The Serious Occurrence Summaries began
in 1992. In addition to those identified by
Mr. Rosenfeld (CR020966, CR020924,
CR176709, HRCE030243,
HRCE002188), the following summaries
have been located:

The Codes of Standards refer to the nine
areas identified in question 3061. In 1971
the Hospital Management Services
Branch of the Mental Health Division,
Ministry of Health, developed and
implemented the codes of standards with
the approval of the Mental Health
Division. The codes of standards were
based on existing staff levels, well
established in other ministry of Health
facilities, and adopted by qualified
consultants with many years of field
experienced who were identified with the
Hospital Management Services Branch,
Ministry of Health. These were then
transferred and adapted to use in Schedule
I facilities for the Mentally Retarded. The
staffing standards appear to be initially
