categories of smokers, readily consume<br />

other addictive substances and often<br />

exhibit psychopathology (41).<br />

In such patients, NRT-assisted smoking<br />

re<strong>du</strong>ction would be an ideal preparatory<br />

solution for cessation (19-42) and at<br />

worst, if they are to continue smoking, it<br />

would re<strong>du</strong>ce the toxicity of their<br />

cigarette consumption.<br />

Conclusion<br />

Complete smoking cessation remains a<br />

priority but since most of the toxicity<br />

associated with smoking is not <strong>du</strong>e to<br />

nicotine (the main biological factor<br />

responsible for tobacco dependence) it is<br />

justified to propose smoking re<strong>du</strong>ction<br />

for:<br />

- smokers who have failed in their<br />

quit attempt,<br />

- smokers who cannot or do not<br />

want to quit,<br />

- smokers who do not want to quit<br />

but are willing to re<strong>du</strong>ce their cigarette<br />

consumption,<br />

- finally, smokers who must<br />

temporarily abstain from smoking.<br />

Although the efficacy of such a<br />

concept has been demonstrated in<br />

re<strong>du</strong>cing the number of cigarettes<br />

smoked, increasing the desire to quit, the<br />

number of quit attempts and effective<br />

quitting, re<strong>du</strong>cing the toxicity of smoking<br />

<strong>du</strong>ring the period of smoking re<strong>du</strong>ction<br />

while maintaining the patient’s quality of<br />

life, and finally in facilitating complete<br />

cessation in the future, as has its high<br />

somatic tolerance… the objective of such<br />

interventions remains preparation for<br />

complete smoking cessation; the<br />

in<strong>du</strong>ction of NRT to re<strong>du</strong>ce cigarette<br />

consumption therefore falls under the<br />

scope of prescription and medical<br />
