11.4 Boilers and Pressure Vessels <strong>Repair</strong> and <strong>Alteration</strong> Report, <strong>AB</strong>-40<br />

<strong>AB</strong>SA Form <strong>AB</strong>-40, Boilers and Pressure Vessels <strong>Repair</strong> and <strong>Alteration</strong> Report, shall<br />

be used to document and certify each alteration and each repair. <strong>AB</strong>-40a is a guide<br />

for completing the <strong>AB</strong>-40. These forms are available on <strong>AB</strong>SA s website at<br />

www.absa.ca.<br />

The repair organization is responsible to ensure an <strong>AB</strong>-40 is submitted to <strong>AB</strong>SA, for<br />

every repair or alteration of boilers and pressure vessels performed in Alberta, within<br />

10 days of the completion of the repair or alteration. This report must also be provided<br />

to the owner.<br />

When the item is to be installed in Alberta, but the repair or alteration has been<br />

completed outside of Alberta, the appropriate Canadian provincial form, or National<br />

Board R1 Report of <strong>Repair</strong> or R2 Report of <strong>Alteration</strong>s, must be submitted to <strong>AB</strong>SA.<br />

The owner must retain the <strong>AB</strong>-40 on file for the life of the equipment.<br />

PESR Section 1(1)(k) lists some of the records that must be maintained by the owner,<br />

PESR Section 41 establishes that records must be maintained and Section 36<br />

establishes requirements for notification and provision of equipment records when<br />

there is a change of ownership.<br />

11.5 <strong>Repair</strong> or <strong>Alteration</strong> Nameplate<br />

An alteration nameplate shall be attached adjacent to the original boiler or pressure<br />

vessel manufacturer s stamping whenever the alteration procedure is required to be<br />

submitted to <strong>AB</strong>SA design survey. A sample of the nameplate is shown on the next<br />

page.<br />

<strong>Repair</strong> nameplates are generally not required for items that are installed in Alberta as<br />

this repair history is available through equipment records and the repair and alteration<br />

reports that <strong>AB</strong>SA keeps on file. Owners are also required to maintain current records<br />

for all repairs or alterations of their equipment.<br />

<strong>Repair</strong> nameplates are required for repairs to pressurized cargo transport vessels,<br />

and if required by the NBIC for R stamped repairs. <strong>Repair</strong> nameplates may also be<br />

required by the owner or the repair organization, or when the <strong>AB</strong>SA Inspector<br />

considers this necessary (e.g., for out-of-province equipment, and when there has<br />

been extensive repair work done).<br />
