Other amounts available upon request. Premiums are based on applicant’s age at<br />
nearest birthday. Policies are non-cancelable as long as premiums are paid. Premiums<br />
may be paid by bank draft, annually, semiannually, or monthly. (A no-cost medical<br />
exam may be required depending on age, health, or amount of coverage desired.)<br />
Above premiums are preferred rates. Level death benefit to age 95. Premiums increase<br />
annually. All policies are issued and subject to underwriting by ABC Insurance Company.<br />
