Please review the following<br />

checklist to ensure the timely<br />

enrollment of your child in<br />

the <strong>YMCA</strong> SACC Program<br />

ENROLL IN PERSON BEGINNING 7/1/13<br />

Beginning Monday, July 1, at 8:00 a.m.,<br />

members and past SACC participants may<br />

enroll at any <strong>YMCA</strong> location. New community<br />

participants may enroll beginning July 8.<br />

FAXED/MAILED ENROLLMENT FORMS<br />

WILL NOT BE ACCEPTED.<br />

<strong>YMCA</strong> income-based pricing is available.<br />

Partial scholarships are available through the<br />

Strong Kids program, and are based on both<br />

income and need. Applications are available at<br />

any <strong>YMCA</strong> location or online at www.ymcawichita.<br />

org. To apply for income-based pricing you will<br />

need a letter of denial by DCF, federal income<br />

tax return, two most recent paycheck stubs,<br />

government assistance verification and proof of<br />

other assistance. Please allow ten business days<br />

for processing of your application. A written,<br />

authorized third party payment plan must be on file<br />

before your child may enroll in SACC.<br />

NO CHILD WILL BE ENROLLED WITH INCOMPLETE<br />

RECORDS. All pages within the enrollment guide<br />

MUST BE COMPLETED WITH PROPER SIGNATURES<br />

before enrollment will be accepted.<br />

ENROLLMENT will not be accepted without<br />

the proper enrollment fee paid in full.<br />

• All enrollment fees are nonrefundable and<br />

nontransferable.<br />

• Full payment is expected for children ONLY<br />

enrolling in SACC School Day Out Camps.<br />

• Make checks payable to <strong>YMCA</strong>.<br />

discounted Rates are offered as a<br />

courtesy to sacc participants on a ymca<br />

family membership. Weekly rates will be<br />

determined based upon your membership status<br />

as of SACC enrollment. Should you join the <strong>YMCA</strong><br />

after enrollment, but before attending SACC, it is<br />

your responsibility to alert the child care account<br />

manager at 219-9622 ext 5559 prior to the<br />

balance due date, to adjust your fees due.<br />

• I provided complete and accurate<br />

information for the SACC participant,<br />

parents/guardians, employers, physicians<br />

and emergency contacts (full address,<br />

social security numbers, birthdate, e-mail<br />

and phone numbers).<br />

• I specified my child’s school district,<br />

school name, start date, days of<br />

attendance, arrival time & departure<br />

time.<br />

• I read the Terms of Agreement, signed<br />

and dated my child’s registration form.<br />

• I specified my hospital preference for<br />

emergency medical care and listed<br />

immunizations as well as medical<br />

conditions such as allergies, asthma,<br />

others or noted NA (not applicable) for<br />

these items in my child’s Health History.<br />

• I included all applicable health insurance<br />

information (provider name, policy<br />

number, military/medical assistance<br />

program number), date of my child’s<br />

last Tetanus (DPT) shot and necessary<br />

custody information on the Authorization<br />

for Emergency Medical Care.<br />

• I signed and dated Health History<br />

Form CCL358. I signed and dated form<br />

CCL010 (Authorization for Emergency<br />

Medical Care) in front of a witness and<br />

had the witness sign/date the form.<br />

• I have planned additional time in my<br />

schedule on the first day of service to<br />

complete a <strong>YMCA</strong> Payment Agreement<br />

and review my child’s records with the<br />

SACC Site Director. These forms must be<br />

completed for my child to participate in<br />

the program.<br />

Early Learning Centers (2 wks - K eligibility)<br />

The Y offers trusted, quality, licensed child care at<br />

the Andover, East & South <strong>YMCA</strong>s year-round and in<br />

nine area high schools during the school year.<br />

In our Early Learning Centers, every project is<br />

designed to activate your child’s imagination.<br />

We focus on creating a nurturing environment<br />

where making friends, learning, physical activity<br />

and exploring interests are the top priority.<br />

Meanwhile, you have peace of mind that your<br />

kids are safe and in a positive environment where<br />

core values such as caring, honesty, respect and<br />

responsibility are used to reinforce the character<br />

summer day camp<br />

Summer Day Camp (completed K- age16)<br />

At the Y’s summer day camps, every day is an<br />

adventure, filled with fun and shared with friends.<br />

For parents, camps are reliable, affordable and<br />

convenient, with hours to accommodate their<br />

work schedules.<br />

Plus, Y campers discover new things each day as they<br />

build confidence through activities that encourage<br />

personal growth & the development of strong values.<br />

Activities include:<br />

• sports<br />

• games<br />

• friendships • team building<br />

• creative arts • character challenges<br />

• weekly themes focused on <strong>YMCA</strong> core values<br />

Camp enrollment guides are available at Y<br />

locations or ymcawichita.org starting each February.<br />

youth development | early learning<br />

A toddler develops a trusting, positive relationship with caregivers in<br />

her child care program and overcomes her shyness around new people.<br />

This is how the Y helps build a strong foundation for our community.<br />

The Y is the largest provider of child care in the<br />

region and our programs are:<br />

• Licensed by the Kansas Department of Health<br />

& Environment, accredited by the NAEYC and<br />

maintain star ratings by KQRIS.<br />

• Able to accept child care plans from the DCF and<br />

other third party payers.<br />

• Partners with school districts, Kansas<br />

Department of Education, United and other<br />

third party organizations.<br />

• Participants in the USDA’s CACFP program to<br />

provide nutritious meals and snacks daily.<br />

• Staffed with caring professionals trained and<br />

An 11-year-old boy learns the art of paddling<br />

a canoe at summer camp. Five years later he’s<br />

learning about responsibility in his role as a<br />

counselor at the same camp. This is how the Y<br />
