mented two pilots running in parallel. The outcomes of the pilot program evaluations have<br />

then been used to guide future planning. In some instances (Switzerland, Germany, Spain)<br />

the prisons selected for the initial pilot programs were relatively small institutions and/or<br />

open or half-open institutions with lower security levels. In these cases, programs were tested<br />

and evaluated in these prison environments before expanding the programs into larger,<br />

closed prisons with higher security levels. However, in Moldova the pilot needle exchange<br />

was done in a medium/maximum-security prison with a population of approximately 1000<br />

prisoners.<br />

The experience of the six countries studied for this report demonstrates that pilot projects<br />

can be undertaken quickly and do not have to delay broader implementation of needle<br />

exchange programs. For example, in Kyrgyzstan a pilot needle exchange was opened in<br />

October 2002, in early 2003 approval was given to expand the program, as of September<br />

2003 programs were operating in six of 11 prisons, and by April 2004 programs were operating<br />

in all 11 prisons. Nor do evaluations have to be fully completed before programs are<br />

expanded to other prisons. For example, in Belarus a program was piloted in one prison<br />

beginning in April 2003, scheduled to run until January of 2004. Although the term of the<br />

pilot was extended to June of 2004, it was also extended to two other prisons, and the<br />

Ministry of Internal Affairs signalled its willingness to expand needle exchange to prisons<br />

throughout the country. It is important to note that in the prison systems presented in this<br />

report, pilot projects have not been relied on as a tactic to delay the broader implementation<br />

of needle exchange programs.<br />

Not only are evaluations important in the expansion of needle exchange programs within<br />

a jurisdiction, but they are also of great use to the broader international community. Rigorous<br />

evaluations of pilot needle exchange programs (and expanded programs) contribute important<br />

information to the international literature regarding prison needle exchange programs.<br />

The findings of evaluations provide the evidence for other jurisdictions. With such evidence,<br />

more jurisdictions can demonstrate leadership and generate consensus surrounding the need<br />

for, and implementation of, prison needle exchange programs.<br />
