Transitional Housing & Affordable Housing<br />

Description: Home of the Sparrow provides Hope, Opportunity and Support that<br />

empowers women and children as they journey from homelessness to self-sufficiency.<br />

Through engaged program services, transitional apartments, transitional shelters and an<br />

affordable housing program, HOS serves over 165 women and children every year.<br />

Eligibility: Homeless woman and children; formerly homeless woman and children.<br />

Maximum Income Guidelines: 300% (depending on program)<br />

Documents Needed: Proof of income, proof of residency, Social Security card, and<br />

other documentation, depending on program.<br />

Fees: Vary; private pay accepted.<br />

Referral Procedures: Potential clients may contact the agency directly or may be<br />

referred by another provider.<br />

SHELTER<br />

Description: Emergency Shelter Services offer safe shelter, food, clothing, case<br />

management, and ministry support to individuals and families in crisis. Serves men,<br />

women and children.<br />

Documents Needed: Clearance Card from the Bloomington Police Department and a<br />

valid, current photo ID.<br />

Fees: None.<br />

Referral Procedures: Residential screening appointments are held weekdays between<br />

9:30-11:00 am. Appointments are reserved on a first come first served basis. Screening<br />

appointments may be reserved in person or over the phone by the individual(s) in need<br />

and/or by a referral agency. Emergency assessments are conducted after regular<br />

business hours and on the weekends as needed.<br />
