The NT Clinical School<br />

… about us<br />

The Northern Territory Clinical School (NTCS) is a department <strong>of</strong> <strong>Flinders</strong><br />

<strong>University</strong> School <strong>of</strong> Medicine. The NTCS was created in 1996 through<br />

collaboration between the Northern Territory Government and <strong>Flinders</strong><br />

<strong>University</strong>. It has campuses at the Royal Darwin Hospital, Alice Springs<br />

Hospital, Katherine and Nhulunbuy. The NTCS teaches <strong>Flinders</strong> Year 3<br />

and Year 4 graduate entry medical (GEMP) students and James Cook<br />

<strong>University</strong> (JCU) Year 5 and Year 6 undergraduate medical students. It<br />

also coordinates and supports clinical placements for <strong>Flinders</strong> <strong>University</strong><br />

final year students in audiology, speech pathology and nutrition /<br />

dietetics.<br />

The Northern Territory Rural Clinical School (NTRCS) is a program within<br />

the NTCS that is funded under a separate grant from the Commonwealth<br />

Department <strong>of</strong> Health and Ageing. The NTRCS has sites in Katherine,<br />

Alice Springs and Nhulunbuy. Some Year 3 NTCS students spend six<br />

months based in the Katherine or Alice Springs NTRCS sites, and Year 4<br />

students have the choice <strong>of</strong> completing rotations in all sites.<br />

The NT Clinical School is committed to enhancing the health workforce<br />

and practices in the NT through the provision <strong>of</strong> an academic<br />

environment enabling high quality, appropriate, culturally safe, clinical<br />

education to medical and allied health students, reflecting the social,<br />

political, economic and geographic contexts in which health care is<br />

delivered in the NT.<br />

Our goals are to:<br />

Provide high quality medical teaching and training to medical<br />

students to prepare them for future medical practice in rural and<br />

remote areas <strong>of</strong> Australia, particularly the NT;<br />

Provide high quality pre- and post-graduate support to allied health<br />

students to prepare them for future practice in rural and remote<br />

areas <strong>of</strong> Australia, particularly the NT;<br />

Contribute to the evolution <strong>of</strong> clinical and allied health education<br />

through innovative curriculum development and identification <strong>of</strong><br />

contextual learning opportunities in the NT;<br />

Undertake in its own right, and in collaboration with other<br />

organisations, research that contributes to improved health<br />

Message from the Clinical Dean<br />

2009 was a very eventful year for the Northern<br />

Territory Clinical School. The NTCS secured funding<br />

to allow the full four-year <strong>Flinders</strong> <strong>University</strong><br />

Graduate Entry Medical Program to be <strong>of</strong>fered in the<br />

Northern Territory as the Northern Territory Medical<br />

Program (NTMP). This will commence in 2011.<br />

In preparation for the commencement <strong>of</strong> the NTMP,<br />

staff were immediately employed in the planning<br />

phases <strong>of</strong> this ground-breaking development. This<br />

included site and architectural planning for building<br />

developments at Charles Darwin <strong>University</strong> and<br />

Royal Darwin Hospital; contextualization <strong>of</strong> the<br />

GEMP curriculum for the Northern Territory;<br />

formation <strong>of</strong> working parties to manage the NTMP<br />

development, marketing activities and employment<br />

<strong>of</strong> new staff.<br />

At the same time as this planning was taking place, the School continued<br />

with its regular responsibilities <strong>of</strong> placing and teaching medical and allied<br />

health students; <strong>of</strong>fering research support to academic staff; undertaking<br />

research activities; regularly evaluating our programs and services;<br />

marketing; and liaising with local community advisory groups and with our<br />

numerous stakeholders.<br />

Many thanks to the NT Clinical School and NT Rural Clinical School staff for<br />

their support and efforts during such a demanding year.<br />

Key Achievements<br />

Key achievements <strong>of</strong> 2009 were:<br />

A successful funding submission led to the Australian Government<br />

committing $27.8 million towards the Territory’s first full medical<br />

education program: the Northern Territory Medical Program (NTMP);<br />

The development <strong>of</strong> an NTCS brokered letter <strong>of</strong> intent regarding the NT<br />

Medical Program between Vice Chancellors <strong>of</strong> <strong>Flinders</strong> <strong>University</strong> and<br />

Charles Darwin <strong>University</strong> (CDU), which was signed in the presence <strong>of</strong><br />

Federal Health Minister Nicola Roxon;<br />

Initial planning for NTMP included: preliminary planning for the delivery <strong>of</strong><br />

the <strong>Flinders</strong> Curriculum for the NT; establishing and engaging working<br />

parties; marketing and making foundation appointments;<br />

Collaboration between <strong>Flinders</strong> <strong>University</strong> and CDU lead to development <strong>of</strong><br />

the NTMP Building Plan for the CDU campus (building under construction in<br />

2010);<br />

Established the Graduate Diploma in Indigenous and Remote Child Health<br />

(first students enrolled in 2010);<br />

Completed NT Rural Clinical School building, a joint project with the Baker<br />

Heart Foundation (the Rubuntja Building opens in 2010);<br />

Upgraded videoconference links to Katherine and Nhulunbuy;<br />

Completed feasibility study for a GP Teaching Practice in Nhulunbuy (this<br />

led to a successful grant application in 2010 for the Medical and Allied<br />

Health Education Centre);<br />

NTRCS Local Advisory Groups assisted with student functions, <strong>of</strong>fered<br />

information on local workforce issues and participated in student-based<br />

research;<br />

Hosted NTRCS Community Advisory Board (CAB) meeting in Nhulunbuy,<br />

attended by the Vice Chancellors <strong>of</strong> both <strong>Flinders</strong> <strong>University</strong> and CDU;<br />

Hosted <strong>Flinders</strong> <strong>University</strong> Regional Community Week for Year 2 students<br />

at all Rural Clinical School sites;<br />

Completed Allied Health Work-Integrated Learning (AHWIL) project data<br />

collection and presented preliminary results at the ANZAME and HERDSA<br />

education conferences;<br />

Graduation <strong>of</strong> three Allied Health students who completed their study<br />

programs through <strong>Flinders</strong> <strong>University</strong> under the NT Quota. The group<br />

included two dietitians and one speech pathologist and in 2010 all three<br />

gained employment in the Northern Territory;<br />

Provision <strong>of</strong> full-day pr<strong>of</strong>essional development workshops on ‘best practice<br />

clinical education’ in Central Australia and the Top End for NT allied health<br />

staff;<br />

Membership <strong>of</strong> the SARRAH steering group that produced the ‘Transition to<br />

remote and rural practice’ website under a successful RHSET grant<br />

Began holding inter-pr<strong>of</strong>essional tutorials for medical students and<br />

community and hospital pharmacy interns;<br />

Offered Mini CEX training to staff and ran several ‘Teaching on the Run’<br />

modules;<br />

Supported research by academic staff through the NTCS Research Officer<br />

and Menzies School <strong>of</strong> Health Research;<br />

Collated publications by <strong>Flinders</strong> <strong>University</strong> (NTCS) staff for 2009;<br />

Increased library resources and access across sites;<br />

Commenced development <strong>of</strong> Indigenous Health curriculum and alignment<br />

to CDAMS objectives;<br />

Employed Aboriginal Cultural Advisor in Alice Springs;<br />

Reached a new agreement with Rio Tinto/Alcan for provision <strong>of</strong> short-term<br />

student accommodation in Nhulunbuy;<br />

Teaching agreement signed with Bath St Medical Practice in Alice Springs;<br />

Established NTCS Database AXON for student, staff, supervisor and site<br />

information;<br />

Updated NTCS website and website incorporated with the <strong>Flinders</strong><br />

<strong>University</strong> site;<br />

Provision <strong>of</strong> Smartboards at majority <strong>of</strong> teaching sites;<br />

Incorporated the use <strong>of</strong> the <strong>Flinders</strong> <strong>University</strong> Wiki for a multitude <strong>of</strong><br />

collaborative projects including the setup <strong>of</strong> the NTMP;<br />

Sponsored & initiated activities through NTCS Green Fund:<br />

• 5 Ride to Work events,<br />

• One Mobilemuster mobile phone recycle station,<br />

• Battery recycle boxes in all campuses,<br />

• Extra recycling facilities in Darwin <strong>of</strong>fice,<br />

• Purchase <strong>of</strong> sustainable and recycled <strong>of</strong>fice and catering products and<br />

• Global Warming seminar at RDH;<br />

Finalist in Power and Water Authority Melaleuca Awards - which recognises<br />

environmental excellence;<br />

Future directions<br />

Key drivers <strong>of</strong> change within the NTCS for 2010 are:<br />

Medical Education and Training<br />

Planning for the NT Medical Program. Years 1 and 2 <strong>of</strong> the <strong>Flinders</strong><br />

Medical Program will be <strong>of</strong>fered for the first time in Darwin in 2011;<br />

Indigenous Pathways funding and new Indigenous support staff;<br />

New Indigenous Entry Stream (IES) to the <strong>Flinders</strong> GEMP program;<br />

CDU and <strong>Flinders</strong> <strong>University</strong> collaboration on pathways to NT Medical<br />

Program;<br />

<strong>Flinders</strong> <strong>University</strong> to manage clinical services at the Palmerston Super<br />

Clinic (due to open in September 2010);<br />

Increased opportunities for interdisciplinary training through<br />

collaborations with CDU, CRH, Baker Institute, NTGPE, Menzies School <strong>of</strong><br />

Health Research, Endeavour Medical Centre and the Palmerston Super<br />

Clinic.<br />

Allied Health Education and Training<br />

The need to reduce fragmentation <strong>of</strong> allied health pr<strong>of</strong>essional placements<br />

by developing centralised systems and strengthening regional networks;<br />

Establishment <strong>of</strong> new inter-pr<strong>of</strong>essional placements.<br />

Innovation and Research<br />

Changing learning environments to better meet student learning needs;<br />

Incorporating complex but simple to use audio visual capabilities into<br />

new buildings;<br />

Developing new research support framework;<br />

Further progress towards eLearning as a viable alternative or<br />

supplement for some current methods.<br />

Northern Territory Health Workforce<br />

Major focus <strong>of</strong> the NT Medical Program will be the recruitment and<br />

training <strong>of</strong> Northern Territory and Indigenous people to become doctors<br />

in the Northern Territory;<br />

NT Medical Program places to be sponsored by NT Government and<br />

graduates will undertake their internships and residency in the NT as<br />

part <strong>of</strong> a bonding arrangement with the NT Government;<br />

Collaborating with remote health clinics to enhance their training<br />

potential (this includes the planned Medical and Allied Health Education<br />

Centre in Nhulunbuy);<br />

Continuing development <strong>of</strong> data sources to provide strong knowledge<br />

bases about optimal health education leading to career uptake in the NT;<br />

Developing suitable attraction and retention packages for health staff<br />

engaged in academic activity with NTCS in the NT.<br />

Organisational Excellence<br />

Offering opportunities for staff to build on their cultural awareness<br />

practices;<br />

Increasing the number <strong>of</strong> clinicians with either full or adjunct academic<br />

status with <strong>Flinders</strong> <strong>University</strong>;<br />

Developing infrastructure across all four sites <strong>of</strong> the NTCS, with<br />

particular emphasis on information and communication technology;<br />

Further developing evaluation processes which provide seamless results<br />

and feedback and monitor ensuing changes;<br />

Building relationships with other workforce and educational and research<br />

stakeholders, particularly Centre for Remote Health; Charles Darwin<br />

<strong>University</strong> (CDU);Northern Territory General Practice Education Limited<br />

(NTGPE); Menzies School <strong>of</strong> Health Research; General Practice Network<br />

NT (GPPNNT); Department <strong>of</strong> Health and Families and Batchelor<br />

Institute <strong>of</strong> Indigenous Tertiary Education;<br />

2009 at a glance<br />

Medical Students<br />

The NTCS provided educational activities to long-term (> six months) and<br />

short-term students from <strong>Flinders</strong> <strong>University</strong>, James Cook <strong>University</strong>, other<br />

Australian universities, and some overseas universities, across four<br />

campuses in the NT.<br />

In providing these services, the NTCS implemented the following programs:<br />

Allied Health Students<br />

The Allied Health Academic team supported 188 weeks <strong>of</strong> training in 30<br />

placements across three disciplines (speech pathology, audiology and<br />

nutrition / dietetics). Third year NT quota dietetics student, Bethany Prosser,<br />

was academic top <strong>of</strong> her class.<br />

Research<br />

The Allied Health Work-Integrated Learning (AHWIL) project <strong>of</strong> the Northern<br />

Territory Clinical School and the Department <strong>of</strong> Health and Families was<br />

completed. The study focused on allied health student placements and their<br />

impacts on the workforce.<br />

NTCS support to academic staff included:<br />

Subsidized places in Menzies School <strong>of</strong> Health Research courses;<br />

Dissemination <strong>of</strong> information about research funding and<br />

opportunities;<br />

Tailored support for individual projects;<br />

Assistance with grant applications.<br />

Student Projects<br />

In 2009, the NTCS <strong>of</strong>fered the following support to our students:<br />

Streamlined process for registration <strong>of</strong> student Clinical Audits (for full<br />

list <strong>of</strong> audits undertaken, refer to Appendix B<br />

Further elucidated the process <strong>of</strong> Clinical Audit (the Clinical Audit<br />

Cycle) and disseminated to staff and students (see diagram below).<br />

Key Result Areas for 2009<br />

Performance <strong>of</strong> the NTCS is measured against the Key Performance<br />

Indicators (KPI) identified in the Strategic Plan 2007-2009.<br />

…Medical Education and Training<br />

KPI 1.1 Students at NTCS perform at least as well in formal<br />

examination, including interim assessments, as their peers outside<br />

NTCS.<br />

Analyses <strong>of</strong> 2009 exam results <strong>of</strong> <strong>Flinders</strong> <strong>University</strong> Yr 3 and JCU Yr 5<br />

students showed NTCS students performed at least as well as their<br />

peers.<br />

Final year student Prizes:<br />

Of the final year JCU Medical students, Kirsty Campbell shared the<br />

Critical and Crisis Care Year 6 Prize and Claire Shanahan was awarded<br />

the Mick McLoughlin Bursary.<br />

Of the <strong>Flinders</strong> medical students Janice Ku, Jeremy Wells, Vanessa<br />

Acero and Sheree Cross all received Dean’s Congratulatory letters and<br />

Sheree Cross graduated with Honours.<br />

KPI 1.2 Students report satisfaction overall with the teaching <strong>of</strong><br />

the curriculum <strong>of</strong> the NTCS, its organisation and scheduling.<br />

Royal Darwin Hospital Based Medical Education (<strong>Flinders</strong> Yr 3, JCU Yr 5<br />

students; 66% response rate):<br />

Students reported satisfaction overall.<br />

Comments regarding Clinical Supervisors were generally positive.<br />

Categories that attracted the most agreement were: "have been<br />

approachable and available", "have assisted me in my learning needs<br />

and "have treated me with respect".<br />

Some students suggested informing students <strong>of</strong> names <strong>of</strong> registrars<br />

and interns, and having a designated rotation supervisor who knows<br />

the student learning goals and monitors student progress and can be<br />

contacted at any time.<br />

When students were asked about skills and competencies in 15<br />

categories, more than 80% <strong>of</strong> responses indicated improvement.<br />

Students were asked to complete paper questionnaires following 13<br />

teaching sessions with 13 different tutors. The response rate was 64%.<br />

Ninety-three percent <strong>of</strong> ranks allocated to "Overall, how would you rate<br />

this teaching session?" were Excellent, Very Good or Good.<br />

Ten JCU students were placed in general practice and with DMOs<br />

servicing remote communities. Students valued the independent<br />

consulting experience that they gained within these community<br />

placements.<br />

Darwin Community Based Medical Education (<strong>Flinders</strong> Yr 3 students; response<br />

rate 50%):<br />

Eight <strong>Flinders</strong> students were placed in urban general practices and<br />

other community-based health providers.<br />

Satisfaction with the student role was generally good in all sites.<br />

Consistent satisfaction was reported with the quality and breadth <strong>of</strong><br />

learning in the GP setting.<br />

Students reported high quality clinical teaching.<br />

In response to student feedback, timetabling was streamlined and new<br />

strategies were implemented to reduce timetabling complexity.<br />

When students were asked about skills and competencies in 15<br />

categories, 67-78% <strong>of</strong> responses indicated improvement.<br />

NTRCS Katherine (<strong>Flinders</strong> Yr 3; 83% response rate)<br />

Students reported high quality teaching.<br />

When students were asked about skills and competencies, 64-71% <strong>of</strong><br />

responses indicated improvement.<br />

NTRCS Alice Springs (<strong>Flinders</strong> Yr 3; 100% response rate)<br />

Satisfaction with student role was generally good in all sites.<br />

Students reported high quality teaching and teachers.<br />

When students were asked about skills and competencies, 93-96% <strong>of</strong><br />

responses indicated improvement.<br />

Short Rotations at Alice and Darwin (<strong>Flinders</strong> Yr 4, JCU Yr 6, interstate and<br />

international)<br />

Students reported high quality clinical teaching.<br />

KPI 1.3 The cultural sensitivity and safety <strong>of</strong> the NTCS is assessed<br />

annually to be appropriate for patients, students, academics,<br />

staff and communities.<br />

In 2009 the cultural sensitivity and safety <strong>of</strong> the NTCS was measured by<br />

surveying students:<br />

The skill which Alice Springs short rotation students felt they had<br />

improved in most was cultural sensitivity (94% <strong>of</strong> respondents);<br />

78% <strong>of</strong> RDH short rotation students who responded reported<br />

improvement in cultural sensitivity;<br />

All Year 4 students placed in Nhulunbuy reported improvement in<br />

cultural sensitivity;<br />

70% <strong>of</strong> HBME students who responded reported improvement in<br />

cultural sensitivity;<br />

Year 3 CBME <strong>Flinders</strong> students undertook an intensive two week<br />

placement in the Laynhapuy Homelands. In this setting, students have<br />

the opportunity to experience first-hand the delivery and complexity <strong>of</strong><br />

health care in a resource poor setting, working with the local medical<br />

service and aboriginal health workers. Most students reported<br />

‘Excellent’ or ‘Good’ learning opportunities in relation to most <strong>of</strong> the 15<br />

CDAMS Indigenous Health Curriculum framework standards. All<br />

students rated the organisation <strong>of</strong> the placement as ‘satisfactory’ or<br />

‘excellent’;<br />

Year 3/5 students enjoyed increased access and frequency <strong>of</strong> visits to<br />

Indigenous and remote clinics. Visits included: District Medical Officer<br />

visits to Tiwi Islands and Daly River (Darwin students), visits with a<br />

dedicated staff member to Sunrise West communities (Katherine<br />

students) and remote clinic visits to Santa Teresa, Hermannsberg and<br />

Amooguna (Alice Springs students).<br />

KPI 1.4 Longer term career choices <strong>of</strong> both students and academics<br />

reflect enhanced likelihood <strong>of</strong> return to the NT to practice.<br />

Of the Year 3/5 students, 36% responded to this survey. Of these<br />

students, 36% reported a career goal <strong>of</strong> rural practice and 64% urban;<br />

36% planned to practice in NT in the long term and 64% elsewhere. 73%<br />

found their interest in practicing in the NT increased after this years’<br />

experience (Chart 1);<br />

53% <strong>of</strong> students on short rotations responded to a post placement survey;<br />

63% <strong>of</strong> these reported increased interest in a career in the NT (Chart 2).<br />

KPI 1.6 Students report overall satisfaction in annual feedback<br />

regarding efficacy, reliability and suitability <strong>of</strong> technology-based<br />

media for learning and studying.<br />

Of 13 teaching sessions, video-conferenced sessions were associated with<br />

a lower probability <strong>of</strong> attracting Exceptional or Very Good ratings than<br />

face to face sessions. In response to this result, NTCS is improving<br />

and monitoring video-conferenced sessions.<br />

97% <strong>of</strong> responses indicated the student would recommend a practicum in<br />

the Northern Territory to other students.<br />

demonstrated students’ increased interest in pursuing a career in the<br />

KPI 3.1 The School undertakes at least two NT-specific health<br />

research projects during 2007-09:<br />

NTRCS employed a Senior Research Fellow (50%) and a Research<br />

Assistant (100%) who researched topics that directly address the health<br />

and quality <strong>of</strong> life <strong>of</strong> remote NT populations;<br />

Members <strong>of</strong> the NTCS academic faculty produced 74 publications in peerreviewed<br />

journals in 2009 (see Appendix A<br />

KPI 3.2 The School undertakes at least two NT-specific educational<br />

research projects during 2007-09.<br />

The Allied Health Work-Integrated Learning project was completed in<br />

2009. Preliminary results were presented at the ANZAME and HERDSA<br />

KPI 4.1 NTCS is engaged on a regular basis, at least twice per year,<br />

with workforce-related activities auspiced by the NT Government<br />

and its agencies / partners.<br />

NTCS met and communicated on a regular basis with members <strong>of</strong> the<br />

Department <strong>of</strong> Health and Families in regards to the NT Medical<br />

Program, student accommodation and the Palmerston Super Clinic.<br />

KPI 4.2 NTCS shares joint clinical / academic / research<br />

appointments with at least four other affiliated organisations at any<br />

one time.<br />

NTCS shares joint clinical academic staff positions with Royal Darwin<br />

Hospital and Alice Springs Hospital.<br />

NTCS shares joint clinical academic staff positions with Menzies School <strong>of</strong><br />

Health Research in Darwin.<br />

NTCS shares joint Cultural Liaison Officer positions with NT Department <strong>of</strong><br />

Health and Families (one located in Darwin, one in Alice Springs).<br />

KPI 4.3 NTCS engages at least annually with at least two general<br />

practice organisations in the NT, addressing workforce issues.<br />

As we were planning for organisational change, these KPIs were not<br />

evaluated in 2009.<br />

KPI 5.9 The School demonstrates an enhanced public pr<strong>of</strong>ile through<br />

increased media contact, expanded spheres <strong>of</strong> influence, and<br />

greater participation in activities in affiliated organisations.<br />

During 2009, the NTCS began marketing the NT Medical Program. Several<br />

public events were held including the signing <strong>of</strong> a Letter <strong>of</strong> Intent for<br />

the NT Medical Program between CDU and <strong>Flinders</strong> <strong>University</strong> and<br />

information sessions;<br />

NTCS collaborated with <strong>Flinders</strong> and CDU marketing teams;<br />

NTCS arranged marketing for the Graduate Diploma in Indigenous and<br />

Remote Child Health;<br />

NTCS held booths at four NT Careers Expos, the 2009 NTGPE Teaching and<br />

Learning Conference in Alice Springs, the 2009 GPNNT Annual CPD<br />

Conference in Darwin and the Garma Festival in Arnhemland;<br />

Produced quarterly Newsletter and disseminated to students, staff and<br />
