Background Information – Respondents<br />
Kyle E. Hoogendoorn, D.P.M.<br />
1. Kyle Elliott Hoogendoorn, D.P.M., obtained his podiatric medical degree in 1997 from the<br />
<strong>Ohio</strong> College <strong>of</strong> Podiatric Medicine in Cleveland, <strong>Ohio</strong>. From 1997 through 1998,<br />
Dr. Hoogendoorn participated in a primary podiatric medical residency at Richmond<br />
Heights Hospital 3 in Richmond Heights, <strong>Ohio</strong>. Subsequently, from August 2000 to<br />
February 2003, Dr. Hoogendoorn participated in a pain management fellowship through<br />
Pain Control Consultants, Inc., in Columbus, <strong>Ohio</strong> (Respondent’s Exhibits<br />
[Resp. Ex.] 103H; Hearing Transcript [Tr.] at 81-84, 411, 2181-2182)<br />
Since 1997, Dr. Hoogendoorn has been licensed by the <strong>Board</strong> to practice podiatric<br />
medicine and surgery in <strong>Ohio</strong>. (Resp. Ex. 103H)<br />
2. Dr. Hoogendoorn has been certified by the American <strong>Board</strong> <strong>of</strong> Orthopedic and Primary<br />
Podiatric Medicine and the American Academy <strong>of</strong> Wound Management. (Resp. Ex. 103H)<br />
In addition, Dr. Hoogendoorn testified that he has been certified by the American Academy<br />
<strong>of</strong> Pain Management, and that he sits on the academy’s committee for continuing<br />
education. Dr. Hoogendoorn noted that M.D.s, D.O.s, D.P.M.s, and dentists who practice<br />
pain management are eligible for membership in that organization, and that they all take the<br />
same certifying examination. (Tr. at 2215-2216)<br />
3. Dr. Hoogendoorn currently practices at Pro-Active Wound Care Clinics, Inc., in Hilliard,<br />
<strong>Ohio</strong>, and the Foot and Ankle Health Center, Inc., in Grove City, <strong>Ohio</strong>. (Resp. Ex. 103H)<br />
W. David Leak, M.D.<br />
4. W. David Leak, M.D., obtained his medical degree in 1979 from the Wake Forest<br />
University, Bowman-Gray School <strong>of</strong> Medicine, in Winston-Salem, North Carolina. From<br />
1979 through 1980, Dr. Leak participated in a rotating internship in the Department <strong>of</strong><br />
Anesthesia at the <strong>Ohio</strong> <strong>State</strong> University Hospitals in Columbus, <strong>Ohio</strong>. From 1981 through<br />
1983, Dr. Leak participated in a residency in anesthesiology at the Hospital <strong>of</strong> the<br />
