CHAPTER XI – PATIENT INFORMATION SYSTEMS<br />

109. This chapter proposes to upgrade patient in<strong>for</strong>mation systems in the national health<br />

system. Effective patient in<strong>for</strong>mation systems are necessary to ensure that a<br />

st<strong>and</strong>ardized, effective <strong>and</strong> efficient system <strong>for</strong> data collection, collation,<br />

monitoring, <strong>and</strong> feedback is in place to facilitate programme implementation,<br />

ensure good quality <strong>care</strong>, <strong>and</strong> achieve good patient/programme outcomes.<br />

110. The specific functions of the patient in<strong>for</strong>mation system are:<br />

110.1 To register patients utilizing a st<strong>and</strong>ard Patient Record.<br />

110.2 To collect relevant clinical <strong>care</strong> in<strong>for</strong>mation at baseline <strong>and</strong> subsequent<br />

follow-up visits to monitor progress of patients.<br />

110.3 To monitor adherence to treatment.<br />

110.4 To monitor adverse drug events.<br />

110.5 To collect other clinical, laboratory, <strong>and</strong> non-clinical data that will be<br />

useful <strong>for</strong> programme monitoring at local, provincial <strong>and</strong> national levels.<br />

111. The patient in<strong>for</strong>mation system will be developed as an integral part of the existing<br />

health in<strong>for</strong>mation system. In<strong>for</strong>mation technology upgrades will occur to enable a<br />

st<strong>and</strong>ard electronic <strong>and</strong> paper-based patient in<strong>for</strong>mation system to meet patient<br />

<strong>care</strong> objectives.<br />

CHAPTER XII – MONITORING AND EVALUATION<br />

112. This chapter proposes that a <strong>comprehensive</strong> monitoring <strong>and</strong> evaluation ef<strong>for</strong>t be<br />

integrated into programme implementation. Ongoing monitoring will be critical to<br />
