Services MENTAL HEALTH Description: The Psychosocial/Rehabilitation Program is designed to address cognitive, mood-state and mental health related disorders that impact each individuals' psychosocial well-being and overall quality of life. Through implementation of a facility-wide interdisciplinary program, awareness, orientation, interactive communication and positive social skill objectives will be targeted. Family Shelter Service Services DOMESTIC VIOLENCE Description: Family Shelter Service is a comprehensive domestic violence program in DuPage County, providing a range of services, support, information and resources to assist victims and their children to overcome obstacles and to become safe. In addition to safe shelter, Family Shelter Service offers individual and group counseling, court advocacy, hotline, children’s services, Spanish language services, and training and prevention. Area Served: Northwest Suburbs. Description: Works to serve the needs of single moms and their children who are homeless or one small crisis away from homelessness. The mission of FHC is to transform the lives of these moms and their children by providing affordable housing and life skills mentoring to guide them from crisis to long-term stability. FHC does not discriminate when selecting women for the program. They work with a diverse population which, at present, includes moms ranging in age from 21 to 45 from various economic, cultural, racial and religious backgrounds. Services HOMELESS SERVICES Description: Fellowship Housing is a two year transitional program that works exclusively with single moms. We are located in the Hoffman Estates/Schaumburg area. If a mom is accepted into the program, they would reside in a two bedroom apartment and pay $550 per month for rent. Fellowship Housing has a strong focus on the financial aspect of independence and stability, so when a mom is accepted into the program, they work with a case manager on budgeting and are expected to follow a strict budget. Eligibility: -Must be over 21 -Currently employed full time (making at least $1400 after taxes) -Have their own vehicle along with a valid driver's license and insurance -Have 3 children or less, ages 0-18, and have full custody -6 months since last domestic violence relationship -1 year sober if there is a history of drug/alcohol abuse Documents Needed: Photo ID, proof of income.
