Accidents. • They happen! • Children <strong>and</strong> adults are injured! • Some of the injuries are quite serious! • Examples are: Broken limbs, fractured skulls, amputations, crush injuries etc. • Why did they happen Can they be prevented • Are all such accidents investigated <strong>and</strong> reported • Because supervision was inadequate. • Because something was faulty. • Because in some cases the board/CCMS or school procedures failed!!!!
