Discharge<br />
If you or a family member have concerns or<br />
questions about what happens after hospital<br />
discharge, ask to speak to a social worker<br />
or charge nurse. They can make referrals to<br />
aftercare and support programs through the<br />
Community Care Access Centre (CCAC).<br />
<strong>Home</strong> At Last<br />
Seniors who are without support of family and<br />
friends at the time of discharge may be eligible<br />
for the <strong>Home</strong> at Last program. You can request<br />
a referral from the hospital, or call the program<br />
directly for more information. This short term,<br />
free service can help with:<br />
• Driving and/or accompanying the patient home<br />
• Picking up medications and/or groceries<br />
• Preparing small meals<br />
• Providing personal care and/or homemaking<br />
services<br />
• Following up with phone calls and/or visits to<br />
check on the patient’s well being<br />
• Making referrals to other community support<br />
services<br />
