Document checklist<br />
Bef<strong>or</strong>e a decision c<strong>an</strong> be made on your application, you must provide the following documents.<br />
Completed questionnaires. Each individual, each partner, each direct<strong>or</strong> <strong>an</strong>d each executive committee member who<br />
is named in the tr<strong>an</strong>sferee section must complete a questionnaire f<strong>or</strong>m as part of this application. Further copies of the<br />
questionnaire f<strong>or</strong>m are available from www.vcglr.vic.gov.au <strong>or</strong> by phoning 1300 182 457.<br />
Approved training f<strong>or</strong> <strong>licence</strong> applic<strong>an</strong>ts. Please refer to the ‘Training f<strong>or</strong> <strong>licence</strong> applic<strong>an</strong>ts’ fact sheet. Following the<br />
completion of new entr<strong>an</strong>t training <strong>an</strong>d Responsible Service of Alcohol (RSA) training, evidence of completion is to be provided<br />
to the Commission.<br />
A pl<strong>an</strong> <strong>or</strong> drawing of the premises . If the <strong>existing</strong> approved pl<strong>an</strong> of the licensed premises held by Vict<strong>or</strong>i<strong>an</strong> Commission f<strong>or</strong><br />
Gambling <strong>an</strong>d Liqu<strong>or</strong> Regulation does not accurately depict the flo<strong>or</strong> layout of the premises, you must submit a pl<strong>an</strong> which<br />
meets the requirements specified by the Commission. Please refer to the 'Pl<strong>an</strong>s of licensed premises' fact sheet.<br />
If you intend to increase the size of the licensed area from what the Commission has auth<strong>or</strong>ised under <strong>an</strong> <strong>existing</strong> approved<br />
pl<strong>an</strong>, then you will also need to complete <strong>an</strong> application f<strong>or</strong> variation to <strong>an</strong> <strong>existing</strong> <strong>licence</strong> <strong>or</strong> <strong>permit</strong> f<strong>or</strong>m.<br />
A letter confirming that settlement has occurred must be provided bef<strong>or</strong>e the <strong>licence</strong> <strong>or</strong> <strong>permit</strong> is issued. The proposed new<br />
licensee <strong>or</strong> <strong>permit</strong>tee c<strong>an</strong>not trade until such time as their name appears as licensee <strong>or</strong> <strong>permit</strong>tee on the <strong>licence</strong> <strong>or</strong> <strong>permit</strong>.<br />
If you wish to ch<strong>an</strong>ge the trading hours/conditions of the <strong>licence</strong>, you will also need to complete <strong>an</strong> application f<strong>or</strong> variation<br />
to <strong>an</strong> <strong>existing</strong> <strong>licence</strong> <strong>or</strong> <strong>permit</strong> f<strong>or</strong>m. You c<strong>an</strong> lodge the variation application at the same time as lodging this tr<strong>an</strong>sfer f<strong>or</strong>m.<br />
Maximum patron capacity (if applicable)<br />
If you are applying to tr<strong>an</strong>sfer a general, on-premises, late night general, late night on-premises, restaur<strong>an</strong>t <strong>an</strong>d cafe <strong>or</strong> full club<br />
<strong>licence</strong> which does not have a maximum patron capacity end<strong>or</strong>sed you must provide the following documents to assist the<br />
Commission in determining a maximum patron capacity:<br />
• a rep<strong>or</strong>t from a registered building survey<strong>or</strong> stating:<br />
- the area in square metres available to the public f<strong>or</strong> the whole of the internal premises (excluding toilets, passageways<br />
<strong>an</strong>d the like) <strong>an</strong>d (if applicable) f<strong>or</strong> <strong>an</strong>y external areas such as courtyards <strong>or</strong> decks, <strong>an</strong>d<br />
- the number of patrons that may be accommodated on the internal premises <strong>an</strong>d (if applicable) on <strong>an</strong>y external areas of<br />
the premises based on a ratio of 0.75 square metres per person. The basis of the calculations must be included in the<br />
rep<strong>or</strong>t.<br />
• <strong>an</strong> occup<strong>an</strong>cy <strong>permit</strong> showing a prescribed patron capacity. If <strong>an</strong> occup<strong>an</strong>cy <strong>permit</strong> c<strong>an</strong>not be supplied, then the rep<strong>or</strong>t from<br />
the building survey<strong>or</strong> must also include patron calculations on the same basis as <strong>an</strong> occup<strong>an</strong>cy <strong>permit</strong> under the Building Act<br />
1993, <strong>an</strong>d<br />
• if available, a copy of the most recently issued pl<strong>an</strong>ning <strong>permit</strong> f<strong>or</strong> the premises which specifies the maximum patron<br />
capacity that is allowed.<br />
Note: Please ensure that the building survey<strong>or</strong> registration number is displayed on the rep<strong>or</strong>t.<br />
F<strong>or</strong> further inf<strong>or</strong>mation please read the attached ‘Maximum patron capacity’ fact sheet.<br />
Note: If you are applying to tr<strong>an</strong>sfer a restricted club <strong>licence</strong> please contact Vict<strong>or</strong>i<strong>an</strong> Commission f<strong>or</strong> Gambling <strong>an</strong>d Liqu<strong>or</strong><br />
Regulation f<strong>or</strong> inf<strong>or</strong>mation regarding the documents you are required to submit in relation to maximum patron capacity.<br />
The Commission may request <strong>an</strong> applic<strong>an</strong>t to provide <strong>an</strong>y other inf<strong>or</strong>mation. If required to do so you will notified in writing.<br />
Application fee<br />
• The application fee must be paid at the time of application. There is no GST payable on this fee.<br />
• Please refer to the 'Liqu<strong>or</strong> <strong>licence</strong> fees' fact sheet f<strong>or</strong> current details. The fee c<strong>an</strong> be paid by cheque, money <strong>or</strong>der <strong>or</strong> credit card.<br />
Cash will only be accepted if paying in person. Cheques <strong>an</strong>d money <strong>or</strong>ders are to be made payable to ‘Vict<strong>or</strong>i<strong>an</strong> Commission f<strong>or</strong><br />
Gambling <strong>an</strong>d Liqu<strong>or</strong> Regulation’.<br />
