Documents Needed: Photo ID, proof of income, social security card. Services Transitional Housing Program Description: Transitional Housing is a temporary shelter for families who are homeless. Families who enroll in this program are provided with a variety of supportive services, live in an apartment, and pay rent based on their income. While in the program, families are required to work with a caseworker to set goals for themselves and then work towards achieving and maintaining those goals. Families are required to participate in weekly workshops and various personal development activities. By the end of the family's stay in TH, the family should be able to move into private housing and to maintain a stable home without any assistance. Eligibility: In order to be eligible for this program, you must have children and be homeless - and have the documentation to prove that you are homeless. This means you must meet one of the following criteria: - Sleeping in an emergency shelter - Have a legal eviction notice with a set date that you have to be out of your home - Sleeping in a place not meant for human habitation (car, park, abandoned building...) - A victim of domestic violence - Being discharged from an institution where you have resided more than 30 consecutive days Turning Point, Inc.
