Floating support for people with<br />
mental health needs
About the service<br />
This service provides floating support to people<br />
with mental health needs, in the London Borough<br />
of <strong>Wandsworth</strong>.<br />
We support people<br />
with mental health and<br />
associated needs<br />
including:<br />
• older people<br />
• substance misuse issues<br />
• domestic violence<br />
• HIV and Aids<br />
• physical disability<br />
• learning disability<br />
• young people at risk<br />
• young people leaving care<br />
• offenders and people at risk<br />
of offending.<br />
We offer support with:<br />
• setting up and maintaining a<br />
tenancy<br />
• developing independent<br />
living skills<br />
• welfare benefits and<br />
maximising income<br />
• working towards achieving a<br />
full and healthy lifestyle<br />
• accessing social and<br />
community facilities and<br />
support<br />
• training, education and social<br />
development<br />
• getting back to work<br />
• social responsibility.

How to apply/eligibility criteria<br />
We help people who:<br />
• are aged between 18 and 64<br />
• have mental health support needs<br />
• are living in the London Borough of <strong>Wandsworth</strong>, or have<br />
a local connection<br />
• are willing to engage with the service.<br />
We accept referrals from a wide range of<br />
sources, including:<br />
• <strong>Wandsworth</strong> Community Mental Health Teams.<br />
• <strong>Wandsworth</strong> <strong>Housing</strong> Options and Projects Teams.<br />
• Social Services.<br />
• Health Services.<br />
• Registered Social Landlords.<br />
• Voluntary Sector Organisations.<br />
• Self referrals.<br />
