‘Eat <strong>Well</strong>, <strong>Do</strong> <strong>Well</strong>’ <strong>is</strong> a rewards <strong>program</strong> that provides on-going rewards for our guests<br />
and helps local schools improve nutrition and fitness <strong>program</strong>s. Members receive $5 for<br />
every 5 v<strong>is</strong>its as well as exclusive offers. Any member can link <strong>the</strong>ir card to an enrolled<br />
local school and a percentage of every dollar <strong>the</strong>y spend will go to <strong>the</strong>ir linked school to<br />
improve nutrition and fitness <strong>program</strong>s. Members can also pre-charge funds to <strong>the</strong>ir<br />
card at silverdiner.com and <strong>the</strong> school will receive 5% of all pre-charged funds.<br />
2. How do I get an EWDW card?<br />
V<strong>is</strong>it <strong>Silver</strong> <strong>Diner</strong> and get a card during your v<strong>is</strong>it, or v<strong>is</strong>it <strong>Silver</strong>diner.com<br />
3. How do I use my card?<br />
Bring your card when you dine with us and present your card to your server.<br />
4. It says ‘$5 every 5 v<strong>is</strong>its within 3 months’; what does that mean?<br />
In order to receive your $5 reward, be sure to v<strong>is</strong>it 5 times within 3 months of <strong>the</strong> first<br />
v<strong>is</strong>it. Once 3 months passes, a v<strong>is</strong>it (or point) will expire, and you will need an extra v<strong>is</strong>it<br />
in order to reach a total of 5.<br />

you can also access your account information, load money onto your card, check your<br />
card balance, and more.<br />
8. <strong>What</strong> happens if I lose my card?<br />
If a card <strong>is</strong> lost or stolen, a replacement card can be <strong>is</strong>sued by calling 1-866-561-0518.<br />
Please note, any stored value on a lost or stolen card cannot be recovered or re-<strong>is</strong>sued.<br />
9. How much money do I have left on my EWDW card?<br />

or you can bring your receipt on your next v<strong>is</strong>it and we can <strong>is</strong>sue your point <strong>the</strong>n. We<br />
cannot give you credit for payment from a previous v<strong>is</strong>it with your school if you forget to<br />
bring your card when you v<strong>is</strong>it.<br />
14. Can my friend use my card?<br />
Anyone age 13 or older may join <strong>the</strong> club. For your protection we ask that you do not<br />
share your card as we cannot be responsible for <strong>the</strong> value on lost or stolen cards.<br />
