INTRAWEST 401(K) RETIREMENT PLAN<br />
Intrawest is pleased to offer eligible employees a tax<br />
deferred savings plan to all employees with a<br />
minimum age of 21. Eligible staff may enroll into<br />
this tax deferred savings plan anytime; deductions<br />
will begin the first day of the quarter after your<br />
enrollment (January 1, April 1, July 1, and October<br />
1).<br />
In order to be eligible to receive employer-matching<br />
contributions, participants must be at least 21 years of<br />
age, and have worked for the Company for a<br />
minimum of 12 months during which they worked at<br />
least 1,000 hours. For more information about this<br />
Plan contact the Benefits Office.<br />
LIFE INSURANCE<br />
Intrawest provides Group Life Insurance and<br />
Accidental Death and Dismemberment insurance to<br />
Core and Regular staff. The current benefit amount<br />
is 1 x annual salary and is provided at no charge to<br />
eligible staff. Eligible employees may also purchase<br />
additional life insurance for themselves as well as<br />
purchase coverage for eligible dependents. Plan<br />
details and rates are available from the Benefits<br />
Office.<br />
WORKERS’ COMPENSATION<br />
SSRC’s Workers' Compensation program is separate<br />
from group health insurance benefits. This is a<br />
company-paid insurance program that protects paid<br />
employees in the event of work-related injuries.<br />
Subject to provisions of the workers’ compensation<br />
laws, this program ensures that: all employees will<br />
receive prompt, proper and complete medical<br />
treatment for work-related injuries or illnesses; wage<br />
or salary replacement; and vocational rehabilitation<br />
when necessary. It is our goal to ensure that this<br />
program is administered correctly and fairly.<br />
Every work-related injury, no matter how minor,<br />
must be reported immediately to your supervisor who<br />
will help you complete a Supervisor Notification of<br />
Injury Form. When this form has been completed,<br />
your supervisor will then send via e-mail the<br />
completed form to the Safety Manager and the Safety<br />
Specialist. Your supervisor will then print out a hard<br />
copy to obtain signatures of the injured employee, as<br />
well as that employee’s respective Supervisor and/or<br />
Manager and respective Vice President. The injured<br />
employee must also sign the Designated Providers<br />
Acknowledgement at this time. Once all signatures<br />
have been obtained, the supervisor is to deliver it to<br />
the Safety Specialist located in Human Resources; if<br />
the injured employee is medically unable to sign at<br />
that time, the supervisor should deliver a photocopy<br />
to the Safety Specialist, and retain the original until<br />
the signature can be obtained. If the supervisor is not<br />
available, the employee should contact the Safety<br />
Specialist directly at extension 5143, or at the Safety<br />
Specialist office located on the 3 rd floor of the<br />
Gondola Building. In order to avoid penalties for<br />
employees or the Company under Colorado law, we<br />
require that injuries be reported within 24 hours after<br />
the incident occurred.<br />
Once your claim has been submitted to the insurance<br />
company, Risk Management will provide your<br />
supervisor with a claim number, contact information<br />
for your claim adjuster and pharmacy information.<br />
Please make sure you communicate with your<br />
supervisor to get these documents.<br />
Designated Providers for Work-Related Injuries<br />
<strong>Steamboat</strong> Ski & Resort Corporation’s workers’<br />
compensation insurance carrier has selected<br />
designated providers for treatment of work-related<br />
injuries as follows:<br />
During the winter operating season from December 8<br />
through April 6, 10am-4pm, injured employees must<br />
be treated initially at the YampaWorks Occupational<br />
Health Center Onsite Clinic located in the Ski Patrol<br />
Transfer Center in One <strong>Steamboat</strong> Place (or the<br />
Emergency Room at Yampa Valley Medical Center<br />
in the event of a true emergency.<br />
During months when YampaWorks at Fetcher Base is<br />
not open, injured employees have the choice of<br />
provider at YampaWorks Occupational Health Center<br />
located at 3001 South Lincoln Avenue, Suite A, or<br />
<strong>Steamboat</strong> Medical Group in Pine Grove Plaza (or<br />
the Emergency Room at Yampa Valley Medical<br />
Center in the event of a true emergency, or if<br />
<strong>Steamboat</strong> Medical Group or YampaWorks is not<br />
open and treatment cannot wait until the next day).<br />
In the event that the Emergency Room is the first<br />
point of contact, the injured employee must followup<br />
with the appropriate designated provider as soon<br />
as practicable to establish work restrictions and a<br />
treatment plan.<br />
Treatment beyond the initial evaluation will be at a<br />
provider by referral from YampaWorks or <strong>Steamboat</strong><br />
Medical Group. Treatment by unauthorized medical<br />
providers may not be covered; and the employee<br />
might be personally responsible for the cost. This<br />
