Patient Advocate Foundation's Co-Pay Relief (CPR) program provides direct financial assistance to qualified patients;<br />

assisting them with medication co-payments that their insurance requires. The program <strong>of</strong>fers personal service to all<br />

patients through the use <strong>of</strong> CPR call counselors. The call counselor works directly with the patient as well as the provider<br />

<strong>of</strong> care to obtain necessary medical, insurance and income certification in an expeditious manner.Co-pay relief assist<br />

those diagnosed with: Autoimmune Disorders, Breast Cancer, Colon Cancer, Diabetes, Kidney Cancer, Lung Cancer,<br />

Prostate Cancer, Sarcoma, Malignant Brain Tumors, Head and Neck Cancers, Multiple Myeloma, Pancreatic Cancer,<br />

Myelodysplastic Syndrome (MDS), Osteoporosis, Hepatitis C, Rhumatoid Arthritis, and Pain. Assistance may also be<br />

available to patients who are experiencing secondary issues as a result <strong>of</strong> cancer treatment.<br />
