for assistance as well as have some form of insurance
coverage with a pharmacy benefit. Call counselor can
assist in determining eligibility
Patient Advocate Foundation's Co-Pay Relief (CPR) program provides direct financial assistance to qualified patients;
assisting them with medication co-payments that their insurance requires. The program offers personal service to all
patients through the use of CPR call counselors. The call counselor works directly with the patient as well as the provider
of care to obtain necessary medical, insurance and income certification in an expeditious manner.Co-pay relief assist
those diagnosed with: Autoimmune Disorders, Breast Cancer, Colon Cancer, Diabetes, Kidney Cancer, Lung Cancer,
Prostate Cancer, Sarcoma, Malignant Brain Tumors, Head and Neck Cancers, Multiple Myeloma, Pancreatic Cancer,
Myelodysplastic Syndrome (MDS), Osteoporosis, Hepatitis C, Rhumatoid Arthritis, and Pain. Assistance may also be
available to patients who are experiencing secondary issues as a result of cancer treatment.
