response to the needs of clients who have<br />

and provide up to date <strong>in</strong>formation on the<br />

range of falls prevention services <strong>in</strong> our area.<br />

About <strong>SA</strong>LHN<br />

<strong>SA</strong>LHN provides care for around 340,000<br />

people liv<strong>in</strong>g <strong>in</strong> the southern metropolitan<br />

area of <strong>Adelaide</strong>. Our 7,000 skilled staff<br />

provide high-quality patient care, education,<br />

research and health promot<strong>in</strong>g services.<br />

Primary and Sub-Acute Care<br />

Services<br />

The Primary and Sub-Acute Care Division<br />

is responsible for the delivery of a range<br />

of primary health care services across the<br />

southern metropolitan area, with a focus on<br />

provid<strong>in</strong>g preventative, health promot<strong>in</strong>g<br />

programs and support<strong>in</strong>g people <strong>in</strong><br />

ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g their health, especially as they<br />

return to the community after a hospital stay.<br />

The Importance of <strong>Falls</strong> <strong>Prevention</strong><br />

One third of people aged more than 65<br />

years fall each year, with 10 percent of<br />

these falls result<strong>in</strong>g <strong>in</strong> <strong>in</strong>jury. This can result<br />

<strong>in</strong> the <strong>in</strong>dividual’s loss of <strong>in</strong>dependence<br />

from an <strong>in</strong>jury or fear of fall<strong>in</strong>g. <strong>Falls</strong> have a<br />

major impact on the health and wellness of<br />

our community.<br />

There are a number of factors that contribute<br />

to an <strong>in</strong>dividual’s risk for falls <strong>in</strong>clud<strong>in</strong>g<br />

issues with balance, vision and changes<br />

<strong>in</strong> health. A multidiscipl<strong>in</strong>ary approach to<br />

address the contribut<strong>in</strong>g factors has been<br />

shown to reduce the risk of fall<strong>in</strong>g. Systems<br />

that support <strong>in</strong>dividuals to connect to local<br />

services improve access and uptake.<br />

The <strong>Southern</strong> Community <strong>Falls</strong><br />

<strong>Prevention</strong> Team<br />

The <strong>Southern</strong> Community <strong>Falls</strong> <strong>Prevention</strong><br />

Team from <strong>Health</strong> Care Transitions offers<br />

coord<strong>in</strong>ation of falls prevention services<br />

<strong>in</strong> <strong>Southern</strong> <strong>Adelaide</strong>, <strong>in</strong>clud<strong>in</strong>g referrals<br />

to the RGH <strong>Falls</strong> Assessment Cl<strong>in</strong>ic. This<br />

common entry po<strong>in</strong>t provides triag<strong>in</strong>g<br />

and screen<strong>in</strong>g of falls prevention referrals<br />

to identify level of risk and urgency<br />

of response required, and to ensure<br />

correct service match<strong>in</strong>g. Some of the<br />

service options available <strong>in</strong>clude RGH<br />

<strong>Falls</strong> Assessment Cl<strong>in</strong>ic (if required), In<br />

Home <strong>Falls</strong> Risk Assessments as part of<br />

the Pathways To Independence Program,<br />

l<strong>in</strong>kage with community falls prevention<br />

services, and follow up and monitor<strong>in</strong>g.<br />

The <strong>Southern</strong> Community <strong>Falls</strong> <strong>Prevention</strong><br />

Team also coord<strong>in</strong>ates the <strong>Southern</strong> Area<br />

<strong>Falls</strong> <strong>Prevention</strong> <strong>Network</strong>, <strong>in</strong>creas<strong>in</strong>g<br />

regional capacity and consistency <strong>in</strong> falls<br />

service provision through education and<br />

jo<strong>in</strong>t <strong>in</strong>itiatives.<br />

<strong>Falls</strong> Risk Assessments for older community dwell<strong>in</strong>g clients <strong>in</strong> the<br />

Day Therapy Centres FAQs<br />

What are Day Therapy Centres<br />

(DTCs)?<br />

Day Therapy Centres are Commonwealthfunded<br />

facilities that provide services such as<br />

physiotherapy, occupational therapy, speech<br />

therapy, social work and podiatry to aged<br />

people who are liv<strong>in</strong>g <strong>in</strong> the community.<br />

Services offered vary at each site.<br />

DTCs aim to provide:<br />

> rehabilitation<br />

> ma<strong>in</strong>tenance of function, <strong>in</strong>dependence,<br />

maximisation of skills, and cop<strong>in</strong>g<br />

> prevention through health promotion and<br />

<strong>in</strong>formation.<br />

DTCs no longer offer full-day programs,<br />

respite, meals, equipment or social activities.<br />

Cost<br />

Clients are required to pay a small fee. Fees<br />

vary at each site. There are weekly caps<br />

on the amount that can be charged when<br />

multiple services are accessed. Most DTCs do<br />

not provide transport.<br />

Who is eligible?<br />

Day Therapy Centres are suited to <strong>in</strong>dividuals<br />

who are:<br />

> over 60 years of age (Aborig<strong>in</strong>al and Torres<br />

Strait Islander people over 45 years of age)<br />

- some exceptions to this are possible<br />

> liv<strong>in</strong>g <strong>in</strong> the community or low-level care<br />

> mobile <strong>in</strong> a wheelchair over a short<br />

distance, or may require light assistance<br />

> able to manage personal care, may need<br />

light assistance or aids for transfers on/off<br />

toilet<br />

> frail, <strong>in</strong> functional decl<strong>in</strong>e, or have lost skills<br />

or confidence<br />

> at risk of fall<strong>in</strong>g, due to a previous fall,<br />

poor balance, unsafe home environment,<br />

or pa<strong>in</strong>ful feet<br />

> learn<strong>in</strong>g to manage a chronic condition,<br />

<strong>in</strong>clud<strong>in</strong>g evaluat<strong>in</strong>g their ability to selfmanage<br />

> experienc<strong>in</strong>g changes or losses <strong>in</strong><br />

cognitive, psychological or social abilities/<br />

needs.<br />

Private Hospital Day Therapy Options<br />

Griffith Rehabilitation Hospital Day Rehabilitation Service<br />

Griffith Rehabilitation Hospital is a 55-bed specialised private rehabilitation hospital <strong>in</strong><br />

metropolitan <strong>Adelaide</strong>, offer<strong>in</strong>g effective and <strong>in</strong>dividually tailored programs with the objective of<br />

maximis<strong>in</strong>g recovery and achiev<strong>in</strong>g a return to home, work or that favourite sport or pastime.<br />

The Day Rehabilitation Program offers a comb<strong>in</strong>ation of Day Rehabilitation Programmes and<br />

hydrotherapy group sessions. The Day Rehabilitation Programmes are available to patients with<br />

a wide range of different conditions. These are group based sessions conducted <strong>in</strong> the hospital’s<br />

comprehensively equipped treatment gyms and hydrotherapy pool. All patients are overseen <strong>in</strong><br />

their programme by a Rehabilitation Specialist.<br />

Who is eligible?<br />

Outpatients, previous <strong>in</strong>patients, clients liv<strong>in</strong>g <strong>in</strong> the community who have private cover<br />

DVA, Work cover and third party clients are also eligible.<br />

Cost<br />

The full cost of rehabilitation is usually covered by the private health fund, DVA, Work cover and<br />

third party.<br />

Strength for Life Programs<br />

What is the Strength for Life Program?<br />

Developed by COTA Seniors Voice the Strength for Life Program runs at multiple gyms and<br />

community centres and provides:<br />

> progressive strength tra<strong>in</strong><strong>in</strong>g sessions, supervised by a qualified fitness <strong>in</strong>structor<br />

> m<strong>in</strong>imum of two sessions per week, at allocated times<br />

> assessments for all clients by an appropriate fitness professional (<strong>in</strong> some cases an exercise<br />

physiologist) and development of a personalised program<br />

> an opportunity for participants to socialise before and after exercise sessions<br />

> an accessible and safe environment for people aged 50 years and over<br />

> Please note: transport is not provided.<br />

> Muscle strength can be rega<strong>in</strong>ed and ma<strong>in</strong>ta<strong>in</strong>ed no matter what the age and physical<br />

condition of the participant.<br />

Department of Veterans’ Affairs HomeFront program<br />

What is the Department of Veterans’ Affairs (DVA) HomeFront<br />

program?<br />

Eligible veterans are entitled to a free annual HomeFront assessment, usually by a qualified<br />

occupational therapist, who will identify fall and accident hazards <strong>in</strong> and around the home.<br />

Follow<strong>in</strong>g the home assessment, and with the veterans consent, recommended safety<br />

modifications can be made <strong>in</strong> the home by qualified trades people. The assessor will also provide<br />

<strong>in</strong>formation concern<strong>in</strong>g other Community and Government services that may be of assistance.<br />

However, HomeFront does not provide assistance with garden ma<strong>in</strong>tenance, car services, or any<br />

aspect of home security.<br />

Cost<br />

DVA contributes a monetary subsidy once every twelve months toward the cost of those m<strong>in</strong>or<br />

house modifications recommended by the HomeFront assessor. Once the subsidy has been<br />

reached the veteran is then required to pay the difference <strong>in</strong> cost.<br />

Who is eligible?<br />

All Department of Veterans’ Affairs Gold and White card holders are eligible.<br />
