Please review the following
checklist to ensure the timely
enrollment of your child in
the YMCA SACC Program
ENROLL IN PERSON BEGINNING 7/1/13
Beginning Monday, July 1, at 8:00 a.m.,
members and past SACC participants may
enroll at any YMCA location. New community
participants may enroll beginning July 8.
FAXED/MAILED ENROLLMENT FORMS
WILL NOT BE ACCEPTED.
YMCA income-based pricing is available.
Partial scholarships are available through the
Strong Kids program, and are based on both
income and need. Applications are available at
any YMCA location or online at www.ymcawichita.
org. To apply for income-based pricing you will
need a letter of denial by DCF, federal income
tax return, two most recent paycheck stubs,
government assistance verification and proof of
other assistance. Please allow ten business days
for processing of your application. A written,
authorized third party payment plan must be on file
before your child may enroll in SACC.
NO CHILD WILL BE ENROLLED WITH INCOMPLETE
RECORDS. All pages within the enrollment guide
MUST BE COMPLETED WITH PROPER SIGNATURES
before enrollment will be accepted.
ENROLLMENT will not be accepted without
the proper enrollment fee paid in full.
• All enrollment fees are nonrefundable and
nontransferable.
• Full payment is expected for children ONLY
enrolling in SACC School Day Out Camps.
• Make checks payable to YMCA.
discounted Rates are offered as a
courtesy to sacc participants on a ymca
family membership. Weekly rates will be
determined based upon your membership status
as of SACC enrollment. Should you join the YMCA
after enrollment, but before attending SACC, it is
your responsibility to alert the child care account
manager at 219-9622 ext 5559 prior to the
balance due date, to adjust your fees due.
• I provided complete and accurate
information for the SACC participant,
parents/guardians, employers, physicians
and emergency contacts (full address,
social security numbers, birthdate, e-mail
and phone numbers).
• I specified my child’s school district,
school name, start date, days of
attendance, arrival time & departure
time.
• I read the Terms of Agreement, signed
and dated my child’s registration form.
• I specified my hospital preference for
emergency medical care and listed
immunizations as well as medical
conditions such as allergies, asthma,
others or noted NA (not applicable) for
these items in my child’s Health History.
• I included all applicable health insurance
information (provider name, policy
number, military/medical assistance
program number), date of my child’s
last Tetanus (DPT) shot and necessary
custody information on the Authorization
for Emergency Medical Care.
• I signed and dated Health History
Form CCL358. I signed and dated form
CCL010 (Authorization for Emergency
Medical Care) in front of a witness and
had the witness sign/date the form.
• I have planned additional time in my
schedule on the first day of service to
complete a YMCA Payment Agreement
and review my child’s records with the
SACC Site Director. These forms must be
completed for my child to participate in
the program.
Early Learning Centers (2 wks - K eligibility)
The Y offers trusted, quality, licensed child care at
the Andover, East & South YMCAs year-round and in
nine area high schools during the school year.
In our Early Learning Centers, every project is
designed to activate your child’s imagination.
We focus on creating a nurturing environment
where making friends, learning, physical activity
and exploring interests are the top priority.
Meanwhile, you have peace of mind that your
kids are safe and in a positive environment where
core values such as caring, honesty, respect and
responsibility are used to reinforce the character

summer day camp
Summer Day Camp (completed K- age16)
At the Y’s summer day camps, every day is an
adventure, filled with fun and shared with friends.
For parents, camps are reliable, affordable and
convenient, with hours to accommodate their
work schedules.
Plus, Y campers discover new things each day as they
build confidence through activities that encourage
personal growth & the development of strong values.
Activities include:
• sports
• games
• friendships • team building
• creative arts • character challenges
• weekly themes focused on YMCA core values
Camp enrollment guides are available at Y
locations or ymcawichita.org starting each February.

youth development | early learning
A toddler develops a trusting, positive relationship with caregivers in
her child care program and overcomes her shyness around new people.
This is how the Y helps build a strong foundation for our community.
The Y is the largest provider of child care in the
region and our programs are:
• Licensed by the Kansas Department of Health
& Environment, accredited by the NAEYC and
maintain star ratings by KQRIS.
• Able to accept child care plans from the DCF and
other third party payers.
• Partners with school districts, Kansas
Department of Education, United and other
third party organizations.
• Participants in the USDA’s CACFP program to
provide nutritious meals and snacks daily.
• Staffed with caring professionals trained and

learning about responsibility in his role as a
