Chapter 5 describes Discharge Planning. We know that if medical teams are aware of issues<br />
that a patient may face when they leave hospital then these can be planned for from the<br />
beginning. Discharge planning should begin at the outset of the patient journey; when left to the<br />
end of hospital stay it leads to delays and inefficiencies that are, not only bad for the hospital,<br />
but also a source of understandable frustration and dissatisfaction for patients and their carers.<br />
Knowing what should happen after going home is another important step on the road to a full<br />
recovery and integration with Primary Care is crucial in this process.<br />
Chapter six sets out the indicators of quality for each of the modalities described for elective<br />
surgery<br />
