orientare la terapia in caso di infezione recente, oppure di proporre misure<br />
profilattiche idonee al rischio di insorgenza di una toxoplasmosi: misure<br />
igienicodietetiche, per le donne in gravidanza non immuni, chemioprofilassi per<br />
i soggetti immunodepressi sieropositivi per il T. gondii.<br />

3. PRINCIPIO<br />
<strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> è un test qualitativo per la determinazione degli anticorpi<br />
<strong>IgM</strong> anti-<strong>Toxo</strong>plasma gondii nel siero o nel plasma umano mediante<br />
immunodosaggio enzimatico con cattura degli anticorpi <strong>IgM</strong> in fase solida.<br />
Gli anticorpi anti-catene µ umane sono adesi alla fase solida (pozzetti della<br />
micropiastra). Come coniugato viene utilizzata una miscela dell'antigene<br />
T. gondii e dell'anticorpo monoclonale anti-antigene T. gondii marcato con<br />
perossidasi. Il test prevede le seguenti fasi:<br />
• Fase 1<br />
I campioni dei pazienti, il calibratore e i controlli vengono diluiti in rapporto<br />
1/21, quindi distribuiti nei pozzetti della micropiastra. Durante questa<br />
incubazione di un'ora a 37°C, gli anticorpi <strong>IgM</strong> presenti nel campione si legano<br />
agli anticorpi anti-µ adesi ai pozzetti della micropiastra. Gli anticorpi non<br />
specifici non legati e altre proteine seriche vengono eliminati dai lavaggi<br />
successivi all'incubazione.<br />
• Fase 2<br />
Il coniugato (miscela di antigene T. gondii e anticorpo monoclonale anti-<br />
T. gondii marcato con perossidasi) viene aggiunto nei pozzetti della<br />
micropiastra. Durante questa incubazione di un'ora a 37°C, il coniugato si lega<br />
agli anticorpi <strong>IgM</strong> specifici anti-T. gondii. Il coniugato non legato viene<br />
eliminato dai lavaggi successivi all'incubazione.<br />
• Fase 3<br />
La presenza di immunocomplessi (Anti-catene µ umane / <strong>IgM</strong> anti-T. gondii /<br />
Antigene T. gondii / anticorpo anti-T. gondii marcato con perossidasi) viene<br />
dimostrata attraverso l'aggiunta di una soluzione di sviluppo enzimatica in ogni<br />
pozzetto.<br />
• Fase 4<br />
Al termine del periodo di incubazione a temperatura ambiente (18-30°C), la<br />
reazione enzimatica viene bloccata attraverso l'aggiunta di una soluzione di<br />
acido solforico 1N. La lettura della densità ottica ottenuta con uno<br />
spettrofotometro impostato su 450/620 nm è proporzionale alla quantità di<br />
anticorpi <strong>IgM</strong> anti-T. gondii presenti nel campione.<br />

• La soluzione di cromogeno (R9) deve essere incolore. La colorazione blu<br />
indica che il reagente non può essere utilizzato, pertanto dovrà essere<br />

• Utilizzare puntali diversi per ogni campione.<br />
• Verificare l'accuratezza delle pipette e il buon funzionamento delle altre<br />
strumentazioni.<br />

ISTRUZIONI DI SICUREZZA E IGIENE<br />
Il materiale di origine umano utilizzato nella preparazione dei reagenti è stato<br />
analizzato e classificato non reattivo per l'antigene di superficie dell'epatite B<br />
(HBs Ag), gli anticorpi per il virus dell'epatite C (anti-HCV) e i virus dell'immunodeficienza<br />
umana (anti-HIV1 e anti-HIV2). Dato che nessun metodo può<br />
garantire con assoluta certezza l'assenza di agenti infettivi, manipolare i<br />
reagenti di origine umana e i campioni dei pazienti come potenzialmente infetti.<br />
• Qualsiasi materiale, comprese le soluzioni di lavaggio, che entri direttamente<br />
in contatto con campioni e reagenti contenenti materiali di origine umana<br />
deve essere considerato potenzialmente in grado di trasmettere malattie<br />
infettive.<br />
• Indossare guanti monouso durante la manipolazione dei campioni e dei<br />
reagenti.<br />
• Non pipettare con la bocca.<br />
• Evitare di rovesciare campioni o soluzioni contenenti campioni. Pulire le<br />
superfici contaminate con candeggina diluita al 10%. Se il liquido<br />
contaminante è un acido, neutralizzare le superfici con bicarbonato di sodio,<br />
quindi pulire con candeggina diluita al 10% e asciugare con carta<br />
assorbente. Il materiale utilizzato per la pulizia deve essere gettato in un<br />
contenitore speciale per rifiuti contaminati.<br />
• Dopo la decontaminazione, eliminare i campioni dei pazienti, i reagenti<br />
contenenti materiale di origine umana, compreso il materiale e i prodotti<br />
contaminati mediante uno dei seguenti metodi:<br />
- immersione nella candeggina alla concentrazione finale di 5% di<br />
ipocloruro di sodio per 30 minuti,<br />
- oppure lavaggio in autoclave a 121°C per almeno 2 ore.<br />
ATTENZIONE: non introdurre soluzioni contenenti ipocloruro di sodio<br />
nell'autoclave
• Evitare qualsiasi contatto dei reagenti, compresi quelli considerati non<br />
pericolosi, con la pelle e le mucose.<br />
• La manipolazione e l'eliminazione dei residui chimici e biologici devono<br />
essere eseguite attenendosi alle buone prassi di laboratorio. Tutti i reagenti<br />
forniti nel kit sono destinati esclusivamente all’uso diagnostico in vitro.<br />
Attenzione: alcuni reagenti contengono ProClin 300 < 1,5%<br />
R43: Può provocare sensibilizzazione per contatto con la pelle<br />
S28-37: In caso di contatto con la pelle, lavarsi immediatamente e<br />
Xi - Irritante abbondantemente con acqua e sapone. Indossare guanti adatti<br />
6. PRELIEVO, PREPARAZIONE E CONSERVAZIONE DEI<br />
CAMPIONI<br />
1. Il siero e il plasma (EDTA, eparina o citrato) sono i tipi di campione<br />
raccomandati.<br />
2. Per la manipolazione, l'elaborazione e la conservazione dei campioni<br />
ematici, attenersi alle seguenti raccomandazioni:<br />
• Prelevare tutti i campioni di sangue secondo le precauzioni in uso.<br />
• Per il siero, consentire la completa coagulazione dei campioni prima di<br />
procedere alla centrifugazione.<br />
• Assicurarsi che le provette siano sempre chiuse.<br />
• Dopo la centrifugazione, separare il siero o il plasma dal coagulo o dai<br />
globuli rossi e conservarlo in una provetta chiusa ermeticamente.<br />
• È possibile conservare i campioni a una temperatura compresa fra 2 e<br />
8°C a condizione che il test venga eseguito entro 7 giorni.<br />
• Se il test non viene eseguito entro 7 giorni, o per motivi di consegna,<br />
congelare i campioni a una temperatura di -20°C o inferiore.<br />
• Non utilizzare campioni scongelati più di 3 volte. I campioni<br />
precedentemente congelati devono essere miscelati bene (Vortex) dopo<br />
lo scongelamento e prima del test.<br />
3. I campioni contenenti 90 g/l di albumina o 100 mg/l di bilirubina non<br />
coniugata, i campioni lipemici contenenti l'equivalente di 36 g/l di trioleina<br />
(trigliceride) e i campioni sottoposti ad emolisi contenenti fino a 10 g/l di<br />
emoglobina non influenzano i risultati.<br />

7.2 Ricostituzione dei reagenti<br />
• R1: Prima di aprire la bustina di plastica, lasciare 30 minuti a temperatura<br />
ambiente (+18-30°C). Estrarre il vassoio, riporre immediatamente le strip<br />
non utilizzate nella bustina e verificare la presenza di essiccante. Richiudere<br />
con cura la bustina e conservarla a +2-8°C.<br />
• R2: Diluire in rapporto 1/20 la soluzione di lavaggio R2 in acqua distillata: ad<br />
esempio 50 mL di R2 e 950 mL di acqua distillata per ottenere la soluzione<br />
di lavaggio pronta per l'uso. Preparare 350 mL di soluzione di lavaggio<br />
diluita per una piastra da 12 strip in caso di lavaggio manuale.<br />
• R3, R4, R5: Diluire in rapporto 1/21 nel Diluente (R7) (esempio: 15 µL di R3<br />
+ 300 µL di R7).<br />
• R6a: L’antigene T. gondii è liofilizzato. Per l’elaborazione di 6 strip,<br />
ricostituire un flacone di antigene liofilizzato aggiungendo 14 mL di Diluente<br />
(R7). Miscelare bene. Dopo la diluizione, la soluzione antigenica (R6a+R7)<br />
deve essere perfettamente limpida.<br />
• R6 (R6a+R6b) - Soluzione di lavoro del coniugato: Aggiungere 140 µL di<br />
coniugato (R6b) in ogni flacone di antigene T. gondii ricostituito (R6a diluito).<br />
Miscelare bene. La soluzione di lavoro del coniugato deve essere ricostituita<br />
almeno 1 ora prima dell’uso.<br />
7.3 Conservazione e validità dei reagenti aperti e / o ricostituiti<br />
Il kit deve essere conservato a 2-8°C. Se viene conservato a 2-8°C prima<br />
dell'apertura, ogni componente può essere utilizzato fino alla data di scadenza<br />
indicata sull'etichetta riportata sul kit.<br />
• R1: Dopo l'apertura, le strip mantengono la stabilità fino a 8 settimane, se<br />
conservate a 2-8°C nella stessa bustina sigillata (verificare la presenza di<br />
essiccante).
• R2: Dopo la diluizione, la Soluzione di lavaggio può essere conservata per<br />
2 settimane a 2-30°C. La Soluzione di lavaggio concentrata conservata a<br />
2-30°C, in assenza di contaminazione, mantiene la stabilità fino alla data di<br />
scadenza indicata sull'etichetta.<br />
• R3, R4, R5, R6b, R7: Dopo l'apertura e in assenza di contaminazione, i<br />
reagenti conservati a 2-8°C mantengono la stabilità fino a 8 settimane.<br />
• R6 (R6a+R6b): Dopo la ricostituzione, la soluzione di lavoro del coniugato<br />
mantiene la stabilità per 8 ore a temperatura ambiente (18-30°C) e<br />
2 settimane a +2-8°C.<br />
• R9: Dopo l'apertura e in assenza di contaminazione, i reagenti conservati a<br />
2-8°C mantengono la stabilità fino a 8 settimane.<br />
• R10: Dopo l'apertura e in assenza di contaminazione, il reagente conservato<br />
a 2-8°C mantiene la stabilità fino alla data di scadenza indicata<br />
sull'etichetta.<br />
7.4 Procedura<br />
Seguire attentamente la procedura descritta di seguito e le buone prassi di<br />
laboratorio.<br />
Prima dell'uso, consentire al reagente di raggiungere la temperatura ambiente<br />
(+18-30°C).<br />
Se si utilizzano pozzetti divisibili, prestare particolare attenzione durante la<br />
manipolazione.<br />
Utilizzare il calibratore, i controlli negativi e positivi in ogni seduta per<br />
convalidare i risultati del test.<br />
1. Definire accuratamente il piano di distribuzione e di identificazione per il<br />
calibratore, i controlli e i campioni dei pazienti.<br />
2. Preparare la Soluzione di lavaggio diluita (R2) [Fare riferimento alla Sezione 7.2].<br />
3. Estrarre il vassoio e le strip (R1) dall'involucro protettivo [Fare riferimento alla<br />
Sezione 7.2].<br />
4. Preparare la soluzione di lavoro del coniugato R6 (R6a+R6b) [Fare riferimento<br />
alla Sezione 7.2].<br />
5. Diluire il calibratore (R4), i controlli (R3, R5) e i campioni dei pazienti (S1,<br />
S2…) nel Diluente (R7) per ottenere una diluizione in rapporto 1/21: 15 µL di<br />
campione e 300 µL di Diluente (R7) [Fare riferimento alla Sezione 7.2].<br />
Vortexare i campioni diluiti.<br />
6. Distribuire in ogni pozzetto 200 µL di calibratore, di controlli diluiti e di<br />
campioni dei pazienti secondo lo schema riportato di seguito:<br />

7. Ricoprire la micropiastra con la pellicola sigillante adesiva ed esercitare<br />
pressione per assicurarne la tenuta. Incubare immediatamente la<br />
micropiastra in un bagnetto termostatato o in un incubatore a secco per<br />
1 ora ± 5 minuti a 37°C ± 1°C.<br />
8. Al termine del primo periodo di incubazione, rimuovere il nastro sigillante<br />
adesivo. Aspirare il contenuto di tutti i pozzetti in un contenitore per rifiuti<br />
biologici (contenente ipocloruro di sodio). Lavare la micropiastra 4 volte con<br />
350 µL di Soluzione di lavaggio (R2). Capovolgere la micropiastra e<br />
picchiettare delicatamente su carta assorbente per rimuovere il liquido in<br />
eccesso.<br />
9. Distribuire 200 µL della soluzione di lavoro del coniugato (R6) in tutti i<br />
pozzetti. Agitare leggermente la soluzione prima dell'uso.<br />
10. Ricoprire la micropiastra con la pellicola sigillante adesiva ed esercitare<br />
pressione per assicurarne la tenuta. Incubare immediatamente la<br />
micropiastra in un bagnetto termostatato o in un incubatore a secco per<br />
1 ora ± 5 minuti a 37°C ± 1°C.<br />
11. Al termine del secondo periodo di incubazione, rimuovere il nastro sigillante<br />
adesivo. Aspirare il contenuto di tutti i pozzetti in un contenitore per rifiuti<br />
biologici (contenente ipocloruro di sodio). Lavare la micropiastra 4 volte con<br />
350 µL di Soluzione di lavaggio (R2). Capovolgere la micropiastra e<br />
picchiettare delicatamente su carta assorbente per rimuovere il liquido in<br />
eccesso.<br />
12. Distribuire rapidamente in ogni pozzetto e al riparo dalla luce 200 µL di<br />
Cromogeno (R9). Far sviluppare la reazione al buio per 30 ± 5 minuti a<br />
temperatura ambiente (18-30°C). Non utilizzare nastri sigillanti adesivi<br />
durante questo periodo di incubazione.<br />

13. Bloccare la reazione enzimatica aggiungendo 100 µL di Soluzione bloccante<br />
(R10) in ogni pozzetto. Utilizzare la stessa sequenza e lo stesso ritmo di<br />
distribuzione utilizzati per la soluzione di sviluppo.<br />
14. Asciugare accuratamente il fondo della piastra. Leggere la densità ottica a<br />
450/620 nm mediante un lettore nei 30 minuti successivi alla reazione. Non<br />
esporre le strip alla luce prima della lettura.<br />
15. Prima della trascrizione dei risultati, verificare la corrispondenza fra la lettura<br />
e il piano di distribuzione delle piastre e dei campioni.<br />
8. CALCOLO E INTERPRETAZIONE DEI RISULTATI<br />
8.1 Calcolo del valore soglia (cut-off) (CO)<br />
Il valore di Cut-Off (CO) corrisponde al valore medio delle densità ottiche (DO)<br />
dei duplicati del Calibratore (R4):<br />
CO = media di DO R4<br />
8.2 Calcolo del rapporto Campione<br />
Il risultato per un campione viene espresso sotto forma di rapporto mediante la<br />
seguente formula:<br />
Rapporto Campione = DO campione/CO<br />
8.3 Controllo di qualità<br />
Includere il calibratore e tutti i controlli in ogni micropiastra e per ogni seduta di<br />
lavoro e analizzare i risultati ottenuti. Per la validazione del test, è necessario<br />
che siano soddisfatti i seguenti criteri:<br />
• Valori delle densità ottiche:<br />
CO ≥ 0,300<br />
0,80 x CO < DO R4 Duplicato 1 < 1,20 x CO<br />
0,80 x CO < DO R4 Duplicato 2 < 1,20 x CO<br />
(La singola DO di ogni duplicato del calibratore (R4) non deve differire più del<br />
20% dal valore CO).<br />
• Rapporti delle densità ottiche:<br />
Rapporto R3 (DO R3 / CO) ≤ 0,30<br />
Rapporto R5 (DO R5 / CO) ≥ 1,80<br />
Se non vengono rispettati i criteri del controllo di qualità, la seduta analitica<br />
dovrà essere ripetuta.<br />

8.4 Interpretazione dei risultati<br />

8.5 Guida alla risoluzione dei problemi<br />
Le reazioni non convalidate o non ripetibili spesso sono causate da:<br />
• Lavaggi della micropiastra inadeguati.<br />
• Contaminazione dei campioni negativi tramite siero o plasma con alto titolo<br />
di anticorpi.<br />
• Contaminazione della soluzione di sviluppo tramite agenti chimici ossidanti<br />
(candeggina, ioni metallici...).<br />
• Contaminazione della Soluzione bloccante.<br />
9. EFFICACIA<br />
L’efficacia del kit <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> è stata valutata in 2 siti su un totale di<br />
863 campioni di donne in gravidanza e donatori di sangue.<br />
9.1 Prevalenza<br />
La prevalenza degli anticorpi <strong>IgM</strong> anti-<strong>Toxo</strong> mediante il test <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong><br />
è stata determinata su di un pannello di 500 campioni di donne in gravidanza.<br />
15 campioni sono risultati positivi per gli anticorpi <strong>IgM</strong> anti-<strong>Toxo</strong>. La prevalenza<br />
determinata mediante il test <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> si attesta intorno al 3%<br />
(15/500).<br />
9.2 Specificità<br />
La specificità è stata determinata su un di un pannello di 737 campioni positivi<br />
mediante el test <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> TMB (72751), provenienti da 2 siti in<br />
Francia e ripartiti come segue:<br />

stati provati. Su queste 19 séroconversions, 18 sono stati individuati in modo<br />
comparabile ed un séroconversion ha presentato una differenza di un prelievo<br />
per il metodo di riferimento.<br />

9.4 Reattività crociata<br />
Un pannello di 205 campioni costituito da 167 campioni positivi per i marker<br />
CMV, Rosolia, EBV, HSV, VZV, orecchioni, morbillo e HIV e 38 campioni positivi<br />
per il fattore reumatoide, auto-anticorpi e anticorpi eterofili e campioni di<br />
myeloma è stato testato con il test <strong>Platelia</strong> <strong>Toxo</strong> <strong>IgM</strong> e un test EIA per lo<br />
screening degli anticorpi <strong>IgM</strong> anti-<strong>Toxo</strong>.<br />
Fra questi campioni, 2 sono risultati positivi e concordanti con gli esiti del kit<br />
EIA utilizzato per il confronto: 1 campione positivo in anti-EBV e 1 campione<br />
positivo in anti-HSV 2 IgG .<br />
9.5 Precisione<br />
• Precisione intra-saggio (ripetibilità):<br />
Al fine di valutare la ripetibilità intra-saggio, un campione negativo e tre<br />
campioni positivi sono stati analizzati 32 volte durante la stessa seduta<br />
analitica. Il rapporto (DO campione / CO) è stato determinato per ciascun<br />
campione. La tabella riportata di seguito fornisce la media dei rapporti, la<br />
deviazione standard (DS) e il coefficiente di variazione (%CV) per ciascuno dei<br />

10. LIMITI DELLA PROCEDURA<br />
La diagnosi dell'infezione da T. gondii può essere stabilita solamente sulla<br />
base di una combinazione di dati clinici e biologici. Il risultato di un singolo test<br />
di titolazione degli anticorpi <strong>IgM</strong> anti-T. gondii non costituisce una prova<br />
sufficiente per una diagnosi di infezione recente da T. gondii.<br />
• Solo una combinazione di dati clinici e biologici (significativo aumento degli<br />
anticorpi IgG anti-T. gondii in 2 sieri prelevati da uno stesso paziente a<br />
distanza di 3 settimane e analizzati in una stessa seduta analitica, presenza<br />
di un livello significativo di <strong>IgM</strong> anti-T. gondii, determinazione di IgG a bassa<br />
avidità) può confermare la diagnosi di un’infezione recente.<br />
• La sola presenza di anticorpi <strong>IgM</strong> anti-T. gondii non costituisce una prova<br />
sufficiente per confermare un’infezione recente poiché gli anticorpi <strong>IgM</strong><br />
possono persistere per numerosi mesi o persino per anni dopo l’infezione.<br />
In presenza di <strong>IgM</strong>, è necessario eseguire una determinazione quantitativa<br />
degli anticorpi IgG anti-T. gondii, nonché un controllo dell’evoluzione degli<br />
anticorpi anti-T. gondii su almeno un secondo campione prelevato tre<br />
settimane più tardi.<br />
• Se un campione viene analizzato troppo precocemente durante una primoinfezione,<br />
gli anticorpi <strong>IgM</strong> anti-T. gondii potrebbero non essere ancora<br />
presenti. In caso di dubbio, è necessario eseguire un secondo prelievo circa<br />
3 settimane più tardi sul quale verrà ripetuta la ricerca delle <strong>IgM</strong>.<br />
11. CONTROLLO DI QUALITÀ DEL PRODUTTORE<br />
Tutti i reagenti prodotti sono preparati conformemente al nostro Sistema di<br />
qualità dal ricevimento delle materie prime fino alla commercializzazione del<br />
prodotto finale. Ogni lotto è sottoposto a un controllo di qualità e può essere<br />
commercializzato solo se conforme ai criteri di accettazione prestabiliti. La<br />
documentazione relativa alla produzione e ai controlli di ogni singolo lotto è<br />
