HEALTH AND LIFE INSURANCE
The Town of Ludlow, through the Board of Selectmen, entered into a contract with Boston Mutual Life Insurance
Company for One more Year and Blue Cross HMO Blue, HMO Care Elect-PPO, HMO Medicare, Managed Blue
for Senior, Medex III and Blue Dental contracts were signed with effective dates running from July 1, 2010 to June
30, 2011. The following rates prevail:
