HOUSING<br />

Description: <strong>Abundant</strong> <strong>Faith</strong> Ministries offers a number of supportive living and housing<br />

facilities to the Springfield community.<br />

The Haven House Project is created to provide both single women and women with<br />

children a peaceful and structured environment that assists in transition from<br />

crisis-dominated lives to lives focused on self-determination and peace. Through this<br />

transitional housing program, <strong>Abundant</strong> <strong>Faith</strong> Supportive Services (AFSS) offers<br />

supportive services (directly or through referrals) like counseling, educational programs,<br />

quality child care, job training, that assist women in removing barriers that hinder them<br />

and their children from reaching their full potential as individuals, as family members, and<br />

participants in the communities in which they reside.<br />

Haven Apartments is an 8-unit apartment complex located at 2501 Empowerment Drive in<br />

Springfield, Illinois. This permanent housing project will serve single homeless disabled<br />

women, disabled women with children and homeless disabled families. <strong>Abundant</strong> <strong>Faith</strong><br />

Supportive Services will assist residents with overcoming individual barriers, which have<br />

hindered them from achieving self-sufficiency and stability.<br />

King's Court is an 18 unit apartment complex located at 2641 East Cook Street in<br />

Springfield, Illinois. What was once a run down, crime infested motel, is now an attractive<br />

housing complex dedicated to providing families with low to moderate incomes an<br />

attractive, stable and safe environment in which to live and raise their families. For more<br />

The Men's Transitional House is a 5 bed facility, located at 716 Livingston in Springfield<br />

and operated by <strong>Abundant</strong> <strong>Faith</strong> <strong>Christian</strong> <strong>Center</strong> to serve homeless men. The target<br />

population is single men ages 21 and up, who can live independently and follow the<br />

shelter rules. Our mission is to assist them in learning <strong>Christian</strong> principles and becoming<br />

productive members of the community. Each man is assisted with developing a<br />

self-sufficiency plan which outlines goals they work toward to help them to overcome<br />

individual challenges, secure an income and obtain permanent housing.<br />

RESIDENTIAL SERVICES<br />

Description: Allendale Association's Residential Treatment Programs and Services<br />

consists of a 148 bed residential treatment program which includes a 100 acre main<br />

campus on Cedar Lake in Lake Villa, IL, a 20-bed facility in Benet Lake, WI (approximately<br />

15 miles north of the main campus), two community based group homes in Waukegan, IL<br />

and a community based group home for girls in Highland Park, IL.<br />

Individual units serve as homes for 8-14 children or adolescents and are designed to<br />

serve a particular population grouped by age, diagnostic profile, and social/emotional<br />

functioning level. These programs serve both male and female children and adolescents<br />

ranging in age from 7-21 who are experiencing a wide variety of problems. The more<br />

common "labels" are emotional/behavioral disability, oppositional defiant disorder,<br />

attention deficit hyperactivity disorder, depression, post traumatic stress disorder, anxiety<br />

disorders, bipolar disorders, pervasive developmental disorders, schizophrenia and other<br />

psychotic disorders.<br />

While each child's course of treatment is individualized, the following treatment<br />

components are available to all in placement, as determined by multidisciplinary<br />

treatment planning:<br />

with 24 hours of on-site supervision for youth ages 17-20 preparing to live on their own.<br />

Program sites are in Zion, IL. The program provides furnishings, case management,<br />

community service linkage, and advocacy.<br />

On Your Own<br />

Description: On Your Own serves youth, ages 14-20, who live in Kankakee or Will<br />

County and meet the following criteria:<br />

-Do not have a place to live;<br />

-Cannot return to the home of their parents or guardian; and,<br />

-Do not have the skills needed to live on their own without their parents or guardian.<br />

Our staff provide a response to the immediate crisis, help stabilize the youth, and work<br />

with the youth to find permanent housing, identify the resources and build the skills<br />

necessary to succeed on their own.<br />

BCH Residential Care<br />

Services<br />

Description: At our Baptist Children’s Home (BCH) campus in Carmi we provide<br />

residential care for children ages 8 to 18 who are experiencing emotional or behavioral<br />

difficulties as well as children who have been victimized by abuse, neglect or<br />

abandonment. Children are housed by gender and live in eight-capacity cottages under<br />

the care of loving, <strong>Christian</strong> BCH houseparents.<br />

Through individual, group and family therapy, children learn to identify and move away<br />

from self-defeating habits and into positive, outwardly focused habits. Therapeutic<br />

recreation is also provided by a full-time recreational therapist.<br />

All children attend a local Baptist church, the public school and are encouraged to<br />

Description: The <strong>Center</strong> for Women in Transition (CWT) is a not-for-profit agency in<br />

Supportive Housing Program<br />

Description: Chestnut Health Systems' Supportive Housing Program partners with HUD<br />

to assist individuals who are homeless find stable permanent living. Core services offered<br />

include:<br />

- Clinical Support - Counseling; assistance with psychiatric appointments; community<br />

support services to increase independent living skills including money management.<br />

- Case Management - Linkage and referral to community resources including<br />

transportation when available; advocacy for client's rights; job searches and assistance<br />

with resumes; apartment inspections by Chestnut maintenance staff; financial assistance<br />

with rent.<br />

Eligibility: Individuals with a mental illness, substance diagnosis, and homelessness<br />

may qualify for the Permanent Supportive Housing Program. This is a HUD funded<br />

program; therefore, rent is 30% of adjusted gross income.<br />

Referral Procedures: Individuals are referred by self-referral, friends, family and social<br />

service agencies. All referrals are placed on a waiting list and a referral form is completed.<br />

When an apartment becomes available, individuals on the waiting list are interviewed in<br />

Description: Children’s Home programs fall into three general categories:<br />

2) Special Education - Educating children who are experiencing behavioral, emotional or<br />

developmental challenges.<br />

Eligibility: Children's Home works with children of all ages, 0-21. The children who live at<br />

Children’s Home receive treatment for severe emotional and/or behavioral problems.<br />

Many of the children have problems as a result of having been abused physically, sexually<br />

and/or emotionally.<br />

Mayors Manor<br />

Description: Permanent housing with supportive services for eligible adults (individuals<br />

who are homeless, have a disability, and/or have low incomes.) Tenants participate in<br />

regular case management sessions to work on goals toward self-sufficiency, receive life<br />

skills training, and are referred to other social services in the community as needed.<br />

Fees: Rent varies by income on select units. All utilities are included in the rent with the<br />

exception of phone and cable.<br />

Transitional Housing Program<br />

Description: Homeless individuals and families are provided with housing for up to two<br />

years while participating in bi-weekly case management services. Applicants must be<br />

working full-time or attending school and working at least part-time. Tenants pay 30% of<br />

their net income and are expected to work on goals toward self-sufficiency. Successful<br />

completion of the program results in increased financial stability and up to $1,000 of paid<br />

rental fees reimbursed to the tenant.<br />

Community Crisis <strong>Center</strong> (Kane & NW Cook Counties)<br />

DOMESTIC VIOLENCE<br />

Description: Services offered include a 24-hour hotline, emergency shelter, on-call<br />

trained volunteer advocates, help obtaining an order of protection, legal advocacy,<br />

individual counseling, children's services, and support groups. CCC's role is to support<br />

the victim, to provide information about resources that will help them move forward, to<br />

advocate for them when systems are blocking their progress, and to provide a safe place<br />

for the victim to stay when home is not safe.<br />

HOMELESS SERVICES<br />

Description: •Emergency shelter for women and their children needing a safe place to<br />

stay -<br />

If you don't have a place to call home, please call us. We have a 40-bed shelter for women<br />

and their children. Everyone deserves to have a safe place to sleep, eat, and plan. A case<br />

manager will work with you while you are here to help you make a service plan for you<br />

and your children.<br />

•This program provides room and board with meals, and 24-hour case management,<br />

counseling, and material assistance to those women and children who are in crisis and<br />

homeless.<br />

TRANSITIONAL HOUSING<br />

Description: Adult and Youth Transitional Living Programs (TLP) - Services available in<br />

the transitional living programs include case management, counseling, mentoring<br />

relationships, client transportation, material assistance, and a rent subsidy.<br />

Youth TLP has a focus on homeless youth, ages 18-20, providing housing assistance for<br />

young women who are transitioning out of the Community Crisis <strong>Center</strong>’s shelter. The<br />

Youth TLP will provide housing assistance for young homeless men if space is available<br />

and eligibility is met. The homeless youth program can provide housing assistance for 1-2<br />

youths per year.<br />

Adult TLP offers housing assistance to women and children ages 18 and over. The<br />

homeless adult program focuses on providing housing assistance to women and children<br />

who are transitioning out of the Community Crisis <strong>Center</strong>’s shelter.<br />

Description: Crosspoint Human Services provides a domestic violence shelter,<br />

are available free of charge. Crosspoint is also part of a statewide network of 30 domestic<br />

violence agencies committed to the prevention and elimination of violence against women<br />

Description: Decatur Manor Healthcare is an Intermediate Care Facility for the<br />

chronically mentally ill. Our facility is licensed for 147 beds. A variety of mental illnesses<br />

are treated in the residential housing setting, including Schizo-Affective Disorder, Major<br />

Depression, Bipolar Disorder, Schizophrenia, as well as those who are dually diagnosed<br />

with a mental illness and substance abuse. The psych-social rehabilitation programs<br />

include anger management, substance abuse groups and medication management.<br />

HOMELESS SERVICES<br />

Description: Fellowship Housing is a two year transitional program that works<br />

exclusively with single moms. We are located in the Hoffman Estates/Schaumburg area. If<br />

a mom is accepted into the program, they would reside in a two bedroom apartment and<br />

pay $550 per month for rent. Fellowship Housing has a strong focus on the financial<br />

aspect of independence and stability, so when a mom is accepted into the program, they<br />

work with a case manager on budgeting and are expected to follow a strict budget.<br />

Eligibility: -Must be over 21<br />

-Currently employed full time (making at least $1400 after taxes)<br />

-Have their own vehicle along with a valid driver's license and insurance<br />

-Have 3 children or less, ages 0-18, and have full custody<br />

-6 months since last domestic violence relationship<br />

-1 year sober if there is a history of drug/alcohol abuse<br />

Description: Transitional Housing Program - FSR works with homeless clients to<br />

holistically address individual barriers that led them to a homeless or near homeless<br />

follow-up to address the causes of homelessness and to prevent recidivism.<br />

Children's Residential Program<br />

Description: The Goldie Floberg <strong>Center</strong>’s Residential Children’s Program serves up to 55<br />

children and young adults with developmental disabilities at our Rockton campus. Our<br />

campus is set in a rural area in Rockton, Illinois, where childdren have 10 acres of fenced<br />

in yard to play in.<br />

The children receive 24-hour support and supervision from our Direct Support<br />

Professionals (DSPs). Our Residential Children’s Program also has on site nursing care.<br />

Physical therapy, psychiatric and behavior management services are also offered. The<br />

children that we serve participate in a wide range of on-site and community activities<br />

developed with their input. Activities include Boy and Girl Scouts, therapeutic horseback<br />

riding, school activities, going to movies, museums, zoos, shopping and a multitude of<br />

other community activities.<br />

Eligibility: Ages 5-21.<br />

We serve children who are ambulatory and non-ambulatory. Our program<br />

focuses on serving children and young adults with a primary diagnosis of Autism and/or<br />

mild, moderate, severe or profound mental retardation. A secondary diagnosis involving<br />

emotional or behavioral issues is also acceptable.<br />

Adult CILA Program<br />

Description: The Goldie Floberg <strong>Center</strong>'s CILA Program (Community Integrated Living<br />

Arrangement) provides a unique opportunity for individuals with developmental<br />

disabilities to experience a natural home setting with the security of 24-hour<br />

supervision/assistance. The CILA program is designed to provide hands on life<br />

experiences for clients in the areas of daily living including shopping, cooking, personal<br />

care, community access, finances, medication and greater independence.<br />

Description: The Congregate Housing Services Program (CHSP) is a federally funded<br />

grant program that has as a goal to prevent the premature institutionalization of persons<br />

with disabilities. Individuals must be able to maintain their apartment with minimal<br />

supports. Residents pay 30% of their income for rent and 20% of their income for the<br />

CHSP participation fees which includes 3 meals a day, 7 days a week.<br />

Description: BHA also administers the Section 8 Housing Assistance Payments Program<br />

for existing housing. This program subsidizes the rent of approximately 650 eligible<br />

participants who live in approved, privately owned housing. This program requires a<br />

Description: ICM provides shelter, support, education and <strong>Christian</strong> counseling to<br />

homeless children, their parents and single women while keeping Biblical truths about<br />

Godly righteousness before our culture, community, and fellow <strong>Christian</strong>s.<br />

The seven main services provided are: safe housing; spiritual guidance; guidance; needs<br />

evaluation; savings program; clothing, household items and furnishings; and community<br />

awareness.<br />

The shelter also offers a <strong>Christian</strong>-based program with 7 levels called Shattered 2 Shalom.<br />

This program can take up to 2 years, depending upon the person/family and their needs.<br />

The approach deals with the total person -- spiritually, emotionally, physically -- and<br />

includes training classes and counseling.<br />

respect the facility's stated guidelines (basic rules for a safe environment, etc.) which are<br />

Description: KCCSI has several rental properties which it maintains in its Affordable<br />

Housing Rental Program for the benefit of families with low-income in Kankakee County.<br />

Each family is assigned a staff person who works with them to make sure they are aware<br />

of services that are available at the agency or throughout the community. Through this<br />

program homeless or formally homeless families are given an opportunity to stabilize<br />

Description: To improve the lives of individuals and to strengthen communities,<br />

Lutheran Social Services of Illinois (LSSI) offers housing opportunities for persons 62<br />

years of age or older, and for individuals who are disabled. The majority of buildings<br />

owned and/or managed by LSSI are newly designed and provide comfortable, safe and<br />

affordable living in a community environment. Each building is unique and has its own<br />

niche in the community where it is located. LSSI currently has 20 affordable housing<br />

developments statewide.<br />

Fees: Rent is generally based on 30 percent of a resident’s monthly income (Social<br />

Security, SSI, pension-wages/salary, interest on assets, etc.). However, a resident’s<br />

expenses (health insurance costs, pharmacy bills and medical expenses) are also taken<br />

TRANSITIONAL HOUSING<br />

Description: Through its Transitional Living Program in Rockford, Lutheran Social<br />

Services of Illinois (LSSI) provides supportive and supervised housing for adolescents<br />

between the ages of 18 and 21 who have complex emotional, mental and behavioral<br />

issues. The program serves clients from northern Illinois communities who are “aging<br />

out” of the child welfare system.<br />

The program, which began in June 2007, provides services and teaches life skills to<br />

young adults to assist them in integrating into the community. The program also assists<br />

them in developing coping strategies and a career plan.<br />

The program is located in a large home in Rockford that can house six youth, who are<br />

supervised by on-site staff. In the future, the program will open a second location that will<br />

provide apartment-like housing, as well as on-site staff supervision. As youth become<br />

more self-sufficient, the program will help them find their own living arrangements and<br />

will provide supportive case management services for them to ensure successful<br />

Description: M.E.R.C.Y. Communities (Mentors, Empowerment and Resource for Change<br />

help to build a new future. Mercy Communities offers homeless and at-risk women along<br />

with their children a safe place to overcome hardships and build a solid foundation for<br />

Community Integrated Living Arrangement (CILA)<br />

Description: This is an option for individuals with a developmental disability, ages 18<br />

and over, needing 24 hour support with activities of daily living. One to eight people share<br />

a home with individual bedrooms and communal living areas. Residents receive<br />

individualized support in all areas of their life. Services concentrate on developing skills<br />

in the areas of economic self-sufficiency, independence, and community integration.<br />

Depending on a person's needs, support ranges from 24 hour to intermittent supports.<br />

Eligibility: Individuals interested in this program must be 18 years or older, be diagnosed<br />

with an intellectual or developmental disability, and have proof of appropriate funding<br />

SHELTER<br />

Description: PADS’ ten emergency church sites are open October 1 through April 30 of<br />

each year. Every night from 7:00 pm to 7:00 am, these sites alternate at participating<br />

McHenry County churches. Each site offers a warm evening meal, a safe place to sleep,<br />

breakfast in the morning, case management and hospitality.<br />

Volunteer Site Managers maintain and oversee the needs of their site, coordinating other<br />

volunteers with meal preparation, laundry, security and clean-up.<br />

All homeless men, women and children are welcome. In addition, PADS now offers four,<br />

year-round sleeping rooms (14 beds) at our Woodstock facility. These rooms are available<br />

for persons with documented proof of residency in McHenry County for the last 30 days<br />

who also meet one of the requirements described below:<br />

- Persons with documentation of physical illness or injury<br />

TRANSITIONAL HOUSING<br />

Description: Open year around, PADS Transitional Program is a transitional shelter for<br />

homeless men, women, and children of McHenry County. The program is located 1/4 mile<br />

West of Hwy 14 on Kishwaukee Valley Road, outside of Woodstock. Its nearly six acres lay<br />

claim to peace and hope. Our transitional home offers second chances. This home can<br />

provide housing for up to 20 individual or family members. Together, our goal is to<br />

provide a “homelike” setting and provide the residents with the life skills that they need to<br />

succeed. These skills include but are not limited to, budgeting, goal planning, job training<br />

and time management. The residents attend individual and group counseling, which helps<br />

the residents realize what caused them to become homeless. Other groups offered are:<br />

anger management, parenting, assertiveness training, decision making, nutrition and<br />

more. The home consists of a community kitchen, living room, dining room, laundry<br />

facilities, and a resource room that contains four Internet access computers used for job<br />

searches and research. There is a men’s dorm that can house up to four single men.<br />

Single women and families have private rooms in the upstairs area. We also have a<br />

handicapped accessible room. The residents take pride in and are responsible for keeping<br />

the home clean and tidy.<br />

The Family Self-Sufficiency (FSS) program is a voluntary program designed to teach<br />

Menard County Housing Authority serves as a HUD Approved Housing Counseling<br />

Agency. We offer pre and post purchase counseling, credit and budget assistance, credit<br />

reports, mortgage delinquency, renter’s assistance, fair housing assistance, Home Equity<br />

Conversion Mortgage (HECM) counseling, loss mitigation and relocation assistance.<br />

Description: MCHA owns and manages 177 units of public housing throughout the<br />

county. Units sizes range from 1-4-bedrooms. Units styles range from high-rise<br />

Prairie Place was established in 2002 and is comprised of 20 rental units located in the<br />

communities of Athens and Petersburg. All units are single-family homes with attached<br />

1-car garage.<br />

County Estates is the newest addition to affordable housing in Menard County. County<br />

Estates is comprised of 68 rental units located in the communities of Petersburg, Athens,<br />

and Greenview. Most of the units are single-family homes with attached 2-car garage.<br />

AUTISM<br />

Description: Sage House is a group home specifically created to provide for the needs of<br />

children with autism. The home offers a supervised environment for children with autism<br />

to learn life skills to transition into the community. The program serves children ages 6 –<br />

21 and utilizes therapeutic techniques to assist clients in obtaining their personal highest<br />

level of independence. It also provides referrals, resources, and linkage services.<br />

HOUSING<br />

Description: Supportive Living enables an adult to live in his or her own apartment, with<br />

support and assistance from the Pioneer <strong>Center</strong> team. Staff help individuals build the<br />

skills they need to live independently, such as budgeting, grocery shopping and<br />

housekeeping. Our team also provides psycho-social support, helping each participant to<br />

cope with daily stresses.<br />

Pioneer <strong>Center</strong>’s Community Homes (or Group Homes) provide a family living<br />

environment, where adults share a home in the community with up to seven peers, under<br />

the supervision of caring staff. Our Community Homes are immaculately maintained and<br />

are renowned for their homey feel and passionate staff support. The residents share in all<br />

aspects of running a Pioneer <strong>Center</strong> group home, such as cooking, cleaning and grocery<br />

shopping. Pioneer <strong>Center</strong> has eight intellectual and developmental disability community<br />

homes nestled in neighborhoods throughout McHenry County.<br />

Description: The Runaway and Homeless Youth Program’s goal is to keep runaway and<br />

at-risk youth safe. We strive to identify the problems a youth and their family are facing<br />

and work towards reunification. This program helps runaways, youth refusing to go<br />

home, youth who have been locked out of their home by parents or guardians, and<br />

families with concerns about their child’s unsafe behaviors. Services provided include:<br />

Transitional Living Program (TLP)<br />

Description: The TLP is for homeless McLean County youth (including homeless youth<br />

who are pregnant or parenting) ages 18-21 who are ready to take on the responsibilities of<br />

learning and using independent living skills. We provide stable and safe housing while<br />

youth learn skills and internalize habits that can end chronic homelessness; reintegrate<br />

youth in to the community; and help youth find and maintain long-term jobs at livable<br />

wages. A maximum of 10 youth can be housed by the program, and additional spaces are<br />

available in the weekly Life Skills classes. Through the 18-month program, youth take on<br />

increasing responsibilities for monthly rent, must maintain employment, must attend Life<br />

Skills, and work with their caseworker to connect to other community services.<br />

Fees: No fee.<br />

Referral Procedures: Must complete an application in person at the Project Oz office. An<br />

Advisory Board will review all applications for placement in available housing, on the<br />

waiting list, or transfer to Street Outreach services.<br />

Youth Initiative Program<br />

Description: 1) 24-hour crisis intervention services to runaway youth ages 10-17 and<br />

their families. Host Homes Program allows placement of youth for 1-2 nights as needed.<br />

Entry to placement is through Oz staff only.<br />

2) Family counseling using "Choice Theory/Reality Therapy." Project Oz is certified by the<br />

Glasser Institute to use this program.<br />

3) Individual counseling services available in community or school settings to youth 10-17<br />

years of age.<br />

Fees: No fee.<br />

Referral Procedures: Primarily through law enforcement, parents, schools, and<br />

Safe Place<br />

Description: In addition to Project Oz, all Bloomington and Normal Fire Stations are Safe<br />

Places. Jesus House is also a Safe Place in Bloomington (located at 724 W. Washington<br />

Street).<br />

How Safe Place Works:<br />

Project Oz<br />

1) A youth in crisis walks into a designated Safe Place location (identified by a Safe Place<br />

sign or decal) and tells the first available employee that they need Safe Place help.<br />

2) The employee will find a quiet, comfortable place for the youth to wait while they call<br />

the local Safe Place contact.<br />

3) The Safe Place contact will call the location back to identify the volunteer or staff<br />

member who will come to meet the young person at the location.<br />

4) Ideally, within 20-30 minutes, the Safe Place volunteer or staff member will arrive to talk<br />

with the youth and transport him or her to the agency for counseling, support, a place to<br />

stay or other resources.<br />

5) Once at the agency, counselors meet with the youth and provide support, resources<br />

and help. They make sure the youth and their families receive the help and professional<br />

referrals they need. (Family members or guardians are called to let them know that their<br />

youth is safe.) The agency helps the youth and the family through a difficult time or crisis<br />

situation through counseling and support services.<br />

Most young people hear about Safe Place during school presentations. Each student is<br />

presented with a Safe Place information card. This card has the local Safe Place phone<br />

number and tells them that Safe Place help is free and confidential. For more details on<br />

Additional Information: Project Oz, in collaboration with the Bloomington Fire<br />

Department, Normal Fire Department and Jesus Coffeehouse, joins many youth service<br />

agencies across the nation in offering a Safe Place Program to their community. Safe<br />

Place provides young people access to immediate help and safety if they find themselves<br />

in a crisis. Young people in our community can access a Project Oz Crisis Worker 24/7 by<br />

walking into any Bloomington or Normal Fire Station or Jesus House and asking for Safe<br />

Place help. Project Oz will respond to youth at all Safe Place locations with crisis<br />

counseling and shelter services when necessary.<br />

Description: Public Housing provides assistance for low-income families, the elderly and<br />

people with disabilities. In Rockford, this program includes a variety of housing<br />

opportunities, including multi-family developments, high-rise towers and scattered site<br />

homes. The Rockford Housing Authority's low-income public housing program includes<br />

four family developments that consist of one-to-six bedroom apartments totaling 669<br />

units. Also included are four high-rise towers, which house elderly residents and<br />

residents with disabilities. These apartments house approximately 1,000 families.<br />

The Rockford Housing Authority has made a commitment to develop housing for persons<br />

with special needs. Under the low-income public housing programs, three developments<br />

were developed to accommodate persons with disabilities.<br />

The Rockford Housing Authority also has a very active scattered site rental program that<br />

consists of single-family and duplex homes located throughout Rockford. The RHA owns<br />

skills training.<br />

Description: The Quincy Salvation Army Emergency Shelter is a 14 bed shelter which<br />

provides temporary housing for homeless families and single men and women. This<br />

shelter is the only one within a 100 mile radius that serves this population.<br />

Eligibility: In order to protect all of our residents, it is required that anyone needing to<br />

stay in the shelter must first go to the Quincy Police Station, located at 8th and Maine,<br />

where they will have their picture taken and be given a yellow transient card to take to The<br />

Salvation Army. Normal intake hours are from 7:00 a.m.- 10:00 p.m.<br />

To ensure the safety and well being of our residents, we will not be able to provide shelter<br />

at our facility for the following:<br />

• Those convicted of sexual crimes.<br />

• Those convicted of violent crimes including child abuse, domestic abuse, assault,<br />

Description: The ARC affords men the opportunity to regain their self-respect and<br />

acquire the life skills needed to take their rightful place in society. Located in Rockford,<br />

The Adult Rehabilitation <strong>Center</strong> (ARC) is a smoke-free facility dedicated to rebuilding<br />

lives. The residence will accommodate up to 72 men. Beneficiaries (program participants)<br />

stay a minimum of six months and may remain up to one year until they are able to<br />

integrate back into a productive life.<br />

Our program is designed to treat the whole person, rather that just a specific problem.<br />

The majority of men who come to this center for assistance are having problems in many<br />

areas of their lives -- social, medical, spiritual, personal and employment. The common<br />

factor for most is that of addiction to drugs and/or alcohol.<br />

Eligibility: Potential male participants must:<br />

- Be between 18 and 65 years of age<br />

- Be willing to admit the need for rehabilitation<br />

Emergency/Transitional Housing Program<br />

Description: The Emergency Housing Program serves homeless families with complex<br />

problems such as addictions, abusive relationships and poor life skills. Housing is<br />

provided in one of Shelter Care's 14 apartments for a maximum of 120 days. Case<br />

managers work with families to develop weekly goals in employment, education, child<br />

services and housing to ensure transition into a more stable lifestyle.<br />

The Transitional Housing Program is an extension of the Emergency Housing Program for<br />

families that have long term needs. Housing is provided in one of 6 apartments for a<br />

maximum of 2 years, however the average stay is 7-9 months. These families continue to<br />

Veteran Families Permanent Supportive Housing (PSH)<br />

Description: Shelter Care Ministries is now accepting applications the Veteran Families<br />

Permanent Supportive Housing (PSH) program. Families selected for PSH will move into a<br />

two or three bedroom house or apartment selected by Shelter Care Ministries, and will be<br />

supported by a Housing Advocate. Rent will be paid each month by Shelter Care, but<br />

program participants hold their own leases. As the lease holder, each resident is<br />

responsible for all utility payments. In addition, by entering PSH, participants are agreeing<br />

to create a program plan, meet with a Housing Advocate regularly, and be accountable for<br />

their choices.<br />

Eligibility: Your family may qualify if you meet the following criteria:<br />

- Family is currently homeless (living in a shelter, doubled up with family or<br />

friends, living outside, current eviction notice, etc.)<br />

- An adult member of the household is a Veteran with an honorable discharge<br />

- Family is comprised of parent(s) and dependent children in his/her custody<br />

- Family has a verifiable monthly income of at least $600.00<br />

- An adult residing in the household can establish both Nicor gas and ComEd electric<br />

services in his/her name, in addition to water and sanitation services through the City of<br />

Rockford and Rock River Water Reclamation District.<br />

Description: The Section 8 housing choice voucher program is the federal government's<br />

major program for assisting very low-income families, the elderly, and the disabled to<br />

afford decent, safe, and sanitary housing in the private market. Since housing assistance<br />

is provided on behalf of the family or individual, participants are able to find their own<br />

housing, including single-family homes, townhouses and apartments. The participant is<br />

free to choose any housing that meets the requirements of the program and is not limited<br />

to units located in subsidized housing projects.<br />

New Horizons<br />

Description: TLS Veterans was founded in 1996 and since 2001 has owned and operated<br />

New Horizons, a center in Hebron, Illinois, for homeless veterans who want a chance to<br />

rebuild their lives.<br />

Veterans from WWII, the Korean War, the Vietnam War, Operation Desert Storm and other<br />

conflicts have participated in our services and have gone on to find employment and<br />

rejoin their families. Graduates are encouraged to enroll in TLS-Aftercare, a program that<br />

offers ongoing support and crisis intervention within the community.<br />

Eligibility: For New Horizons: To participate, an individual must be a veteran and<br />

homeless, defined as follows:<br />

Veteran-a person who served in the active military, naval, or air service and who was<br />

discharged or released under conditions other than dishonorable.<br />

Homeless-anyone who lacks a fixed, regular and adequate nighttime residence. It also<br />

includes veterans whose primary nighttime residence is either a supervised public or<br />

private shelter designed to provide temporary living accommodations; an institution that<br />

provides a temporary residence for individuals intended to be institutionalized; or a<br />

private or public place not designed for, or ordinarily used as, a regular sleeping<br />

accommodation for human beings.<br />

In addition, those seeking enrollment must:<br />

- Be drug and alcohol free for at least 30 days and be able to pass testing.<br />

- Pass a criminal background check. Sex offenders and some violent crimes will not be<br />

eligible.<br />

- Be eligible for VA benefits.<br />

- Be willing to seek employment and meet with TLS employment specialist on a weekly<br />

basis until they are employed.<br />

- Be willing to attend all training programs and group sessions that are provided on TLS<br />

property if not employed.<br />

- Be willing to attend three outside AA meetings and provide documentation of their<br />

attendance if in recovery from addictions.<br />

- Be willing to participate in the decision-making process regarding his/her Individual Care<br />

Plan.<br />

- Agree to searches of person, personal belongings, rooms and vehicles at any time the<br />

staff deems necessary as either routine procedures for cause.<br />

- Be willing to establish a saving plan if he/she has income from any source.<br />

- Be willing to participate in weekly chores if physically able.<br />

- Have completed or have proof of a scheduled appointment for a physical and<br />

psychological evaluation at Captain James A. Lovell Federal Health Care <strong>Center</strong> in North<br />

Chicago.<br />

- Provide proof of a negative TB test within the first 30 days of enrollment.<br />

- Complete an alcohol-substance abuse profile.<br />

- Complete a social history.<br />

Transitional Housing Program<br />

Description: Transitional Housing is a temporary shelter for families who are homeless.<br />

Families who enroll in this program are provided with a variety of supportive services, live<br />

in an apartment, and pay rent based on their income. While in the program, families are<br />

required to work with a caseworker to set goals for themselves and then work towards<br />

achieving and maintaining those goals. Families are required to participate in weekly<br />

workshops and various personal development activities. By the end of the family's stay in<br />

TH, the family should be able to move into private housing and to maintain a stable home<br />

without any assistance.<br />

Eligibility: In order to be eligible for this program, you must have children and be<br />

homeless - and have the documentation to prove that you are homeless. This means you<br />

must meet one of the following criteria:<br />

- Sleeping in an emergency shelter<br />

- Have a legal eviction notice with a set date that you have to be out of your home<br />

Description: Lifespring Ministry is Wayside's long term transitional living program for<br />

house up to 57 women and children and provides the opportunity for families to learn the<br />

necessary skills to live independently.<br />

Description: Winnebago County Housing Authority (WCHA) is committed to providing<br />

safe, decent and affordable housing. WCHA administers Section 8 vouchers and the Hope<br />

IV program.<br />

Housing Choice Voucher Programs: This program is designed to provide low-income<br />

families rent payment assistance and to increase their affordable housing options. While<br />

the rules and regulations for the Housing Choice Voucher Program are determined by the<br />

US Department of Housing and Urban Development (HUD), WCHA works with local<br />

families who are eligible for the programs and currently administers over 400 Housing<br />

Choice Vouchers.<br />

Family Self-Sufficiency Programs (FSS): FSS is a HUD program that encourages<br />

communities to develop local strategies to help families that rely on housing voucher<br />

programs to become economically independent and self-sufficient. WCHA partners with<br />

Homeless Youth Programs<br />

Description: Youth who are under the age of 18 and homeless must reside at the Youth<br />

Service Bureau group home with parental permission unless the youth is an emancipated<br />

minor. Homeless youth between the ages of 18 to 21 reside in subsidized community<br />

apartments.<br />

The HUD Program for homeless youth provides services to adolescents and legally<br />

emancipated minors that are homeless. Through outreach and the Youth Service Bureau's<br />

intake process, homeless youth are provided with transitional shelter. In addition to safe<br />

shelter, the program will address the following:<br />

- Individual counseling<br />

- Drug and alcohol assessments<br />

- Tutoring for GED and High School Diplomas<br />

- Training in obtaining and maintaining employment<br />

- Money management<br />

- Basic housekeeping<br />

- Independent living skills<br />

Eligibility: Homeless youth, ages 16 to 21. Homeless is defined as having no legal<br />

domicile and/or residing on the street or in an unsafe living environment. Youth<br />

participants will be eligible for transitional living services for up to 24 months or until their<br />

21st birthday. Married homeless youth are included.<br />

The HUD program serves youth that are non-wards of the state between the ages of 16 to<br />

18 that are homeless. Youth who meet these criteria include those who have been locked<br />

out of their homes and cannot be reunified with their families.<br />

Comprehensive Community Based Youth Services (CCBYS)<br />

Description: CCBYS program provides 24-hour crisis intervention, individual and family<br />

counseling, outreach, emergency shelter, information and referral, and linkage to other<br />

agencies. Trained counselors provide individuals with face-to-face crisis intervention,<br />

individual, group, and family counseling. Staff responds to crisis situations as the result<br />

of police intervention 24-hours per day, 7 days a week, 365 days per year.<br />

When a crisis or need arises, an emergency shelter is available for youth ages 11 to 17.<br />

The shelter is licensed by the Illinois Department of Children and Family Services and can<br />

provide shelter for up to 10 youth at any one time.<br />

The CCBYS program provides a wide variety of services that are short term, intensive,<br />

home based services, focusing on the family unit. Services provided include:<br />

Safe Place<br />

Description: The Youth Service Bureau (YSB) has organized an outreach effort to benefit<br />

runaway and homeless youth, as well as youth who feel that they are being threatened.<br />

Project Safe Place offers youth in emergencies temporary refuge in businesses and public<br />

buildings. Included are youth who are street kids, homeless, have run away, or feel they<br />

are being threatened. The project is designed to reach youth in the early stages of a crisis<br />

situation. Each "Safe Place" displays a large sign featuring a yellow and black<br />

diamond-shaped "Safe Place" logo. When a child has approached a Safe Place for help,<br />

the YSB sends a pre-assigned volunteer, trained to handle these types of situations, to the<br />

scene within 15 minutes. Once on the scene, the volunteer finds out what the problem is<br />

and acts accordingly. Parents are notified as soon as possible of the whereabouts of their<br />

Grand Apartments Permanent Supportive Housing<br />

Description: Provides permanent, decent, safe and affordable housing to single adult<br />

men and women who would otherwise be homeless.<br />

Eligibility: Single man or woman, homeless or in danger of becoming homeless. Also,<br />

must be 90 days clean and sober.<br />

Documents Needed: Birth certificate, proof of income, proof of residency, State ID or<br />

Driver's License.<br />

Fees: Apartment fees vary.<br />
