Notes on Special Preparation of AF FOl'DI 112 for VFOB Reports

® Prep,U'inp; units should pl ace t heir UFOB report de 'ignat i on in the
upper half of the box, i n order t o allow space f or a Hq log number.
® For purposes of UFOB report i ng, the "Ar ea Reported On" will be
taken to be the location (city and state) of t he s i ghting.
0) "Date of Information" will be the d:ite of the initia.l sighting
of the UFOB by the source treated in Part One.

® "fleferences " ltill i nclude the preliminary report citation, any
Hq messagaa assic:ning the sight1ng for investigation, and ,'lilY

The first tm> or three "Parts" will be assigned to the sources
who actually witnessed the sighting and will contain the name,
address, occupation end ot her pertinent data about the source.
Following the "Parts" assir-ned to th.e sources will be shown the
"Parts" assil,'Iled for the SupplementBfY Investigative Efforts and
the Conunents of the Preparing Officer.

@ The' enclosures are listed with the "Parts" to which they belong.
If an inclosure is made on a form 112-Part II, it takes a page
number l ike any other pa[';e of the Report; if it cannot be
plaoed on a form 112-Part II, it does not carry a page number
of the Report.
The "Part" number will be carried in the box for p age numbering,
just above t he page cumbers
The "Suppl ementary Inveati gat:lve ~forts" section Will include a
factual aCCollnt of t he results of your cOltbact.e 1d.th all persons
and agencies other than the sources who actu.all;y witness the
3ir;ht ing. In thie section will also be incl uded allY opinions
about the Si&llting entertain.e by any of these other persons
