A coalition of homeless service<br />
providers whose mission is to develop a<br />
shared cooperative continuum of care<br />
through which concerned <strong>Delaware</strong>ans<br />
from all walks of life strive together to<br />
remedy homelessness throughout the<br />
First State. Through the annual point-intime<br />
studies and the DE-HMIS, the HPC<br />
is the main source of homeless data in<br />
<strong>Delaware</strong>.<br />
The HPC also coordinates Continuum of<br />
Care activities, serves as a liaison to<br />
HUD and the <strong>Delaware</strong> Interagency<br />
Council on Homelessness, and manages<br />
the yearly SHP grant process. The<br />
HPC provides data management services<br />
to homeless service providers, as<br />
well as government and nonprofit agencies,<br />
community centers, job centers<br />
and health providers.<br />
Private, nonprofit agency dedicated to<br />
serving families in crisis by providing effective<br />
and responsive programs and<br />
services. Through Emmaus House, we<br />
provide a safe, transitional environment<br />
where families can take the steps necessary<br />
to make positive changes in their<br />
lives.<br />
Nonprofit, community-based enterprise<br />
dedicated to the revitalization of individuals<br />
to create a self-sustaining community<br />
of service. The agency provides<br />
comprehensive programs and services<br />
in areas of substance abuse, transitional<br />
housing and community outreach to foster<br />
independence and self-worth.<br />
National, nonprofit organization, which<br />
has helped local organizations in rural<br />
America build affordable housing since<br />
1971. Headquartered in Washington,<br />
DC, HAC has offices in Georgia, Missouri,<br />
New Mexico and California. HAC<br />
assists public, nonprofit and private organizations<br />
in the development of both<br />
owner-occupied and rental housing programs<br />
that focus on local solutions, empowerment<br />
of the poor, reduced<br />
dependence, and self-help strategies.<br />
The mission is to improve housing conditions<br />
for the rural poor with an emphasis<br />
on the poorest of the poor in most<br />
rural areas.<br />
<strong>Delaware</strong> Homeless Management Information<br />
System (DE-HMIS):<br />
A database application that allows agencies<br />
within the continuum of care to better<br />
provide services to the homeless<br />
population by collecting basic demographic<br />
and other information in a secure<br />
site.<br />
The HMIS is designed to be flexible and<br />
secure to protect client confidentiality.<br />
The HPC provides training, technical assistance,<br />
report creation and ongoing<br />
support for the database. The HPC also<br />
gathers data from the HMIS and the annual<br />
point-in-time study and produces<br />
reports to inform policy making and<br />
community planning.<br />
<strong>Services</strong>: Provides residential shelter,<br />
battered women’s support group, emergency<br />
food closet, transitional housing,<br />
case management, parenting classes<br />
and extended support for those who<br />
move from Emmaus House.<br />
Emmaus House: Emergency shelter for<br />
30 days for families in crisis. Transitional<br />
housing for eight months to one year for<br />
families in crisis.<br />
Long-Term Recovery/Emergency Shelter/Transitional<br />
<strong>Housing</strong>:<br />
Provides transitional housing, HIV/AIDS<br />
counseling and testing, outreach and referral<br />
services, long-term recovery,<br />
emergency shelter and a program for<br />
youth at risk.<br />
