Rockford MELD<br />
Area Served: Primarily Winnebago and Boone counties, however there are no<br />
geographic restrictions.<br />
Eligibility: Young mothers must be under the age of 21 but over the age of seventeen.<br />
Minors must be emancipated or receive approval from the local crisis program. Mothers<br />
must be pregnant and/or parenting and have custody of their child. Family must be<br />
homeless or at imminent risk of becoming homeless.<br />
Maximum Income Guidelines: 50% below federal poverty level.<br />
Documents Needed: Birth certificate, proof of income, Social Security card, proof of<br />
pregnancy, homeless verification, consents.<br />
Referral Procedures: Referrals are taken by phone only (815)963-3369 x17. If shelter is<br />
full, ask about waiting list.<br />
Transitional Living Program<br />
Description: The Transitional Living Program (TLP) provides housing for young mothers<br />
and their children. The program consists of 22 apartments for independent living. Clients<br />
may remain in the program for 2 years or to the age of 22. TLP residents receive access to<br />
all MELD resources and programming including case management services and life/job<br />
skills training.<br />
Eligibility: - Young mothers between the ages of 17 and 21 and their children<br />
- Pregnant and/or parenting<br />
- Homeless or at imminent risk of becoming homeless<br />
Maximum Income Guidelines: 50% below federal poverty level.<br />
Documents Needed: Birth certificate, proof of income, Social Security card, verification<br />
of homelessness; consents.<br />
Fees: Sliding scale.<br />
