SNOW MOUNTAIN RANCH BILLING INFORMATION AGREEMENT During your discussion with Group Sales, an agreement is made to the type of contract you will create for your group/family reunion. If you choose the Online Reservation option, each of your attendees will make their own reservation online under your agreed contract and each are responsible for their own payments. If you decide not to choose this option, then you as group leader are responsible for collecting all payments from your attendees and will pay the total balance due at departure. Any changes to the billing plan created for your group must be received by the Conference Office at least THIRTY DAYS prior to arrival. NO CHANGES TO THE BILLING WILL BE MADE AFTER THAT DATE. ALL REUNION CABINS WILL BE BILLED AS A UNIT PRICE AND THE TOTAL CHARGE WILL BE THE RESPONSIBILITY OF THE GROUP LEADER. UNIT PRICE WILL NOT BE SPLIT AMONG INDIVIDUAL GUESTS. Each attendee will be responsible for their room charges and incidentals and will pay the balance due at departure. Along with the room roster, I agree to provide the Conference Office with: 1. Addresses and phone numbers for all attendees. 2. The amount of any booking fees that have been applied to the master reservation that you would like applied to each individual reservation (Booking fees were paid at the time of contract signing). Please list the above on a separate piece of paper and attach to this form. I, the undersigned, agree to the billing terms specified above and understand that no changes will be made within THREE WEEKS of check in or during my stay. I also understand that FULL PAYMENT IS DUE AT CHECK-OUT. Signature: MANDATORY AFFIDAVIT OF TAX STATUS Date: ANY GROUP that makes purchases from the YMCA of the Rockies must complete this affidavit and declare their tax status. An original of this affidavit must be completed, signed and submitted prior to check-in for each visit. We DO NOT qualify as non-taxable under Colorado law or we do not have appropriate documentation to that end. We agree to pay all applicable taxes associated with the charges incurred for goods and services provided by the YMCA of the Rockies (complete items 3 through 9 below). We DO qualify as non-taxable or tax exempt under Colorado law as outlined below. I understand and declare, under penalty of perjury, that the following statements are true: • The name of our tax exempt organization is included (in-part or full) in the name on this reservation. • Any payments for non-taxable goods and services will be made with a check, drawn on the tax exempt organization’s bank account, or with a credit card in name of the tax exempt organization. • The tax exempt organization listed has not and will not receive any reimbursement through direct payment, collection or donation from any individuals or other organization for the use or consumption of said goods and services (complete items 1 through 9 below). 1. FREE ACTIVITIES * The following activities are included in your stay, depending on the season! In winter enjoy ice skating, snow tubing and sledding at our Winter Fun Park. Year-round: basketball, billiards, foosball, indoor pool, indoor roller rink, table tennis, volleyball Spring (weather dependent), Summer and Fall only: bocce ball, croquet, hiking, horseshoes, ladderball, miniature golf, playgrounds, pumptrack, sand volleyball, tennis, tetherball, 18-hole disc golf Winter: ice skating, snow tubing, sledding OTHER ACTIVITIES Self-scheduled, fee required. WINTER ACTIVITIES Nordic Center Our Nordic Center offers more than 100 km of cross-country and snowshoe trails, which can accommodate everyone from the first-time cross-country skier and snowshoer to the elite racer, as well as skiers with special needs. For your convenience we offer cross-country ski and snowshoe rentals on-site. The Nordic Center’s full-service ski shop is where you’ll find deals on group lessons, private lessons, and snowshoes, skate skis, and classic ski rentals. We also have the latest cross-country ski gear and apparel for sale. Please remove this page, complete and then make copies of all forms before mailing. Family Programs Please pick up a copy of our Program Bulletin at check-in. You’ll find information on scheduled activities, musical performances, dancing, wildlife education, family campfires, night hikes and other family fun! Historical Sites The Rowley Homestead lets you experience the way life was as a Rocky Mountain rancher in the late 1800s and early 1900s. Indoor Swimming Pool Lifeguards are on duty during all scheduled pool times. Cameras, camera phones and video equipment are not allowed in the pool area or locker rooms. • 25-yard heated pool and adult lap time • aquatic climbing wall • sauna and showers Library Stop by the library for a great selection of books and games. Wi-Fi and computers are available for use. Trail Running/Walking Snow Mountain Ranch offers miles of beautiful trails for running and walking. * In most cases, children must be accompanied by a responsible adult 18 years or older. May require refundable deposit for equipment rental. Horse Activities Trail rides, wagon rides and sleigh rides, all ages and levels. Advance reservation required. Sombrero Stables: 970-887-1999 Explore our beautiful trails by horseback. Children age 6 and older may ride their own horse. We offer breakfast rides, lunch and dinner rides and wagon or sleigh rides. Helmets available upon request. Pony rides are also available for children ages 5 and under. Mountain Biking Explore our miles of scenic trails summer and fall.
