The EP<strong>IT</strong>AL is a declaration <strong>of</strong> service transforming health care sector by means <strong>of</strong> disruptive innovation to establish: Patient<br />

empowerment, one point <strong>of</strong> contact for citizens to healthcare by means <strong>of</strong> a call and coordination center, reorganized health services,<br />

coordinated health care based on digitalization, telemedical health services and patient centricity.<br />

The first EP<strong>IT</strong>AL has been established in Copenhagen in a controlled environment and includes general practitioners and health<br />

providers in the municipalities. <strong>e<strong>Health</strong></strong> tools for cross sectorial planning and coordination, disease condition measurement and<br />

interpretation, video conference are being tested and evaluated.<br />

Although mobile devices are already integrated in telehealth solutions, we still need to make better use <strong>of</strong> m<strong>Health</strong> services in their<br />

role as strategic enablers for the transformation <strong>of</strong> health care delivery. Tablet PCs and smartphones <strong>of</strong>fer a unique potential for<br />

activating patients in improving their health and thus support prevention and a healthy lifestyle. The RENEWING HeALTH project<br />

will present three case studies from across Europe, highlighting the synergies <strong>of</strong> health coaching combined with a mobile technology<br />

platform, empowering patients with chronic conditions.<br />

2.0 TM & You, international conferences in Paris. The overview will be followed by two case studies, the use <strong>of</strong> mobile apps at the<br />

pediatric Hospital San Juan de Dios (Barcelona) under the guidance <strong>of</strong> our speaker, Jorge Juan Fernandez and the case <strong>of</strong> smartphone<br />

distance consultation by physicians <strong>of</strong> patients they have never met, presented by speaker, David Doherty founder <strong>of</strong> 3GDoctor<br />

(UK/Ireland). Audience participation will be highly encouraged!<br />
