<strong>FREE</strong> AND REDUCED-PRICE SCHOOL MEALS POLICY<br />
Lancaster City Schools today announced its school year 2009-2010 policy for free and reduced-price meals for families unable to<br />
pay the full price of meals served to their children in the National School Lunch, School Breakfast and After School Care Snack<br />
Programs. Local school officials have adopted the following in<strong>com</strong>e eligibility scale for free and reduced-price meals.<br />

Children from households with in<strong>com</strong>e at or below the levels shown above may be eligible for free or reduced-price<br />
meals. Schools will distribute applications and the lunch and breakfast menus to parents/guardians of all Lancaster<br />
City school children at the beginning of the school year.<br />
To apply for free or reduced-price meals, parents/guardians must fill out an application and return it to school. Additional<br />
applications are available at the office and the cafeteria in each school. Households that currently receive<br />
SNAP* (formerly the Food Stamp Program) or OWF funds for a child must provide the child’s name, the SNAP*<br />
or OWF case number and signature of an adult household member on the application. All other households must<br />
provide the following information: the names of all household members, the amount and source of in<strong>com</strong>e received<br />
by each household member in the last month, the signature of an adult and their social security number (or state<br />
"none" if they do not have one). A <strong>com</strong>plete application is required. In<strong>com</strong>plete applications will result in denied<br />
benefits until the required information is provided.<br />
The information provided on the application is confidential and will only be used for the purpose of determining<br />
eligibility and may be verified at any time during the school year by school or other program officials. To discourage<br />
the possibility of misrepresentation, the application form contains a statement certifying that all information furnished<br />
is true and correct. Applications are being made in connection with the receipt of federal funds. Deliberate<br />
misrepresentation of information may subject the applicant to prosecution under applicable state and federal laws.<br />
Parents/guardians will be notified of the approval or denial of benefits. If a SNAP* or OWF case number is listed,<br />
parents/guardians must inform school officials when they no longer receive SNAP* or OWF funds.<br />
Parents or guardians may apply for free or reduced-price meals for their child(ren) anytime during the school year.<br />
If a household is not currently eligible, but the household size increases or in<strong>com</strong>e decreases because of unemployment<br />
or other reasons, the family may submit a new application. Such changes may make the children of the<br />
family eligible for free or reduced-price meals if the family in<strong>com</strong>e falls at or below the levels shown in the In<strong>com</strong>e<br />
Eligibility Scale in this release.<br />
Applications for free or reduced-price meals may also be approved for foster children, as well as migrant, homeless<br />
or runaway children, regardless of the household's in<strong>com</strong>e. Contact the school principal or the food service office<br />
(687-7350) for more information.<br />
Under the provisions of this policy, applications are reviewed by the food service director to determine eligibility.<br />
If a parent or guardian is dissatisfied with the decision on the application or the result of verification, these actions<br />
may be discussed informally with the food service staff. If a formal appeal is desired, parents/guardians have the<br />
right to a fair hearing. A hearing may be requested either by phone (740-687-7350) or by writing the Food Service<br />
Director at 901 E. Fair Ave, Lancaster, Ohio 43130. A copy of the <strong>com</strong>plete meals benefits policy is available for<br />
review in the Food Service Office.<br />
Free Health Care: Families with children eligible for free or reduced-price school meals may be eligible for free<br />
health care coverage through Healthy Start & Healthy Families programs. These programs include coverage for doctor<br />
visits, immunizations, physicals, prescriptions, dental, vision, mental health, substance abuse and more. Please<br />
call 1-800-324-8680 for more information or to request an application. Information can also be found on the web<br />
at http://jfs.ohio.gov.ohp/bcps/hshf/index.stm. Anyone who has an Ohio Medicaid card is already receiving these<br />
services.<br />
