DEVELOPMENTAL DISABILITIES<br />
Description: Mosaic provides residential supports for people with developmental<br />
disabilities.<br />
Eligibility: Must meet eligibility for Medicaid Waiver Services for Adults with<br />
Developmental Disabilities.<br />
Documents Needed: Birth certificate, Social Security number, and ISC (Independent<br />
Service Coordination) Screening.<br />
Fees: Funding through Illinois DHS; private pay also accepted.<br />
Referral Procedures: Mosaic must receive a referral from one of the Illinois Independent<br />
Service Coordinator entities if funding is needed. Private pay clients may call direct.<br />
