REPORTS<br />
5.1 The first report on the project evaluation would be compilation<br />
<strong>of</strong> in<strong>for</strong>mation in the <strong>for</strong>mat given at Appendix I.<br />
5.2 The study <strong>of</strong> the data and the classification <strong>of</strong> the dam in<br />
respect <strong>of</strong> its hazard potential <strong>for</strong>m the second report. In case<br />
the hazard potential <strong>of</strong> a dam is classified as high, the same<br />
should be reported immediately to the project authority, State<br />
Government, <strong>Central</strong> <strong>Water</strong> <strong>Commission</strong> (Dam <strong>Safety</strong><br />
Organization).<br />
5.3 Phase – I Report<br />
5.3.1 The Phase I report is an investigation report after a study <strong>of</strong><br />
the design data and behaviour <strong>of</strong> the dam and a detailed<br />
inspection <strong>of</strong> the site. Appendix II gives the guidelines <strong>for</strong><br />
per<strong>for</strong>ming field inspection and can serve as a basis <strong>for</strong><br />
developing a detailed checklist to suit the characteristics <strong>of</strong> the<br />
particular project or structure. The fundamental objective <strong>of</strong><br />
the inspection is the detection <strong>of</strong> any existing or developing<br />
structural or hydraulic weakness. Appendix III indicates the<br />
structures, features, events and evidence which should be<br />
looked <strong>for</strong>.<br />
5.3.2 The Phase I report should contain a summary <strong>of</strong> existing<br />
engineering data, including geological maps and in<strong>for</strong>mation,<br />
results <strong>of</strong> inspections and photographs.<br />
5.3.3 Evaluation <strong>of</strong> operational adequacy <strong>of</strong> reservoir regulation and<br />
maintenance <strong>of</strong> dam and operating facilities and features that<br />
pertain to the safety <strong>of</strong> the dam.<br />
5.3.4 Description <strong>of</strong> warning system in effect, if any. Evaluation <strong>of</strong><br />
the hydraulic and hydrological assumptions and structural<br />
stability.<br />
5.3.5 An assessment <strong>of</strong> the general condition <strong>of</strong> the dam with<br />
respect to safety based upon the findings <strong>of</strong> the visual<br />
inspection and review <strong>of</strong> engineering data. Where data on the<br />
original design indicate significant departures from or noncon<strong>for</strong>mance<br />
with guidelines contained herein, the engineerin-charge<br />
<strong>of</strong> the investigation will give his opinion <strong>of</strong> the<br />
significance, with regard to safety, <strong>of</strong> such factors. Any<br />
