There should be documentary evidence of a patients‟ consent if they are happy<br />
An appointment is made for attendance at a Pre-admission Assessment Clinic<br />
Details of planned surgery are conveyed to the Primary and Community Care Team<br />
Patients receive education on post-operative needs e.g. dietary, mobility aims<br />
