Appropriate care planning either initiated or reviewed within both 'victim' and perpetrator's notes so as to minimise risk of re-occurrence. As is current practice, staff continue to inform next of kin/care(s) of all assaults and untoward incidents. Nursing staff discuss with multidisciplinary team immediately or at earliest opportunity as deemed necessary having assessed the situation.
