Your enrollment window<br />
Existing Employees<br />
If you do not actively enroll in or waive certain coverage during your enrollment<br />
window, you will automatically receive the coverage described below for as long<br />
as your <strong>2013</strong> benefits remain in effect. This means that the applicable benefit<br />
deductions will be taken from your paycheck and you will not be able to enroll<br />
in any other coverage unless you have a qualifying life event (as described in Life<br />
Events section of your plan’s Summary Plan Description). See the How to enroll<br />
section on page 12 for enrollment instructions<br />
New Hire Enrollment<br />
If you are newly hired (or rehired more than 30 days after your prior <strong>Sprint</strong><br />
employment ended), you may actively enroll in or waive coverage during your New<br />
Hire Enrollment window ending on the 30th calendar day after your hire/rehire date.<br />
Your benefits are effective on the 30th consecutive calendar day after your date<br />
of hire (except for Supplemental LTD, which is effective as of the first calendar<br />
day in which you are actively at work immediately after you complete 12 months<br />
of employment).<br />
If you do not enroll or waive certain coverage during this period, you will receive<br />
the following coverage effective on the 30th calendar day after your hire/rehire date:<br />
• Medical/Prescription Drug:<br />
- Plan: <strong>Sprint</strong> Health Account Plan<br />
- Coverage level: Employee only (no dependents)<br />
- No Healthy Living Discount<br />
How to get the most out of benefits enrollment<br />
• Review your current and expected health care expenses. Do you have the<br />
coverage you need? Would another plan provide cost savings?<br />
• Review this <strong>2013</strong> <strong>Sprint</strong> <strong>Benefits</strong> <strong>Guide</strong> and benefits information (including<br />
Summary Plan Descriptions) at i-Connect > My Life & Career > <strong>Benefits</strong> or<br />
at sprint.com/benefits.<br />
• Review provider networks online or by phone – see the Helpful contact<br />
information chart on page 104 of this guide<br />
• Estimate any health care and dependent care expenses that could be funded<br />
through a Flexible Spending Account (FSA) – Not available with the <strong>Sprint</strong><br />
Basic Plan.<br />
• Don’t forget that your per-pay-period premiums for some benefits are based on<br />
your:<br />
- Tobacco/non-tobacco user status (affects Medical/Prescription Drug and Life<br />
Insurance premiums)<br />
- Completion of a Health Assessment at sprintalive.com by a specific<br />
deadline (affects Medical/Prescription Drug premiums)<br />
- <strong>Benefits</strong> Eligible Earnings: This is your annual base pay plus any targeted<br />
incentives or commissions for which you are eligible.<br />
- Age as of Jan. 1, <strong>2013</strong> (affects Life Insurance and Long-Term Disability)<br />
• Estimate how much Life and Accidental Death & Dismemberment Insurance<br />
you and your eligible dependents might need.<br />
• Life Insurance:<br />
- Coverage amount: $10,000<br />
- Coverage level: Employee only<br />
- No Non-smoker Discount<br />
Newly hired/re-hired employees will also receive information regarding enrolling in<br />
<strong>Sprint</strong>’s Voluntary <strong>Benefits</strong> programs. Voluntary <strong>Benefits</strong> are supplemental benefits<br />
programs; these plans require separate enrollment from the New Hire enrollment<br />
described above, but some do have limited enrollment windows. For more<br />
