För att konstatera total hjärninfarkt med de direkta kriterierna krävs att hjärnan och reflexer är farmakologiskt och metabolt opåverkade, att kroppstemperaturen är >33 grader Celsius samt att orsaken till den totala hjärninfarkten är känd. Betydelsen av dessa krav och deras innebörd kommer att diskuteras. INFECTIOUS PROBLEMS AND ORGAN FAILURE IN THE ICU SAL 2 13.30-15.00 MULTI-RESISTANT BACTERIA (MRB) IN THE ICU Christina Agvald-Öhman, MD PhD EDIC, Anestesi-och Intensivvårdskliniken Karolinska Universitetssjukhuset Huddinge The increasing resistance among bacteria is a threat worldwide and an especially great problem in the ICUs. This lecture will give an overview of the incidence worldwide and in Sweden with a special focus on the carbapenamases. Transmission of MRB and how to deal with the problem in our own units will also be a subject for this talk. In addition, prevention hygienic measures will be discussed as well as the increasing need for isolation of patients in our ICUs. EPIDEMIOLOGICAL SURVEY OF INVASIVE CANDIDA/YEAST INFECTIONS IN SURGICAL PATIENTS IN INTENSIVE CARE UNITS IN EUROPE: PRELIMINARY RESULTS Lena Klingspor (Karolinska University, Sweden) 1, A. Dóczi (Hungary) Candida species are the fourth most common isolates in nosocomial bloodstream infections and the incidence of candidaemia has been shown to be highest in intensive care units (ICU) and in general surgery units (44.7% and 40.2%, respectively) . The European Confederation of Medical Mycology (ECMM) initiated a prospective survey on deep seated Candida infections in ICU surgical patients. The study started 2006 and ended 2008. Each participating centre had to report the cases to the coordinator of the study, fill out a questionnaire (concerning demographic data, index of severity, risk factors or predisposing diseases, diagnosis of fungal infection, prophylaxis, antifungal treatment and outcome at 30 days after diagnosis),and mail it, to the Coordinator. The aims of the survey are: i) to expand the knowledge on the characteristics of surgical ICU patients with deep seated mycoses and on the diagnostic procedures used, ii) to collect information on the management of the infection and outcome, iii) to determine the distribution of fungal species causing infections in this patient population. A total of 779 surgical patients from 14 countries had invasive candidosis. It was 62.5% males and 37,5 % females . Mean (median) age: was 59 (63) years, (range 3 days-91 years.). Of the 779 patients 401 underwent abdominal surgery,108 thoracic surger ,49 vascular , 47 neuro -surgery, 54 multiple trauma and 108 other surgery. The median rates of candidaemia were 9 per 1000 surgical admissions. Duration of ICU stay until detection of invasive Candida infection was a median of 12 days. Candida albicans was identified in 53 % of the episodes followed by C. parapsilosis (18.2%), C. glabrata (13.6%), C. tropicalis (6%) C.krusei 2.5% and other Candida species (6.7 %). Crude mortality at day 30 in surgical patients in ICU was 38 % . More data and additional results will be presented. TREATMENT OF ACUTE RENAL FAILURE IN SEPSIS Prof. Dr. Detlef Kindgen-Milles, Leiter Interdisziplinäre Operative Intensivstation ZOM-I Klinik für Anästhesiologie, Universitätsklinikum Düsseldorf Severe sepsis and septic shock account for more than 50 % of all cases of acute kidney failure in the ICU. Despite recent advances the mortality in those patients is extraordinarily high. In this lecture it will be discussed whether patients with AKF and sepsis require a specific renal replacement therapy, i.e. higher dialysis doses, different modes of RRT, or newly designed haemofilters targeting at mediator removal. St George’s Healthcare NHS Trust, London, SW17 0QT, UK The human and financial cost of post-operative mortality and morbidity is highly significant, and is potentially avoidable. Patients who suffer from post-operative complications are also at risk of long-term morbidity, however these patients make up only a small proportion of total surgical cases. Identification of patients who are at greater risk of post-operative complications is crucial, as therapeutic measures aimed at this specific group of patients may reduce morbidity and mortality. It is estimated that 12.5% of all surgical procedures account for 80% of post-operative deaths, yet not more than 15% of these patients are admitted to critical care units after surgery. Complications following surgery can be related to a number of factors that include the post-operative physiological derangement, the co-morbid burden and the age of the patient, the type of procedure performed and the urgency in which the procedure was undertaken. Surgery is associated with a systemic inflammatory response and an associated increase in oxygen demand. An increase in oxygen delivery and tissue oxygen extraction may meet this increase in oxygen demand. Failure to do so can result in anaerobic metabolism, cellular dysfunction, and ultimately organ failure. Surgical procedures that cause a significant increase in peri-operative oxygen demand, such as major intra-abdominal surgery, are therefore high-risk. 23 PROGRAM
