65. Dr. Hoogendoorn testified that he had injected only s<strong>of</strong>t tissue during his fellowship.<br />
Dr. Hoogendoorn further testified that he never performed spinal injections beyond the<br />
muscles that surround the spinal column. (Tr. at 2516-2517)<br />
Dr. Hoogendoorn also testified that he never performed epidurals or placed spinal<br />
stimulators, although he had assisted in such procedures. When asked why he had been<br />
taught to perform some interventional pain management procedures but not others,<br />
Dr. Hoogendoorn replied:<br />
Trigger point and chemoneurolytic injections are easily transferred from the<br />
back into the foot and ankle area in the s<strong>of</strong>t tissues. The same principles<br />
apply. * * *<br />
It was never intended that for any reason I was going to be doing epidurals,<br />
sympathetic blocks; implant stimulators * * *. * * * It was more for me to<br />
learn technique, instrumentation, to develop that and bring it down to the foot<br />
and ankle where appropriate.<br />

Level <strong>of</strong> Supervision <strong>of</strong> Dr. Hoogendoorn during Procedures<br />
Testimony <strong>of</strong> Dr. Leak<br />
66. Dr. Leak testified that it is possible that Dr. Hoogendoorn had been allowed to perform<br />
chemoneurolytic injections using Sarapin as the chemoneurolytic agent without an<br />
attending present in the same room. Dr. Leak further testified that Sarapin “is a slow,<br />
slow-moving agent that goes with local anesthetic. And it’s just like—it’s literally an<br />
intramuscular injection that will hopefully neutralize the nerve fibers that penetrate the<br />
muscle.” (Tr. at 446-448)<br />
Later in the hearing, Dr. Leak testified that he had been present with Dr. Hoogendoorn<br />
whenever Dr. Hoogendoorn was performing trigger point or chemoneurolytic injections.<br />
Dr. Leak further testified that Dr. Griffin had spent more time with Dr. Hoogendoorn, and<br />
that Dr. Leak had left to Dr. Griffin’s judgment how Dr. Griffin “would staff”<br />
Dr. Hoogendoorn. (Tr. at 2768)<br />
