SEPTIFAST - A MULTIPLEX REAL TIME PCR METHOD FOR THE DETECTION AND<br />
IDENTIFICATION OF BACTERIAL AND FUNGAL PATHOGENS IN THE BLOODSTREAM<br />

Worldwide, se<strong>ve</strong>re sepsis is increasingly a cause of death, and rapid, correct initial antimicrobial<br />
treatment reduces mortality. Although blood culture is considered to be the gold standard, the etiological<br />
agent(s) cannot always be found by this technology. A no<strong>ve</strong>l multiplex PCR test (SeptiFast),<br />
that can be performed in less than six hours and can identify a panel of 20 bacterial and fungal species<br />
directly from blood, was compared with blood culture in a multicenter trial in patients with<br />
suspected bacterial or fungal sepsis. Fi<strong>ve</strong> hundred and fi ft y-eight paired samples of blood culture<br />
and SeptiFast from 359 patients were evaluated. Blood culture found 96 isolates and SeptiFast identifi<br />
ed 186 microorgansims. Of the 96 blood culture isolates, 50 were also detected by SeptiFast and 22<br />
blood culture isolates were identifi ed as contaminant isolates. Eighteen of the remaining 24 blood<br />
culture clinical isolates were not detected by SeptiFast and six blood culture isolates were not included<br />
in the SeptiFast masterlist. SeptiFast detected 186 microorganisms, of which 12 were considered<br />
to be contaminants. Of the 174 clinical microorganisms identifi ed by SeptiFast, 50 were also detected<br />
by blood culture. 67 of the remaining 124 SeptiFast positi<strong>ve</strong> and blood culture negati<strong>ve</strong> microorganisms<br />
were also identifi ed in routine cultures from clinical relevant anatomical sites from the patient.<br />
Future clinical utility studies will re<strong>ve</strong>al if SeptiFast can provide a rapid, signifi cant add-on advantage<br />
to blood culture in the detection of blood stream pathogens.<br />
