Primary Care<br />

Telephone Care (also known as TLC or Telephone Liaison Care):<br />

Speak with a nurse, 24 hours a day – ext. 6041<br />

• because you are sick<br />

• have medical problems or questions<br />

• have a question <strong>for</strong> your Primary Care Team<br />

• need test results<br />

• need a prescription renewed because you have no refills left.<br />

Cancelling an Appointment - Please don’t be a “No Show”<br />

Help us to provide timely services. If you cannot keep your appointment, please notify us as soon<br />

as possible so we can schedule another appointment <strong>for</strong> you and use your appointment slot <strong>for</strong><br />

another Veteran. To cancel an appointment, call the scheduling/cancellation line at ext. 5105.<br />

What to Expect at Your First Primary Care Appointment<br />

<strong>Veterans</strong> who elect to obtain their primary care at a <strong>VA</strong> facility will be assigned to a primary care<br />

team. A Primary Care Team is comprised of physicians, physician assistants, nurse practitioners,<br />

nurses, and/or clerks at the <strong>Lebanon</strong> <strong>VA</strong> <strong>Medical</strong> <strong>Center</strong> or a CBOC. The first primary care visit<br />

will be scheduled at your convenience.<br />

During Your First Visit You Will:<br />

• Meet your assigned provider<br />

• Have a physical examination<br />

• Provide a complete medical history<br />

The first primary care visit is an important one. This is the time <strong>for</strong> you to develop a relationship<br />

and get acquainted with members of your primary care team. Please bring along your own<br />

medical records from prior non-<strong>VA</strong> physicians.<br />

Important items to remember <strong>for</strong> each Primary Care visit.<br />

1. Write down everything you want to talk to the provider about since it is hard to remember<br />

questions when you have your appointment.<br />

2. Make a list of your medications or bring them with you so that you and the provider<br />

understand what you are taking.<br />

3. Ask questions about your condition and treatment so that you understand everything<br />

be<strong>for</strong>e you leave.<br />
