This full time equivalent (FTE) training post provides an ample opportunity to gain experience<br />
in managing acutely unwell mental health working age clients. The post is primarily based in<br />
Glenbourne Unit and has close liaison work with Home Treatment Team as a part of gate<br />
keeping admissions to this inpatient unit. This post is under Inner City Locality of<br />
HTT/Glenbourne Unit. The inner city locality caters to people living and registered with GP<br />
practices within inner city catchment area and works in close association with 2 Community<br />
Mental Health Teams (<strong>South</strong> East-Tamarside & <strong>South</strong> <strong>West</strong>- Waterfront localities). The<br />
populace of nearly 98,000 people are covered by 14 GP practices in this catchment area. As<br />
this area is deprived, the patients present with more complex psychosocial needs in addition<br />
to mental health needs and often carry dual diagnoses (with comorbid substance use,<br />
personality disturbances, learning difficulties and forensic involvements). The Integrated Care<br />
Pathways are being developed in lines with NICE & RCPsych Guidelines to manage these<br />
complex conditions. The inpatients are managed as voluntary or detained under Mental<br />
Health Act. Mostly admissions to Glenbourne are gatekept by HTT and discharged through<br />
the same route under newly developed Nurse Led Discharge Policy & facilitated by Satellite<br />
Service of HTT.<br />
The secondary mental health services are being redefined currently under QIPP & TCS<br />
agendas and apex services like Glenbourne & HTT are at the forefront of leading some<br />
quality improvement changes. The post holder would gain a greater understanding of these<br />
reforms and would provide a substantial support to teams working here. The post holder can<br />
accompany educational supervisor in some of these administrative meetings e.g., Consultant<br />
Psychiatrists Meetings on alternate Mondays and Glenbourne Development Meetings on<br />
once a month Wednesday mornings.<br />
The clinical responsibilities include assessment of new admissions to Glenbourne (Inner City<br />
Clients), reviewing them on regular basis, liaise with GPs, carers or key workers, discharge<br />
planning and leading weekly multidisciplinary ward rounds and community clinical reviews<br />
each under direct supervision of trainer. Additional skills in preparing reports and representing<br />
RC in Mental Health Tribunal Reviews as needed. Sec. 12 (2) approval or training for the<br />
same would be required.<br />
The post holder would be directly responsible to Dr. Naresh K. Buttan, Lead Consultant HTT<br />
& Glenbourne Unit. Trainee would also have opportunities to interact with Dr. Mike Cooper,<br />
Consultant Psychiatrist covering outer city division of Glenbourne & HTT. The clinical<br />
supervision during day time would be available from Dr. Buttan, Educational Supervisor (in<br />
addition to protected 1 hr weekly session on Tuesday mornings) and his team at Glenbourne<br />
& HTT.<br />
The Glenbourne Unit has Foundation Year 2 Trainees posted for 4 months in each wards (Harford &<br />
Bridford). The post holder would be expected to provide work based supervision to FY2 Dr. in Harford<br />
