17.3. Transporting Personnel. Workers transporting the waste should be<br />

equipped with appropriate personal protective equipment including heavyduty<br />

gloves, coveralls, thick-soled boots and leg protectors.<br />

SECTION 18. Off-site Transportation of Health Care Waste. The health care<br />

waste generator is responsible for the safe packaging and adequate labeling of waste to<br />

be transported off-site for treatment and disposal. Packaging and labeling should comply<br />

with the national regulation governing the transport of hazardous wastes (RA 6969) and<br />

maintaining that it presents no danger to the public during transport. Likewise, the waste<br />

generators must see to it and ensure that their wastes are property treated and disposed<br />

of in the treatment facility. The city will be the one to collect such waste facility.<br />

Section 19. Consignment <strong>No</strong>te. AH Medical hazardous toxic waste to be<br />

transported to an approved off-site waste treatment facility shall be transported only by a<br />

DENR-accredited transporter or carrier. The authorized transporter / <strong>City</strong> Government<br />

carrier shall maintain a completed consignment note of all health care waste taken from<br />

the health care establishment for treatment or disposal.<br />

By the time that waste transporter receives the waste, the transporter shall provide the<br />

waste generator with a copy of the consignment note for the generator's waste records.<br />

SECTION 26. Content of the Consignment Mote. The transporter and the<br />

generator shall separately maintain a copy of the consignment note. The consignment<br />

note shaH include, but not limited to the following information:<br />

a. The name address, telephone number, and accreditation number of the transporter<br />

b. The type and quantity of waste transported<br />

c. The name, address, and telephone number of the generator<br />

d. The name, address, telephone number, permit number, and the signature of an<br />

authorized representative of the approved facility receiving the waste<br />

The date that the waste is collected or removed from the generator's facility, the date that<br />

the waste is received by the transfer station, or point of consolidation, if applicable, and<br />

the date that the waste is received by the treatment facility.
