this tax deferred savings plan anytime; deductions
will begin the first day of the quarter after your
1).
In order to be eligible to receive employer-matching
contributions, participants must be at least 21 years of
age, and have worked for the Company for a
minimum of 12 months during which they worked at
least 1,000 hours. For more information about this
Plan contact the Benefits Office.
LIFE INSURANCE
Intrawest provides Group Life Insurance and
Accidental Death and Dismemberment insurance to
Core and Regular staff. The current benefit amount
is 1 x annual salary and is provided at no charge to
eligible staff. Eligible employees may also purchase
additional life insurance for themselves as well as
purchase coverage for eligible dependents. Plan
details and rates are available from the Benefits
Office.
WORKERS’ COMPENSATION
SSRC’s Workers' Compensation program is separate
from group health insurance benefits. This is a
company-paid insurance program that protects paid
employees in the event of work-related injuries.
Subject to provisions of the workers’ compensation
laws, this program ensures that: all employees will
receive prompt, proper and complete medical
treatment for work-related injuries or illnesses; wage
or salary replacement; and vocational rehabilitation
when necessary. It is our goal to ensure that this
program is administered correctly and fairly.
Every work-related injury, no matter how minor,
must be reported immediately to your supervisor who
will help you complete a Supervisor Notification of
Injury Form. When this form has been completed,
your supervisor will then send via e-mail the
completed form to the Safety Manager and the Safety
Specialist. Your supervisor will then print out a hard
copy to obtain signatures of the injured employee, as
well as that employee’s respective Supervisor and/or
Manager and respective Vice President. The injured
employee must also sign the Designated Providers
Acknowledgement at this time. Once all signatures
have been obtained, the supervisor is to deliver it to
the Safety Specialist located in Human Resources; if
the injured employee is medically unable to sign at
that time, the supervisor should deliver a photocopy
to the Safety Specialist, and retain the original until
the signature can be obtained. If the supervisor is not
available, the employee should contact the Safety
Specialist directly at extension 5143, or at the Safety
Specialist office located on the 3 rd floor of the
Gondola Building. In order to avoid penalties for
employees or the Company under Colorado law, we
require that injuries be reported within 24 hours after
the incident occurred.
Once your claim has been submitted to the insurance
company, Risk Management will provide your
supervisor with a claim number, contact information
for your claim adjuster and pharmacy information.
Please make sure you communicate with your
supervisor to get these documents.
Designated Providers for Work-Related Injuries
Steamboat Ski & Resort Corporation’s workers’
compensation insurance carrier has selected
designated providers for treatment of work-related
injuries as follows:
During the winter operating season from December 8
through April 6, 10am-4pm, injured employees must
be treated initially at the YampaWorks Occupational
Health Center Onsite Clinic located in the Ski Patrol
Transfer Center in One Steamboat Place (or the
Emergency Room at Yampa Valley Medical Center
in the event of a true emergency.
During months when YampaWorks at Fetcher Base is
not open, injured employees have the choice of
provider at YampaWorks Occupational Health Center
located at 3001 South Lincoln Avenue, Suite A, or
Steamboat Medical Group in Pine Grove Plaza (or
the Emergency Room at Yampa Valley Medical
Center in the event of a true emergency, or if
Steamboat Medical Group or YampaWorks is not
open and treatment cannot wait until the next day).
In the event that the Emergency Room is the first
point of contact, the injured employee must followup
with the appropriate designated provider as soon
as practicable to establish work restrictions and a
treatment plan.
Treatment beyond the initial evaluation will be at a
provider by referral from YampaWorks or Steamboat
Medical Group. Treatment by unauthorized medical
providers may not be covered; and the employee
might be personally responsible for the cost. This
